RE: APPROVE AGREEMENT FOR PROFESSIONAL SERVICES FOR FOSTER AND

NORTH RANGE BEHAVIORAL HEALTH (NRBH)
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Agreement for Professional Services
for a Foster and Kinship Care Engagement Specialist between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, and a North Range Behavioral Health (NRBH), commencing
April 1, 2018, and ending December 31, 2018, with further terms and conditions being as stated
in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Agreement for Professional Services for Foster and Kinship Care
Engagement Specialist between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services, and
North Range Behavioral Health (NRBH), be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 9th day of May, A.D., 2018, nunc pro tunc April 1, 2018.

RE: Agreement for Professional Services with North Range Behavioral
Health
Please review and indicate if you would like a work session prior to placing this item on the Board's agenda.
Request Board Approval of the Department's Agreement for Professional Services with North Range Behavioral
Health. This agreement will provide behavioral health engagement, training, consultation, linkage and case management
support to Weld County foster and kinship homes. The goal of these services is to assist with the growth and retention of
quality homes and increased permanency for children and youth placed in those homes. The position will also provide
support and education to the Department's Foster Care Coordinators and Kinship Care Coordinators. The agreement
includes a planning period of up to four (4) months to identify specific deliverables, qualitative and quantitative outcomes,
and assessment tools to be used to measure success.
The initial term of this agreement is April 1, 2018, through December 31, 2018, and it is renewable annually, with a
maximum reimbursement of $87,464.00 per year, pro -rated for periods less than 12 months.
I do not recommend a Work Session. I recommend approval of this Agreement.

WELD COUNTY AGREEMENT FOR PROFESSIONAL SERVICES
BETWEEN WELD COUNTY AND NORTH RANGE BEHAVIORAL HEALTH, (NRBH)
(FOSTER AND KINSHIP CARE ENGAGEMENT SPECIALIST)
•THIS AGREEMENT is made and entered into this q day of _ __ 2018, by and
between the County of Weld, a body corporate and politic of the State of Co , by and
through its Board of County Commissioners, whose address is 1150 "O" Street, Greeley,
Colorado 80631 hereinafter referred to as "County," and North Range Behavioral Health
(NRBH), whose address is 1300 North 171Avenue, Greeley, Colorado 80631, hereinafter
referred to as "Contractor".
WHEREAS, the County desires to retain Contractor as an independent Contract
Professional to perform services as more particularly set forth below; and
WHEREAS, Contractor has the ability, qualifications, and time available to timely
perform the services, and is willing to perform the services according to the terms of this
Agreement.
WHEREAS, Contractor is authorized to do business in the State of Colorado and has the
time, skill, expertise, and experience necessary to provide the services as set forth below;
NOW, THEREFORE, in consideration of the mutual promises and covenants contained
herein, the parties hereto agree as follows:
1. Introduction. The terms of this Agreement are contained in the terms recited in this
document and in Exhibits A, B and C, which forms an integral part of this Agreement. Exhibits
A, B, and C are specifically incorporated herein by this reference.
2. Service or Work. Contractor agrees to procure the materials, equipment and/or products
necessary for the Project and agrees to diligently provide all services, labor, personnel and
materials necessary to perform and complete the Project described in Exhibit A.
3. Term. The term of this Agreement shall be from April 1, 2018, through December.31,,.2018,
or Contractor's completion of the responsibilities described in Exhibit A. This contract may be
extended annually upon written agreement of both parties.
4. Termination. Either party may terminate this agreement with thirty (30) days written notice,
with or without cause. Furthermore, this Agreement may be terminated at any time without
notice upon a material breach of the terms of the Agreement.
5. Extension or Modification. Any amendments or modifications to this agreement shall be in
writing signed by both parties. No additional services or work performed by Contractor shall be
the basis for additional compensation unless and until Contractor has obtained written
authorization and acknowledgement by County for such additional services.

