DHS submitted a PA in June that noted our 2017-18 Respite Care Providers. It was approved on Consent. Unfortunately,
we have had 10 providers notify us that they will not be continuing or they have elected to not return their agreements
by the deadline, As such, it appears we need to notify the Board that these parties will not be moving forward so they
do not stay open on Consent. I've noted the parties below and attached the original PA with their names
highlighted. Please let me know if anything further is needed or if this should be sent to someone else.

Welfare 2 017 -18 Respite Care Provider Agreements
Please review and indicate if you would like a work session prior to placing these items on the
Board's agenda.
Request Board Approval of the Department's hild Welfare 201748 Respite Care
Provider Agreements. The below list reflects the providers that have been selected by the
Departments Division of Child Welfare to provide respite care during the coming program year.
Individuals and couples, primarily Weld County Certified foster parents, have met certification
requirements. Respite care is limited to four (4) hours per week per child. The hours of care may
be provided in any combination throughout a month, but may not exceed 16 hours per month.
Payment is $16.75 per each four (4) hour period, but may not exceed $67.00 per month for 16
hours. The term of the agreements is July 1, 2017, through June 30, 2018. Upon Board approval
of this pass -around, individual signed agreements will be obtained from providers and submitted
through the Contract Management System (CMS) for final Board approval.
