RE: APPROVE APPLICATION FOR TIER I EMERGENCY MEDICAL SERVICES LICENSE
AND AUTHORIZE CHAIR TO SIGN - FRONT RANGE FIRE RESCUE
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, pursuant to Section 7-2-100 of the Weld County Code, the Weld County
Department of Public Health and Environment and the Weld County Emergency Medical/Trauma
Service Council have reviewed the application of the Front Range Fire Rescue for a Tier I
Emergency Medical Services License and recommend approval, and
WHEREAS, the Board was presented with a request for waiver concerning an employee's
traffic offense, and
WHEREAS, Section 7-3-50 of the Weld County Code, states: Unless waived by the Board
of County Commissioners, no personal shall be employed by an ambulance service as an
ambulance crew member who has been convicted of any of the following offenses with the
previous twelve (12) months from the date of application: felony misdemeanor or Class I or
Class I1 traffic offense.
WHEREAS, after review, the Board deems it advisable to grant the requested waiver and
approve said Application for a Tier I Emergency Medical Services License, a copy of which is
attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the request for waiver be, and hereby is, granted and the employment and
crew member status be allowed for the cited employee.
BE IT FURTHER RESOLVED by the Board that the application of the Front Range Fire
Rescue for a Tier I Emergency Medical Services License be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said license.

SUBJECT: 2018 Applications for Ambulance Service License
Enclosed for the Board's review are eleven applications for ambulance service licenses pursuant to the Weld County Code
Chapter 7, Emergency Medical Services, from the ambulance service providers listed below. The Weld County Emergency
Medical Trauma Service (EMTS) Council reviewed the application for each ambulance service provider on November 16,
2016, and approved all 11 applications.
Please note: Front Range Fire Rescue is asking the BOCC for a waiver per county code for an employee convicted of a
Class I driving offense. Front Range Fire Rescue has included a letter with further detail in their application.
Weld County Code, Section 7-3-50. Criminal record of ambulance crew member; "Unless waived by the Board of
County Commissioners, no person shall be employed by an ambulance service as an ambulance crew member
who has been convicted of any of the following offenses within the previous twelve (12) months from the date
of application: felony, misdemeanor or Class I or Class II traffic offense. (Weld County Code Ordinance 2007-8;
Weld County Code Ordinance 2013-7)."
Based on the delegated responsibility of the Department, we have reviewed the applications and have deemed all
ambulance services listed below to have met all applicable licensure application requirements found in Chapter 7. As
such, I am recommending the following ambulance service providers for licensure:

Recommendation of the Weld County Emergency Medical/Trauma Service Council
for: Front Range Fire Rescue
As required in Section 7-2-100.B.5., the EMTS Council should review the application to assess as to
whether the ambulance service will contribute to an efficient, effective, and coordinated emergency
medical response to residents of the County? Please include any findings related to response time,
coordination with other ambulance services, location, service area, etc. that is the basis for the Council's
recommendation:
FINDING #1: Front Range Fire Rescue (FRFR) services approximately 75 square miles of Weld County,
including the Towns of Johnston and Milliken running 1,500 calls for service. FRFR runs 2 ambulances.
FINDING #2: FRFR meets the Standard of Cover Response times adopted by the Board of Directors.
Performance results are included in the application.
FINDING #3: FRFR is in good standing with the Weld County Department of Public Health & Environment.
FINDING #4: Per neighboring agencies, FRFR provides mutual and auto aid in an efficient, effective and
coordinated manner. Included in the application is the mention of a new mutual aid agreement with
Banner Health NCMC Paramedic Services.

Other EMTS Council recommendations or comments: Information included in the application to address

Sec. 7-2-10 License for Ambulance Service. No person shall provide or operate an ambulance
service publicly or privately in the County unless that person holds a valid license to do so issued
by the Board of County Commissioners, except as provided in Section 7-2-80 below. The fee for
said license shall be set by separate ordinance. The license shall issue only in the following tiers
of service:
Tier I: Licensure authorizing for Primary Care, as defined in Section 7-1-30 of this Chapter.
Tier II: Licensure authorizing for transports of patients that originate in Weld County from
licensed medical facilities. This licensure does not provide for primary care, as defined in
Section 7-1-30 of this Chapter.
Tier III: Licensure authorizing for Standby Service, as defined in Section 7-1-30 of this Chapter.

Greetings! Page five of the Front Range Fire Rescue Authority (FRFRA) 2018 Tier 1 license application has an unchecked
box; this letter is to provide further and complete explanation.
On July 23, 2017 an RFT member of the FRFRA was involved in rear -end type traffic accident. My best understanding of
the event, as described by that employee was, that he was stopped at a stoplight on Highway 119 in Firestone, CO. This
individual believed that, after the light had turned green, traffic was moving. He was momentarily distracted and rear
ended a vehicle in front of him at low speed. That vehicle had a driver and a front seat passenger. Both of these
.ndividuals were able to self -extricate, but the driver was, ultimately transported by ambulance with back or neck
pain. WCSO responded initially and CSP took over the scene eventually. MVFPD was the fire agency that responded.
FRFRA policy requires employees to, "report all citations for moving violations...immediately" and, "violation of this
policy may result in disciplinary action, up to and including termination." This employee followed Authority policy and,
without haste, notified his captain.
On September 5, 2017, he appeared in court and, per C.R.S. 42 — 4 —1402 (2)(b), was cited with a Class 1 traffic offense.
Based upon Weld County Code (Sections 7-2-90 and 7-3-50), the low-level nature of this traffic offense, the employee's
immediate self -reporting, no employer concerns about this individual's ability to provide stellar EMS to the community,
and direction from Weld County Program Coordinator Kevin Antuna, I respectfully submitted a letter to ask the Board of
County Commissioners to waive the violation for renewal of our Tier I license on October 19, 2017. I am including this
letter because, as of the date above, I haven't received a formal response from that request.
I have endeavored to be as transparent as possible in my correspondence, but because this application may be
considered an open record I have, for obvious reasons, been somewhat muted. None of this delicate approach should
be interpreted as evasive, but rather deliberate and respectful. With that