6. Compensation/Contract Amount. County agrees to pay an amount no greater than
$87,464.00 per year, as set forth in Exhibit B. County agrees to pay Contractor through an
invoice process during the course of the contract in accordance with the Scope of Work as
described in Exhibit A. Contractor agrees to submit invoices which detail the work completed
by Contractor. The County will review each invoice and if it agrees Contractor has completed
the invoiced items to the County's satisfaction, it will remit payment to Contractor. Contractor
agrees to work within the confines of the Scope of Work outlined in Exhibit A. County will not
withhold any taxes from monies paid to the Contractor hereunder and Contractor agrees to be
solely responsible for the accurate reporting and payment of any taxes related to payments made
pursuant to the terms of this Agreement.
7. Independent Contractor. Contractor agrees that it is an independent Contractor and that
Contractor's officers, agents or employees will not become employees of County, nor entitled to
any employee benefits from County as a result of the execution of this Agreement. Contractor
shall perform its duties hereunder as an independent Contractor. Contractor shall be solely
responsible for its acts and those of its agents and employees for all acts performed pursuant to
this Agreement. Contractor, its employees and agents are not entitled to unemployment insurance
or workers' compensation benefits through County and County shall not pay for or otherwise
provide such coverage for Contractor or any of its agents or employees.
8. Subcontractors. Contractor acknowledges that County has entered into this Agreement in
reliance upon the particular reputation and expertise of Contractor. Contractor shall not enter
into any subcontractor agreements for the completion of this Project without County's prior
written consent, which may be withheld in County's sole discretion.
9. Ownership. All work and information obtained by Contractor under this Agreement or
individual work order shall become or remain (as applicable), the property of County.
10. Confidentiality. Contractor agrees to keep confidential all of County's confidential
information. Contractor agrees not to sell, assign, distribute, or disclose any such confidential
information to any other person or entity without seeking written permission from the County.
Contractor agrees to advise its employees, agents, and consultants, of the confidential and
proprietary nature of this confidential information and of the restrictions imposed by this
agreement.
11. Warranty_ Contractor warrants that the services performed under this Agreement will be
performed in a manner consistent with the standards governing such services and the provisions
of this Agreement. Contractor further represents and warrants that all services shall be performed
by qualified personnel in a professional and workmanlike manner, consistent with industry
standards, and that all services will conform to applicable specifications.
12. Acceptance of Services Not a Waiver. In no event shall any action by County hereunder
constitute or be construed to be a waiver by County of any breach of this Agreement or default
which may then exist on the part of Contractor. Acceptance by the County of, or payment for,
the services completed under this Agreement shall not be construed as a waiver of any of the
County's rights under this Agreement or under the law generally.
13. Insurance and Indemnification. Contractor shall procure at least the minimum amount of
automobile liability insurance required by the State of Colorado for the use of any personal
vehicle. Proof of said automobile liability insurance shall be provided to County prior to the
performance of any services under this Agreement. Professional Liability (Errors and
Omissions Liability) The policy shall cover professional misconduct or lack of ordinary skill for
those positions defined in the Scope of Services of this contract. Contract Professional shall
maintain limits for all claims covering wrongful acts, errors and/or omissions, including design
errors, if applicable, for damage sustained by reason of or in the course of operations under this
Contract resulting from professional services. In the event that the professional liability
insurance required by this Contract is written on a claims -made basis, Contract Professional
warrants that any retroactive date under the policy shall precede the effective date of this
Contract; and that either continuous coverage will be maintained or an extended discovery period
will be exercised for a period of two (2) years beginning at the time work under this Contract is
completed.

14. Indemnity. The Contractor shall defend, indemnify and hold harmless County, its officers,
agents, and employees, from and against injury, loss damage, liability, suits, actions, or claims of
any type or character arising out of the work done in fulfillment of the terms of this Contract or
on account of any act, claim or amount arising or recovered under workers' compensation law or
arising out of the failure of the Contractor to conform to any statutes, ordinances, regulation, law
or court decree.
15. Non -Assignment. Contractor may not assign or transfer this Agreement or any interest
therein or claim thereunder, without the prior written approval of County.
16. Interruptions. Neither party to this Agreement shall be liable to the other for delays in
delivery or failure to deliver or otherwise to perform any obligation under this Agreement, where
such failure is due to any cause beyond its reasonable control, including but not limited to Acts
of God, fires, strikes, war, flood, earthquakes or Governmental actions.
17. Compliance with Law. Contractor sha I strictly comply with all applicable federal and
State laws, rules and regulations in effect or hereafter established, including without limitation,
laws applicable to discrimination and unfair employment practices.
18. Non -Exclusive Agreement. This Agreement is nonexclusive and County may engage or
use other Contractors or persons to perform services of the same or similar nature.
19. Entire Agreement/Modifications. This Agreement, including the Exhibits attached hereto
and incorporated herein, contains the entire agreement between the parties with respect to the
subject matter contained in this Agreement. This instrument supersedes all prior negotiations,
representations, and understandings or agreements with respect to the subject matter contained in
this Agreement. This Agreement may be changed or supplemented only by a written instrument

