Final Performance Evaluation Worksheet
Upon completion of a grant contract The DOLA Grant Manager is responsible for determining
whether the grantee has complied with the terms of the contract pursuant to CRS 24-103.5-
101(3). The DOLA Grant Manager shall complete this FINAL performance evaluation and
submit it to the division with the final draw request. The rating shall be added to the state
Contract Management System (CMS) within 30 days after contract completion and become
publicly available as part of the State's public website. This worksheet will be added to the
contract file. The grant manager should make the grantee aware of the rating. lithe grantee
wishes to dispute any information contained in the evaluation, they may exercise the contract
right set forth in CRS 24-109-106, 107, 201 or 202
Within 30 days of contract completion, DLG staff will scan this document and send to the
Grantee for their review and comment.
