Community Services Block Grant (CSBG) Contract
Amendment
On March 28. 2017 the Department requested authorization to submit a contract amendment to the
Community Services Block Grant (CSBG) through pass -around (see attached). The amendment would
allow case management services to be provided through Area Agency on Aging. After receiving BOCC
approval, the request for amendment was submitted and the following response was received. There is
no need to amend the contract per the State.

Because the key eligible expenses of case management. staff salary and benefits.
mileage and supplies are already listed under eli,ihle expenses, we have determined
that a contract amendment is not necessary for these costs to he coy Bred.
I understand that you have expanded the types of clients who can receive these
services but because the costs are covered under your current services and the CSBG
Act, it is not necessary to restate those costs for an expanded population.

Please review and indicate if you would like a work session prior to placing this item on the Board's agenda.
Request Board Approval of the Department's Community Services Block Grant (CSBG) Contract
Amendment. The Department of Iluman Services ould like to request an amendment to our 2015-2018
contract and Scope of Project, which modifies the contract to allow C'SBG funds to be used to:
Provide case management services through Area Agency on Aging to the elderly, persons with disabilities,
persons with mental health needs, persons with brain and spinal cord injuries that meet the CSBG eligibility
guidelines. The funds will he used to pay for staff salary plus benefits, mileage and supplies, to help maintain
the lives of Weld County residents, in the least restricting living environment.
I do not recommend a Work Session. I recommend approval of Contract Amendment Request and Board Chair
signature.
