RE: APPROVE TASK ORDER CONTRACT AMENDMENT #1 FOR PREVENTION OF
INFERTILITY AND MANAGEMENT OF SEXUALLY TRANSMITTED INFECTIONS (STI)
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Contract Amendment #1 for
the Prevention of Infertility and the Management of Sexually Transmitted Infections (STI) between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Department of Public Health and Environment, and
the Colorado Department of Public Health and Environment, commencing January 1, 2018, and
ending December 31, 2018, with further terms and conditions being as stated in said amendment,
and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Task Order Contract Amendment #1 for the Prevention of Infertility and
the Management of Sexually Transmitted Infections (STI) between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Public Health and Environment, and the Colorado Department of
Public Health and Environment, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.

SUBJECT: Continuation Contract with CDPHE for the
prevention of infertility and the management
of sexually transmitted infections (STI)
Enclosed for the Board's approval is a task order contract amendment between the Colorado
Department of Public Health and Environment (CDPHE) and the Board of County Commissioners
for the use and benefit of the Weld County Department of Public Health and Environment
(WCDPHE) for the management of sexually transmitted infections (STI).
This project serves to support the prevention of infertility and Chlamydia trachomatis and N.
gonorrhea infection through the provision of screening, treatment, and partner management for
these infectious diseases. Chlamydia continues to be the most commonly reported sexually
transmitted infection (STI) in Colorado. Project activities include testing in the federally funded
Title X clinic(s) including Family Planning (FP) and STD clinics within WCDPHE, and the Platte
Valley Youth Detention Center (PVYDC). WCDPHE has partnered with CDPHE and received cost
reimbursement for C. trachomatis and N. gonorrhea testing for at least the past six years.
The Task Order reimburses the cost of 620 Chlamydia trachomatis and N. gonorrhea tests at a
rate of $21.00 per test for a total project reimbursement amount of $13,020 (370 in FP & STD;
and 250 for PVYDC). The term of this contract is from January 1, 2018, through December 31,
2018.
This contract amendment was approved for placement on the Board's agenda via pass -around
memo dated November 14, 2017. I recommend approval of this task order continuation contract
with CDPHE for the prevention of infertility and the management of sexually transmitted
infections (STI).

TASK ORDER AMENDMENT #1
This Task Order Amendment is made this 2nd day of November, 2017, by and between the State of Colorado, acting
by and through the DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT, whose address or principal
place of business is 4300 Cherry Creek Drive South, Denver, Colorado 80246, hereinafter referred to as the "State";
and, BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY, (a political subdivision of the state of
Colorado) 1150 "O" Street, Greeley, Colorado 80631 for the use and benefit of the Weld County Department of
Public Health and Environment, whose address or principal place of business is 1555 North 17th Avenue, Greeley,
Colorado 80631, hereinafter referred to as the "Contractor".
FACTUAL RECITALS
The parties entered into a Master Contract dated January 17th, 2012, with contract routing number 13 FAA 00051.
Pursuant to the terms and conditions of the Master Contract the parties entered into a Task Order Contract dated
February Pt, 2017, with encumbrance number 201700002993, and task order contract routing number 17 FHHA
95844, hereafter referred to as the "Original Task Order Contract", whereby the Contractor was to provide to the
State the following:
This project serves to support the prevention of infertility and Chlamydia Trachomatis (CT) and Neisseria Gonorrhea
(GC) infection through the provision of screening, treatment, and partner management for these
infectious diseases.
Changes were required to extend for an additional term.
The State promises pay the Contractor the sum of Thirteen Thousand. Twenty Dollars (513.020.001 for the renewal
term of One (1) year ending on December 31, 2018, in exchange for the Contractor to perform the specifications to
the work described herein.
NOW THEREFORE, in consideration of their mutual promises to each other, stated below, the parties hereto agree as
follows:
1. Consideration for this Contract Task Order Amendment to the Original Task Order Contract consists of the
payments and services that shall be made pursuant to this Contract Amendment, and promises and agreements
herein set forth.
2. It is expressly agreed to by the parties that this Contract Task Order Amendment is supplemental to the
Original Task Order Contract, contract routing number 17 FHHA 95844, referred to herein as the Original
Task Order Contract, which is by this reference incorporated herein. All terms, conditions, and provisions
thereof, unless specifically modified herein, are to apply to this Contract Amendment as though they were
expressly rewritten, incorporated, and included herein.

