Thank you for your Recruitment Et Retention grant application. We are pleased to announce that Weld
County Department of Human Services has been awarded $1,000.
We would like to remind you of the following criteria that apply to your grant award:
• Funded effort/activity must be completed by April 30, 2017.
• Each person attending or participating in the funded effort or activity must complete the
Individual Recruitment or Retention Evaluation Form. Grantees must download the pdf form and
submit all evaluations using the Compilation Form along with the online Summary Form on
or before May 15, 2017.
• Names and email addresses for individuals who participated in an event or activity must be
collected and submitted along with the activity summary form and evaluation forms by the
grantee. Participants may opt -out of providing contact information.
• Photographs of the funded efforts or activity are not required but highly encouraged.
• Failure to submit documents and invoices or enter expenditures into CFMS by the associated
deadlines may impact future grant awards. The deadline is May 5, 2017.
o Please note that you may submit for reimbursement after you have incurred an expense,
even if the grant project is not yet complete. However, you may not submit for
reimbursement prior to incurring the expense.

• CPAs: Please email your invoices to mary.gerlach®state.co.us
• If the grantee has an issue with meeting a deadline, CDHS must be contacted prior to the
deadline to discuss alternative arrangements.
• If the scope of your recruitment and/or retention effort/activity changes significantly, CDHS
must be contacted prior to the deadline to discuss the new plan and get approval for any
changes.
• If you indicated your county or CPA would matching funds or use in -kind donations, you will be expected
to follow through on that obligation as that will be used in next year's grant consideration.
• Late submissions on any part of the grant requirements will be noted and considered in next year's grant
application and award amount.

Recruitment & Retention Grant Application
Please review and indicate if you would like a work session prior to placing this item on the
Board's agenda.
Request Board Approval to Submit Colorado Department of Human Services (CDHS)
Recruitment & Retention Grant Application. The (CDHS) Recruitment and Retention Grant
provides short-term funds for efforts or activities that recruit new foster and adoptive parents or
support the retention of active foster families. Grand awards range up to $5,000 and awards are
based on previous grant outcomes, number of certified foster homes for the organization and the
amount of funds available.
If the award is received, funds will be used to host a signature event entitled the Weld County
Hope Event, encompassing both foster care home recruitment and retention. The event will
bring together current and potential community partners and foster parents in one location. It
will include testimonials from current and past foster parents, an adult biological child whose
family has fostered for many years, and a former Weld County foster youth.
Attached is a description of the Event/Activity that is proposed and detailed budget. The event
budget is capped at $6,000 and we are requesting a grant award of $5,000. The amount of
