Associated production equipment, including separators, a tank battery, metering equipment, flow lines, gathering lines, pipelines and an access road will be constructed (Refer to the enclosed Drilling and Site Improvements Plan for a specific equipment list and corresponding heights). It is anticipated that wells will be drilled in one phase and completed in one phase. Access improvements to the well pad are also proposed. Conditional Use Grant Application Oil & Gas and Mineral Extraction Page 2 of 2 Revised 8/1/2016 EXTRACTION Oil & Gas Milkshake Pad: Project Information & Narrative Section 32, Township 6 North, Range 67 West Windsor, CO PROPERTY: The property is located in the west half of the southwest quarter of Section 32, Township 6 North, Range 67 West of the 6th Principal Meridian in Windsor, Colorado. The project site is generally located northeast of the intersection of the Crossroads Boulevard (WCR 62) and County Road 15. DESCRIPTION OF THE PROPERTY: The property was annexed into the Town of Windsor as part of the LaBue Farm Annexation completed in 2016. The proposed well pad will be located on Lot B of RE 1415 which consists of approximately 155 acres. Per the Weld County Assessor GIS Map it appears that Lot B of RE 1415 is comprised of 4 parcels, 3 of which are owned by Vima Partners, LLC. The proposed Milkshake Well pad is proposed on the two western Vima Partners lots. Per the LaBue Annexation all parcels are zoned Recreational and Open Space (O). Currently, oil and gas development is located on the property. Extraction Oil and Gas, Inc. (Extraction) operates the Pavistma Pad, COGCC Location ID 427262, and the Pavistma South Pad, COGCC Location ID 433139, and Kerr McGee operates the Pavistma-66n67w/32NWSW pad, COGCC location ID 333270. Currently Extraction operates 11 wells on the Pavistma pad on the northern end of the property with associated production equipment and 6 wells on the Pavistma South pad with associated production equipment on the south half of the existing well pad. Kerr McGee operates 3 wells on the property between the two Extraction locations. Refer to the Topographic Survey included with this submittal for additional information pertaining to existing information. All three locations were drilled and constructed prior to the location being annexed into the Town of Windsor and therefore do not operate under a Conditional Use Grant. As part of the proposed Milkshake well pad, Extraction proposes to remove and redesign the associated production equipment of the Pavistma and Pavistma South pads to consolidate and design the well pad for additional development. Please refer to the Topographic Survey and Drilling and Site Improvements plan for additional information. SURROUNDING LAND USES: The property is surrounded by residential uses; both residential mixed use as well as estate zoning. PROPERTY OWNER: Vima Partners, LLC 1625 Pelican Lakes Point Suite 201 Windsor, Colorado 80550 OIL & GAS OPERATOR: Extraction Oil & Gas, Inc. 370 17th Street, Suite 5300 1 Denver, Colorado 80202 BRIEF PROJECT NARRATIVE PROPOSED USE: The proposed use is a multiple oil and gas well pad site located adjacent to the existing Pavistma well pads. Extraction is entering into an agreement with Vima Partners, LLC allowing Extraction to utilize the surface of the property to conduct drilling operations and to install and operate related oil and gas production facilities. A copy of this agreement will be provided to the Town once executed. Extraction is proposing to drill up to 23 additional horizontal oil and gas wells on the western portion of the property in addition to the 17 existing wells that are currently operated by Extraction. Associated production equipment, including separators, tank batteries, metering equipment, LACT units, flow lines, gathering lines, and pipelines will be constructed (Refer to the Concept Site Plan). As stated above Extraction intends to reconfigure and redesign the existing Pavistma and Pavistma South pads are part of the proposed Milkshake development. The production facilities necessary for the Pavistma and Pavistma South wells will be incorporated into the Milkshake facility design. Part of the redesign includes re -grading portions of the existing pad to better accommodate stormwater drainage and to re -position the existing screening berm along the eastern edge of the pad to provide screening of the proposed additions. Earthwork operations pertaining to the existing screening berm will be coordinated with the surface owner as the berm was constructed by and is maintained by the surface owner. The existing access road will be utilized. It is anticipated that wells will be drilled in one phase and completed in one phase. To accommodate the drilling rig, it is anticipated that the screening berm will not be wrapped around the south of end of the pad until drilling operations are complete. Please note that it is anticipated that the Kerr McGee wells and associated production equipment will remain in operation. The locations and dimensions of proposed and existing easements and rights -of -way will be obtained and shown in subsequent submittals upon the approval of the proposed concept. NUMBER OF PEOPLE WHO WILL USE THIS SITE: During drilling and completion of the wells, Extraction employees and contractors will be accessing the site. Upon completion of the wells, the site will be accessed daily by Extraction employees for monitoring purposes. Tanker trucks will also access the site daily to pick up produced water. Produced oil will be transported off -site via a crude oil pipeline. NUMBER OF EMPLOYEES TO BE EMPLOYED AT THIS SITE: During drilling and completion of the wells, Extraction employees