RE: APPROVE 2017-2018 OUT -OF -HOME CONTRACT PROVIDER LISTS AND RATES
FOR CPA GROUP HOME/CENTER PROVIDERS AND RESIDENTIAL CHILD CARE
FACILITY PROVIDERS
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with 2017-2018 Out -of -Home Contract
Providers and Rates for CPA Group Home/Center Providers and Residential Child Care Facility
Providers between the Weld County Department of Human Services, and various providers, as
listed on the detailed provider lists, and
1. CPA Group Home/Center Providers
2. Residential Child Care Facility Providers
WHEREAS, after review, the Board deems it advisable to approve the 2017-2018 Out -of -
Home Contract Providers and Rates, a copy of which is attached hereto and incorporated herein
by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the 2017-2018 Out -of -Home Contract Providers and Rates for CPA Group
Home/Center Providers and Residential Child Care Facility Providers between the Weld County
Department of Human Services, and various providers, be and hereby are, approved.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 9th day of August, A.D., 2017.

Certified Group Homes, Group Centers and Residential Child Care
Facilities (RCCF) Providers and Rates
Please review and indicate if you would like a work session prior to placing this item on the Board's agenda.
Request Board Approval of the Departments' FY2017-18 Certified Group Homes, Group Centers, and
Residential Child Care Facilities (RCCF) Providers and Rates. Counties are authorized, using a
methodology that is approved by the State, to negotiate rates, services, and outcomes with providers for out -of -home
(OOH) placement services for children and youth in custody. The proposed providers and rates for FY2017-18 are
attached. The Group Home and Group Center rates represent a cost of living adjustment which was previously
approved by the Board. The RCCF rates have been set by the State.
I do not recommend a Work Session. I recommend to approve the Group Home, Group Center and RCCF
providers and associated rates as attached.
