RE: APPROVE CONTRACT TASK ORDER AMENDMENT #2 FOR LEAD SURVEILLANCE
- COLORADO CHILDHOOD LEAD POISONING PREVENTION PROGRAM AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Task Order Amendment #2 for
the Lead Surveillance - Colorado Childhood Lead Poisoning Prevention Program between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Weld County Department of Public Health and Environment, and the
Colorado Department of Public Health and Environment, commencing upon full execution of
signatures, and ending September 29, 2018, with further terms and conditions being as stated in
said task order amendment, and
WHEREAS, after review, the Board deems it advisable to approve said task order
amendment, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Contract Task Order Amendment #2 for the Lead Surveillance - Colorado
Childhood Lead Poisoning Prevention Program between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Colorado Department of Public Health
and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order amendment.

SUBJECT: Extension of Period of Performance for Lead
Surveillance — Colorado Childhood Lead
Poisoning Prevention Program (CCLPPP) Task
Order Contract Amendment
Enclosed for the Board's approval is a Task Order Contract Amendment for the Lead Surveillance
— Colorado Childhood Lead Poisoning Prevention Program between the Colorado Department of
Public Health and Environment (CDPHE) and the Board of County Commissioners of Weld County
for the use and benefit of the Weld County Department of Public Health and Environment
(WCDPHE).
The purpose of this contract amendment is to change the period of performance by extending it
by one year, changing the current ending date of September 29, 2017, to September 29, 2018.
The revised period of performance is September 30, 2016, through September 29, 2018.
WCDPHE currently conducts surveillance, investigation, and reporting of elevated childhood
blood lead levels under a contract with CDPHE in the amount of $13,000 to provide operational
funding to fund our local lead program. This funding originates at the U.S. Department of Health
and Human Services through the CDC. This is part of a State initiative to provide funding to local
public health agencies that experience the largest burden of elevated childhood blood lead cases
in Colorado. As a part of this proposed contract, WCDPHE will continue to actively participate in
CCLPPP workgroups and meetings to develop efficiencies through the development of systematic
guidelines, processes, and reporting requirements for elevated blood lead cases by collaborating
with the State and other counties. In addition, WCDPHE will continue to conduct case
investigations on elevated childhood blood lead levels as we have in the past and our findings
will be reported to CDPHE. No additional FTE will be required to provide the grant deliverables.
This contract amendment was approved for placement on the Board's agenda via pass -around
memo dated August 23, 2017. I recommend approval of the Task Order Contract Amendment
with CDPHE.

This Task Order Amendment is made this 26th day of July, 2017, by and between the State of Colorado, acting by and
through the DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT whose address or principal place of
business is 4300 Cherry Creek Drive South, Denver, Colorado 80246, hereinafter referred to as the "State"; and,
BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY, (a political subdivision of the state of
Colorado)for the use and benefit of the Weld County Department of Public Health and Environment whose
address or principal place of business is 1555 North 17A Avenue, Greeley, Colorado 80631, hereinafter referred to as
the "Contractor".
FACTUAL RECITALS
The parties entered into a Master Contract dated January 17th, 2012, with contract routing number 13 FAA 00051.
Pursuant to the terms and conditions of the Master Contract the parties entered into a Task Order Contract dated July
24th, 2016, with encumbrance number 20160000000000001338, and task order contract routing number 16 FHHA
82441, hereafter referred to as the "Original Task Order Contract", whereby the Contractor was to provide to the
State the following:
This project serves to provide operational funding to local public health agencies that experience the largest burden of
elevated childhood blood lead cases in Colorado. The public health agencies will actively participate in CCLPPP
workgroups and meetings to develop efficiencies through the development if systematic guidelines, processes, and
reporting requirements for elevated blood lead cases.
Changes were required to extend current term.
The State hereby exercises a "no cost" change within the current term of the Original Task Order Contract.
NOW THEREFORE, in consideration of their mutual promises to each other, stated below, the parties hereto agree as
follows:
1. Consideration for this Contract Amendment to the Original Task Order Contract consists of the payments and
services that shall be made pursuant to this Contract Amendment, and promises and agreements herein set
forth.
2. It is expressly agreed to by the parties that this Contract Amendment is supplemental to the Original Task
Order Contract, contract routing number 16 FHHA 82441, as amended by Task Order Amendment # 1,
routing number 17 FHHA 93542; Grant Funding Change Letter#1, routing number 17 FHHA 96684
collectively referred to herein as the Original Task Order Contract, which is by this reference incorporated
herein. All terms, conditions, and provisions thereof, unless specifically modified herein, are to apply to this
Contract Amendment as though they were expressly rewritten, incorporated, and included herein.