3. It is expressly agreed to by the parties that the Original Task Order Contract is and shall be modified, altered,
and changed in the following respects only:
I . This Contract Task Order Amendment is issued pursuant to paragraph 5 of the Original Task Order
Contract. This Contract Task Order Amendment is for the renewal term of,Ianuary 1". 2018 through and
including December 31st. 2018, The maximum amount payable by the State for the work to be performed by
the Contractor during this renewal term is Thirteen Thousand. Twenty Dollars (S13.020.00' for an
amended financial obligation of the State of Twenty Four Thousand. Nine Hundred Fifty Five Dollars
(S24.955.00)
a. For this renewal term the maximum amount payable attributable to United States Government
funding source (see Catalog of Federal Domestic Assistance (CFDA) number 93.268) is Thirteen
Thousand. Twenty Dollars (S13.020.OO1, and the amended total contract financial obligation
attributable to United States Government funding source (see Catalog of Federal Domestic
Assistance (CFDA) number 93.268) is Twenty Four Thousand. Nine Hundred Fifty Five Dollars

The modified specifications to the original Statement of Work and Budget are incorporated herein by this
reference and identified as "Exhibit F" and "Exhibit G".
4. The Effective Date of this Contract Task Order Amendment is January 1s'. 2018, or upon approval of the
State Controller, or an authorized delegate thereof, whichever is later.
5. Except for the General Provisions and Special Provisions of the Original Task Order Contract, in the event of
any conflict, inconsistency, variance, or contradiction between the terms and provisions of this Contract
Amendment and any of the terms and provisions of the Original Task Order Contract, the terms and provisions
of this Contract Task Order Amendment shall in all respects supersede, govern, and control. The Special
Provisions shall always control over other provisions of the Original Task Order Contract or any subsequent
amendments thereto. The representations in the Special Provisions to the Original Task Order Contract
concerning the absence of personal interest of state of Colorado employees are presently reaffirmed.
6. FINANCIAL OBLIGATIONS OF THE STATE PAYABLE AFTER THE CURRENT FISCAL YEAR ARE
CONTINGENT UPON FUNDS FOR THAT PURPOSE BEING APPROPRIATED, BUDGETED, AND
OTHERWISE MADE AVAILABLE.

STATEMENT OF WORK

These provisions are to be read and interpreted in conjunction with the provisions of the contract
specified above.
I. Entity Name: Weld County Department of Public Health & Environment
II. Project Description: This project serves to support the prevention of infertility, Chlamydia
Trachomatis (CT), and Neisseria gonorrhea (GC) infection through the provision of screening,
treatment, and partner management for these infectious diseases. Chlamydia continues to be the most
commonly reported sexually transmitted infection (STI) in Colorado. Project activities include testing in
the federally funded Title X clinic, STI clinic, and community health clinics.
III. Definitions:
I. CDC — Centers for Disease Control and Prevention
2. CT — Chlamydia Trachomatis - a sexually transmitted disease that when left untreated, may
result in infertility in women.

3. GC — Neisseria gonorrhea - a sexually transmitted disease that when left untreated, may result in
infertility in women
4. Sequelae — A pathological condition resulting from a disease, i.e., a complication of untreated
disease
5. STD — Sexually transmitted disease(s)
6. Title X - Title X of the Public Health Service Act (Public Law 91-572 Population Research and
Voluntary Family Planning Programs). The federal grant program dedicated solely to providing
persons with comprehensive family planning and related preventive health services.
7. Target population — Young females 15-24 years of age and other persons presenting to the Title
X clinic.
IV.
Goal #1: Prevent the spread of sexually transmitted diseases, their sequelae, including infertility, in Colorado.
Objective #1: No later than the expiration date of the contract, the Contractor shall screen and conduct treatment
for Chlamydia trachomatis and Neisseria gonorrhea persons receiving services in the federally funded Title X
clinic.
Primary Activity #1
The Contractor shall screen for Chlamydia Trachomatis (CT) and Neisseria gonorrhea
(GC) for the Target Population.
Sub -Activities #1
1. The Contractor shall collect the following information for quarterly progress
reports submitted to CDPHE for all persons for whom a CT and/or GC test is
performed by the Contractor:
a. epidemiologic data
b. test results
c. Medicaid status

Primary Activity #2
The Contractor shall administer treatment for persons exposed to, or testing positive
for CT and/or GC.
Sub -Activities #2
1. The Contractor shall ensure all treatment administered is documented pursuant
to generally accepted standards of care for this agency and included in the
quarterly progress reports submitted to CDPHE.
2. The Contractor shall ensure types of treatment are agreed upon with CDPHE
STI/HIV program staff prior to administering treatment to clients.

