On behalf of United Way of Weld County, THANK YOU! As a member of the Blue Ribbon Committee, you
spent 13 months meeting, discussing and planning what has come to be known as "Weld's Way Home: A
Strategic Plan to Address and Prevent Homelessness in Weld County." Enclosed, you will find the final
version of the strategic plan that, without your help, would not have been possible.
It is this plan that is driving the work of the Weld's Way Home Executive Council. One of the biggest
unanswered questions the Blue Ribbon Committee had was how we were going to implement all of the
great ideas that are captured in Weld's Way Home. We now have an answer to that question: The
Weld's Way Home Executive Council has been tasked with the implementation of the plan that you
helped write. A Weld's Way Home Coordinator will also be hired as the lead staff person to help the
Executive Council implement this work. Six working groups, comprised of dedicated volunteers and
community partners, have also been established to help us make progress on the successful and
sustainable implementation of the eight priority areas you outlined at the end of our planning process.
The six working groups are as follows:
1. Communications —working on community outreach
2. Faith Community Engagement
3. Permanent Facilities --working on the navigation center and sustainable solution for the cold
weather overflow shelter
4. Outreach and Care Management —working on discharge planning, street outreach, and
expanded case management services
5. Affordable Housing
6. Coordinated Assessment and Housing Placement System (CAHPS)—working on shared intake
and assessment

Weld's Way Home is more than just a strategic plan. It has become a community -wide initiative with
action plans and excited staff and volunteers. None of this would have been possible without your
