HomeMy WebLinkAbout20171797.tiffRESOLUTION RE: APPROVE 2017-2018 CHILD WELFARE RESPITE CARE CONTRACT PROVIDER LIST AND RATE WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with 2017-2018 Child Welfare Respite Care Contract Provider List and Rate between the Weld County Department of Human Services, and various providers, as listed on the attached provider list, and WHEREAS, after review, the Board deems it advisable to approve the 2017-2018 Child Welfare Respite Care Contract Provider List and Rate, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the 2017-2018 Child Welfare Respite Care Contract Provider List and Rate between the Weld County Department of Human Services, and various providers, be and hereby are, approved. Griego, Director, Human Services RE: Weld County Department of Human Services' Child Welfare 2017-18 Respite Care Provider Agreements Please review and indicate if you would like a work session prior to placing these items on the Board's agenda. Request Board Approval of the Department's Child Welfare 2017-18 Respite Care Provider Agreements. The below list reflects the providers that have been selected by the Departments Division of Child Welfare to provide respite care during the coming program year. Individuals and couples, primarily Weld County Certified foster parents, have met certification requirements. Respite care is limited to four (4) hours per week per child. The hours of care may be provided in any combination throughout a month, but may not exceed 16 hours per month. Payment is $16.75 per each four (4) hour period, but may not exceed $67.00 per month for 16 hours. The term of the agreements is July 1, 2017, through June 30, 2018. Upon Board approval of this pass -around, individual signed agreements will be obtained from providers and submitted through the Contract Management System (CMS) for final Board approval.