Requirements
1. The Contractor shall perform CT and GC screening in accordance with CDC
screening guidelines available on the following website
http://www.cdc.gov/std/tg2015/screening-recommendations.htm and are
incorporated and made part of this contract by reference.
2. The Contractor shall ensure the quality of CT and GC control activities in the Title
X clinic and community health clinics through chart review activities agreed upon
in conjunction with the CDPHE STI/HIV program staff.
3. The Contractor shall adhere to the Sexually Transmitted Diseases Summary of
CDC Treatment Guidelines, 2015, available on the following website
http://www.cdc.gov/std/tg2015 and are incorporated and made part of this contract
by reference.
4. The Contractor shall comply with the data collection and reporting requirements
stated in the Data Collection and Reporting Guidelines. This document can be
accessed at https://www.colorado.gov/pacific/cdphe/sti-and-hiv-contractor-and-
provider-resources is incorporated and made part of this contract by reference.
5. The Contractor shall comply with the guidelines concerning data security and
confidentiality stated in the Centers for Disease Control's Data Security and
Confidentiality Guidelines for HIV, Viral Hepatitis, Sexually Transmitted Disease
and Tuberculosis Programs. This document can be accessed at

6. The Contractor shall complete quarterly progress reports that include the
epidemiologic data, Medicaid enrollment status, and CT and GC results for all
persons tested by the Contractor and submit to CDPHE.
7. The Contractor shall comply with the following rules and regulations when
performing data collection and entry, reporting, and the implementation of
activities. These guidelines and standards are located on the state of Colorado
website http://www.sos.state.co.us/CCR/Welcome.do and are incorporated and
made part of this contract by reference:
a. Colo. Rev. Stat § 25-4-14 concerning HIV infection.
b. Colo. Rev. Stat § 18-4-412 concerning the theft of medical records.
c. Colorado State Board of Health Rules for the Reporting, Prevention and
Control of AIDS, HIV Related Illness and HIV Infection (6CCR 1009-9).
8. The content of electronic documents located on CDPHE and non-CDPHE websites
and information contained on CDPHE and non-CDPHE websites may be updated
periodically during the contract term. The Contractor shall monitor documents and
website content for updates and comply with the most recent revisions.

1. Persons exposed to, or testing positive for CT or GC shall have documentation of
adequate treatment.
2. Not less than 80 percent of persons screened for CT and GC shall be females 15-
24 years of age.

of age as determined by aggregate testing data at the 6 -month and 12 -month period

1. The Contractor shall submit quarterly progress reports to the

format. Report shall include the epidemiologic data, Medicaid
enrollment status, and CT and GC results for all persons tested by

V. Monitoring:
CDPHE's monitoring of the purchase order for compliance with performance requirements will be
conducted throughout the purchase order period by the Contract Monitor. Methods used will include a
review of documentation determined by CDPHE to be reflective of performance to include invoices,
inventory logs, STD Request Forms, Cardinal Health invoices and other fiscal and programmatic
documentation as applicable.
VI. Resolution of Non -Compliance:
The Contractor will be notified in writing within 15 calendar days of discovery of a compliance issue.
Within 30 calendar days of discovery, the Contractor and the State will collaborate, when appropriate, to
determine the action(s) necessary to rectify the compliance issue and determine when the action(s) must
be completed. The action(s) and time line for completion will be documented in writing and agreed to by
both parties. If extenuating circumstances arise that requires an extension to the time line, the Contractor
must email a request to the Contract Monitor and receive approval for a new due date. The State will
oversee the completion/implementation of the action(s) to ensure time lines are met and the issue(s) is
resolved. If the Contractor demonstrates inaction or disregard for the agreed upon compliance resolution
plan, the State may exercise its rights under the Terms and Conditions of this Purchase Order.
