RE: 1977 DENTAL SERVICES CONTRACT FOR WELFARE RECIPIENTS
BY WELD COUNTY DENTAL SOCIETY.
WHEREAS, the Weld County Dental Society has submitted a
contract to the Board of County Commissioners , Weld County,
Colorado, for the year 1977, a copy of which is attached hereto
and made a part hereof by reference, and
WHEREAS , after careful review and consideration of the
contract, the Board believes it to be in the best interest
of the County to accept the same.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners, Weld County, Colorado, that the 1977 Dental
Services Contract for welfare recipients as submitted by the
Weld County Dental Society be, and the same is hereby approved
and accepted for signature by the Board.
The above and foregoing Resolution was, on motion duly
made and seconded, adopted by the following vote on the 10th
day of January, A.D. , 1977.

WELD COUNTY DEPARTMENT OF SOCIAL SERVICES
DENTAL CONTRACT
This contract executed at Greeley, Colorado this 10th day of January, 1977 ,
by and between the Board of County Commissioners of Weld County, Colorado,
hereinafter called the County, and the Weld County Dental Society, hereinafter
called the Society:
WITNESSETH:
That in consideration of the mutual promises and obligations and other con-
siderations herein expressed, it is mutually agreed until December 31 , 1977:
I . The Weld County Dental Society shall furnish dental services as outlined
in the fee schedule for county indigents. Approved billings shall be sub-
mitted bi-monthly. Payment is to be made to the Weld County Dental Society.
The budgeted annual cost for this service is $10,500.00. When/if this amount
becomes depleted, no provision is made for continued services.
2. The Society will appoint an administrative committee to assist in checking
bills for services rendered.
3. The Society will appoint its own committee, empowered to make all arrangements
to properly administer and fairly divide the funds among participating members
rendering service, and conduct the business of the Society under this contract.
The decision of this committee shall be final and binding to all doctors par-
ticipating in this contract . It is expressly understood, however, that any
member of the Weld County Dental Society may be present at any meeting of
such committee and have full opportunity to examine the books to see that the
funds are divided fairly and without prejudice among all participating doctors
who have performed services. The members of this committee act only as agent
for the Society and in no case shall they be held liable.
4. Any participating doctor suspected of irregular bills or falsifications of
his records may be disqualified from participating in the contract by the
Administrative Committee.
5. The Society will , so far as it lies within the power of its members, see
that proper dental care and service are rendered to all indigents in Weld
County.
6. Weld County indigents receiving dental services under this contract shall
be liable for 10 per cent of the authorized service amount, to a maximum
liability of $10.00.
7. It is understood by all parties to this contract that Title XIX of the
Social Security Act (Medicaid) provides dental services for all children
of eligible public assistance recipients up to the age of 21 . Billing for
services rendered children covered by this Medicaid program will be made
directly to the fiscal agent by the provider.
8. Maximum adult coverage shall be $100.00 per year. This benefit excludes
dentures or other extensive specialized services.

9. Any unexpended funds at the end of the year 1977 shall revert back to
Weld County, Colorado.
IN WITNESS THEREOF, Said parties have subscribed and executed these presents,
the County in Its proper name and by its proper authorized officers, attested
by its Clerk and Seal , and said Society by its proper name and proper officers
thereunto duly authorized by it, the date first shown mentioned.