and contractors will be working on -site. Upon completion of the wells, the site will be monitored daily by Extraction employees or contract employees. HOURS OF OPERATION: Drilling and completions operations will utilize a 24 -hour schedule and the wells will produce 24 hours a day. 2 ACCESS POINTS AND HAUL ROUTES: The existing access point from Crossroads Boulevard (WCR 62) will be utilized to access this location. Truck traffic will utilize Crossroads Boulevard to I-25, Crossroads Boulevard to Weld County Road 13 to Highway 34, or Crossroads Boulevard to Weld County Road 17 to Highway 34. No truck traffic will travel north through the roundabout to 7th Street or east of the roundabout past CR 17. TYPE AND NUMBER OF STRUCTURES TO BE ERECTED (BUILT) ON THIS SITE: Extraction is proposing to drill 23 additional wells and redesign the existing production facilities into the proposed Milkshake facility. Refer to the Drilling and Site Improvements plan for specifics. Generally the production equipment will include oil tanks, water tanks, 3 -phase separators and other miscellaneous equipment including emission control devices, vapor recovery units, compressors, metering equipment and sheds, vapor recovery towers and additional separation equipment. The oil tanks and water tanks will have a maximum capacity of 400 barrels. All attempts will be made to make the wells and tanks as low profile as practicable and painted to blend -in with surrounding landscapes. Typically production equipment heights are anticipated to be: • Oil and produced water tanks: 13' • Low pressure separators: 20' • High pressure separators: 15' • 3 phase separators: 19' • Emission control devices: 25' • Vapor recovery towers: 35' • Wellheads: 6' Per COGCC Regulations, secondary containment will be constructed around the tanks. The secondary containment system is constructed of a perimeter of metal walls that are post driven into the ground around a flexible geotextile base. All components are then sprayed with a polyurea liner technology. This liner technology maintains impermeability and puncture resistance under exposure to UV rays, weather extremes, and chemicals commonly encountered in oil and natural gas production, and provides seamless protection. The secondary containment is designed to contain 150% of the capacity of the largest tank. All containment systems will be visually checked weekly, at a minimum, to ensure proper working condition. Inspection and record retention of inspections will be in accordance per SPCC regulation. All records will be made available upon request SCREENING, SOUND BARRIERS AND LANDSCAPING: Prior to the start of operations a background ambient sound level survey will be conducted by a third party specialist. This study will be used to determine appropriate sound mitigation measures required to comply with COGCC noise abatement regulations, Rule 802. It is anticipated that temporary sound walls will be utilized during drilling and completions operations in conjunction with the screening berm along the north and east sides of the location to mitigate noise and visual impacts. Currently the screening berm is constructed along the north and east sides of the location. The southern half of the eastern berm will be re -graded prior to the start of drilling operations to ensure that the berm is in place prior to the start of operations. The temporary sound wall panels are up to 32 feet tall. To ensure long-term noise 3 mitigation, all production facilities on this location will be powered by electricity, rather than the traditional diesel generators. Long term screening of this location will be achieved through additional berming. Extraction is working closely with the surface owner and developer of these properties to achieve screening through earthen berms on all sides. By working with the developer, the proposed well pad will ultimately be screened on all sides by berms ranging in height from 10 to 20 feet. Refer to the Drilling and Site Improvements plan for additional information regarding screening. No landscaping is proposed by Extraction, but it is anticipated that landscaping will be incorporated into the developer's designs. SITE LIGHTING: Proposed site lighting shall be designed to mitigate light pollution. Refer to the lighting cut sheet provided from light type. TYPE AND NUMBER OF ANIMALS, IF ANY, TO BE ON THIS SITE: No animals will be on this site. KIND OF VEHICLES (TYPE, SIZE, WEIGHT) THAT WILL ACCESS THIS SITE AND HOW OFTEN: Once drilling and completions operations are complete, the site will only be accessed for general maintenance and inspection purposes and produced water pickups. Produced oil will be transported offsite via an oil pipeline. The anticipated production traffic to the site includes up to 15 pickup trucks per day for monitoring and maintenance, and tanker trucks up to ten times per day to collect produced water. Trucks types will include semi -trucks with 130 bbl tankers with a max weight of 80,000 pounds. IF STORAGE OR WAREHOUSING IS PROPOSED, WHAT TYPE OF ITEMS WILL BE STORED? No warehousing or storage of equipment or materials is proposed. However, produced oil and water will be stored within the tank battery until it is transported off -site via pipeline or tanker truck. AIR AND WATER QUALITY: The proposed use will not require an on -site water source. WATER SOURCE ON THE PROPERTY: Air and water quality requirements shall be met, as applicable, per EPA, CDPHE and COGCC criteria. WASTE/SEWAGE DISPOSAL SYSTEM ON THE PROPERTY: The proposed use will not require sewage disposal. During construction of the pad and drilling of the wells, portable sanitary facilities will be provided per COGCC Rule 602.f.
