RE: APPROVE SIGNATORY DELEGATION OF AUTHORITY FORM FOR COMMUNITY
DEVELOPMENT BLOCK GRANT FOR COUNTY ROAD 53 BRIDGE PROJECT AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Signatory Delegation of Authority Form
for Community Development Block Grant for County Road 53 Bridge Project from the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Office of Emergency Management, to the Colorado Department of Public Safety,
Division of Homeland Security and Emergency Management, commencing upon full execution of
signatures, and
WHEREAS, after review, the Board deems it advisable to approve said form, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Signatory Delegation of Authority Form for Community Development
Block Grant for County Road 53 Bridge Project from the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Office of
Emergency Management, to the Colorado Department of Public Safety, Division of Homeland
Security and Emergency Management be and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said form.

Commissioners,
In November we were notified by the State that we were selected to receive partial funding for the matching
dollars for the County Rd 53 bridge project.
Attached is a letter from the state advising of our next steps to move forward with the application process for
the CDBG-DR round 3 funding for the County Road 53 project. Funding in the amount of 333,085.00 has been
set aside for the county match for the bridge project. The Form 2 needs to be signed by the Chair authorizing
Barb Connolly signing authority for quarterly reports, payment requests, monitoring documents and certified
payroll.
This is consistent with what we have completed on other flood recovery grants.
Recommendation to the BOCC: I recommend the BOCC allow me to add this to the BOCC consent agenda.

Community Development Block Grant —

The Colorado Division of Homeland Security and Emergency Management (DHSEM) is pleased to
inform you that Weld County has been preliminarily awarded funding from the third allocation
of the Community Development Block Grant - Disaster Recovery Infrastructure (CDBG-DR-I)
program. Funding in the amount of up to $333,085 has been set aside to assist the County with
the County Match FHWA Sites 5 53/58A project. Please note that this letter is not an
agreement to provide funds for this project. Upon completion of the next steps outlined
below, the County will receive a grant agreement from DHSEM for review and approval.
This project will now move to a final review of eligibility, the project schedule, and Low -
Moderate Income (LMI) status (if applicable). If this project is later determined to be
ineligible, is unable to be completed within the regulatory timeframe, or is determined to be a
non-LMI project (if applicable); DHSEM may revoke this set -aside amount and move funding to
the next highest scoring project.
The next steps for the County in the CDBG-DR-I grant process are:
• Please complete the attached Form 1 - Project Data Sheet which summarizes the
data needed to prepare the Grant Agreement and schedule project funding. A key
element in the third allocation of CDBG-DR-I funding is that there is some flexibility in
creating a project schedule that fits the County's needs. Please return Form 1 as an
electronic document to DHSEM at cdps_dhsem_cdbg@state.co.us;
• Please create and complete a Signatory Authority/Delegation of Authority form
on County letterhead using one of the templates provided in Form 2. This form
identifies the person(s) who have been delegated authority to make decisions and
complete various reporting tasks for your project. Please return an electronic copy of
the executed Signatory Authority/Delegation of Authority along with the appropriate
document reflecting approval to cdps_dhsem_cdbg@state.co.us; and
• DOLA and DHSEM have completed a preliminary review of the Environmental
Review Worksheet associated with this project. The next step in the review process
requires that the County submit project design, planning, permitting and engineering
documents into the DOLA environmental review process. Please contact AGEISS, Inc. at

review. Any federal or state environmental reviews that have already been started or
completed should also be included with your submittal to AGEISS, Inc.
The items and forms listed above are required to move your project forward in the grant
approval process. Please be aware that Federal regulations prohibit the State of Colorado from
obligating CDBG-DR-I funds until the environmental review has been completed after which
funding will be formally released by the State. Failure to meet Housing and Urban
Development's (HUD) program requirements or obtain necessary federal, state, or local
permits may jeopardize grant funding. Also, please remember that the environmental
requirements for this grant are specific to HUD's CDBG-DR program. Other funding sources used
for this project may have additional compliance requirements.
Upon completion of the environmental compliance review, DHSEM will prepare a draft grant
agreement for review by the County. If you have any questions on these next steps please