It is expressly agreed to by the parties that the Original Task Order Contract is and shall be modified, altered,
and changed in the following respects only:
A. The period of performance of the current term is hereby extended by One (1) year, changing the
current ending date from September 29th, 2017 to September 29th, 2018. The revised period of
performance is September 30th, 2016 through and including September 291, 2018.
B. This Contract Amendment is issued pursuant to paragraph 5 of the Original Task Order Contract. This
Contract Amendment is for the current term of September 30th, 2016, through and including September
29th, 2018. The revised Statement of Work of the Original Task Order Contract is incorporated herein by
this reference and identified as "Exhibit G".
C. The Contract and all prior amendments thereto, if any, are modified as follows:
i. The following lines are hereby modified:
a. Contract Term in Section I, Page lof the Exhibit G.
b. Section VI, Page 2 of the Exhibit G.
The Original Task Order Contract is modified accordingly. All other terms and conditions of the Original
Task Order Contract are reaffirmed.
4. The Effective Date of this Contract Amendment is August 1Sa, 2017, or upon approval of the State Controller,
or an authorized delegate thereof, whichever is later.
5. Except for the General Provisions and Special Provisions of the Original Task Order Contract, in the event of
any conflict, inconsistency, variance, or contradiction between the terms and provisions of this Contract
Amendment and any of the terms and provisions of the Original Task Order Contract, the terms and provisions
of this Contract Amendment shall in all respects supersede, govern, and control. The Special Provisions shall
always control over other provisions of the Original Task Order Contract or any subsequent amendments
thereto. The representations in the Special Provisions to the Original Task Order Contract concerning the
absence of personal interest of state of Colorado employees are presently reaffirmed.
6. FINANCIAL OBLIGATIONS OF THE STATE PAYABLE AFTER THE CURRENT FISCAL YEAR ARE
CONTINGENT UPON FUNDS FOR THAT PURPOSE BEING APPROPRIATED, BUDGETED, AND
OTHERWISE MADE AVAILABLE.

IL Project Description:
This project serves to provide operational funding to local public health agencies that experience the largest
burden of elevated childhood blood lead cases in Colorado. The public health agencies will actively participate in
CCLPPP workgroups and meetings to develop efficiencies by establishing systematic guidelines, processes, and
reporting requirements for elevated blood lead cases. Additionally, utilizing current, established elevated blood
lead case management/investigation protocols within their respective agency, they will conduct case investigations
on elevated childhood blood lead levels and report investigation findings to CDPHE.

Goal #1: Prevent and ultimately eliminate childhood lead poisoning in Colorado.
Objective #1: No later than the end of the contract, the Contractor shall provide each child identified with an Elevated Blood
Lead Level in the Target Geographical Area with proper care, education, and information about lead poisoning in an effort to
reduce his/her lead level.
Primary Activity #1
The Contractor shall perform exposure investigations in the Target Geographical Area_
Sub -Activities #1
1. The Contractor shall conduct exposure investigations to determine the cause of an
Elevated Blood Lead Level in the home of children with confirmed Elevated Blood Lead
Level test results, as reported to the Contractor by CDPHE.
2. The Contractor shall notify CDPHE, via email, if there is insufficient staff capacity and
ability to perform exposure investigations.
a. If the Contractor does not have the staff capacity and/or ability to perform an
exposure investigation, CDPHE will assist to coordinate or conduct the
investigation.
3. The Contractor shall collect exposure investigation information via a method approved by
CDPHE, and submit these findings to CDPHE.
4. The Contractor shall report investigation findings via a follow-up letter to the families of
the children with Elevated Blood Lead Levels and the family's healthcare providers.
Primary Activity #2
The Contractor shall design and provide educational materials related to lead poisoning.
Sub -Activities #2
1. The Contractor shall have the option of creating new educational materials under this
project.
2. The Contractor shall provide the family of every child with an Elevated Blood Lead Level
in the Target Geographical Area, with educational materials that promote lead poisoning
prevention.
Primary Activity #3
The Contractor shall attend trainings and CCLPPP meetings as directed by CDPHE.

Sub -Activities #3
1. The Contractor shall attend a minimum of one (1) CCLPPP conference call orientation
meeting as directed by CDPHE.
2. The Contractor shall attend and participate in a minimum of one (1) Colorado Lead
Coalition meetings as directed by CDPHE. The goal of these meetings shall be to develop
efficiencies by establishing systematic guidelines, processes, and reporting requirements
for Elevated Blood Lead Level cases.
Standards and

1. The Contractor shall use a CDPHE-approved method to collect exposure investigation
information to submit to CDPHE.
a. Exposure investigations shall be conducted no later than 60 days after CDPHE
notifies the Contractor of a confirmed Elevated Blood Lead Level test result.
2. The Contractor shall use a CDPHE-approved follow-up letter to report investigation
findings to the families of the children with Elevated Blood Lead Levels, no later than 30
days after the investigation is complete.

V. Monitoring:
CDPHE's monitoring of this contract for compliance with performance requirements will be conducted throughout
the contract period by the Lead Surveillance Coordinator. Methods used will include a review of documentation
determined by CDPHE to be reflective of performance to include progress reports and other fiscal and
programmatic documentation as applicable. The Contractor's performance will be evaluated at set intervals and
communicated to the contractor. A Final Contractor Performance Evaluation will be conducted at the end of the
life of the contract.
VI. Resolution of Non -Compliance:
The Contractor will be notified in writing within forty-five (45) calendar days of discovery of a compliance issue.
Within forty-five (45) calendar days of discovery, the Contractor and the State will collaborate, when appropriate,
to determine the action(s) necessary to rectify the compliance issue and determine when the action(s) must be
completed. The action(s) and time line for completion will be documented in writing and agreed to by both
parties. If extenuating circumstances arise that requires an extension to the time line, the Contractor must email a
request to the Lead Surveillance Coordinator and receive approval for a new due date. The State will oversee the
completion/implementation of the action(s) to ensure time lines are met and the issue(s) is resolved. If the
Contractor demonstrates inaction or disregard for the agreed upon compliance resolution plan, the State may
exercise its rights under the provisions of this contract.