signed by both parties.
20. Fund Availability. Financial obligations of the County payable after the current fiscal
year are contingent upon funds for that purpose being appropriated, budgeted and otherwise
made available. Execution of this Agreement by County does not create an obligation on the part
of County to expend funds not otherwise appropriated in each succeeding year.
21. Employee Financial Interest/Conflict of Interest — C.R.S. §§24-18-201 et seq. and §24-
50-507. The signatories to this Agreement state that to their knowledge, no employee of Weld
County has any personal or beneficial interest whatsoever in the service or property which is the
subject matter of this Agreement.
22. Severability. If any term or condition of this Agreement shall be held to be invalid, illegal,
or unenforceable by a court of competent jurisdiction, this Agreement shall be construed and
enforced without such provision, to the extent that this Agreement is then capable of execution
within the original intent of the parties.
23. Governmental Immunity. No term or condition of this contract shall be construed or
interpreted as a waiver, express or implied, of any of the immunities, rights, benefits, protections
or other provisions, of the Colorado Governmental Immunity Act §§24-10-101 et seq., as
applicable now or hereafter amended.
24. No Third -Party Beneficiary. It is expressly understood and agreed that the enforcement of
the terms and conditions of this Agreement, and all rights of action relating to such enforcement,
shall be strictly reserved to the undersigned parties and nothing in this Agreement shall give or
allow any claim or right of action whatsoever by any other person not included in this
Agreement. It is the express intention of the undersigned parties that any entity other than the
undersigned parties receiving services or benefits under this Agreement shall be an incidental
beneficiary only.
25. Board of County Commissioners of Weld County Approval. This Agreement shall not
be valid until it has been approved by the Board of County Commissioners of Weld County,
Colorado or its designee.
26. Choice of Law/Jurisdiction. Colorado law, and rules and regulations established pursuant
thereto, shall be applied in the interpretation, execution, and enforcement of this Agreement. Any
provision included or incorporated herein by reference which conflicts with said laws, rules
and/or regulations shall be null and void. In the event of a legal dispute between the parties,
Contractor agrees that the Weld County District Court shall have exclusive jurisdiction to resolve
said dispute.
27. Public Employment Retirement Program. Contractor is responsible for notifying Weld
County of any previous participation in the Colorado Public Employee Retirement Program.
Contractor must notify Weld county of the most recent employment for a PERA contributing
employer.

28. Public Contracts for Services C.R.S. §8-17.5-101. Contractor certifies, warrants, and
agrees that it does not knowingly employ or contract with an illegal alien who will perform work
under this contract. Contractor will confirm the employment eligibility of all employees who are
newly hired for employment in the United States to perform work under this Agreement, through
participation in the E -Verify program or the State of Colorado program established pursuant to
C.R.S. §8-17.5-102(5)(c). Contractor shall not knowingly employ or contract with an illegal
alien to perform work under this Agreement or enter into a contract with a subcontractor that
fails to certify with Contractor that the subcontractor shall not knowingly employ or contract
with an illegal alien to perform work under this Agreement. Contractor shall not use E -Verify
Program or State of Colorado program procedures to undertake pre -employment screening or job
applicants while this Agreement is being performed. If Contractor obtains actual knowledge that
a subcontractor performing work under the public contract for services knowingly employs or
contracts with an illegal alien Contractor shall notify the subcontractor and County within three
(3) days that Contractor has actual knowledge that a subcontractor is employing or contracting
with an illegal alien and shall terminate the subcontract if a subcontractor does not stop
employing or contracting with the illegal alien within three (3) days of receiving
notice. Contractor shall not terminate the contract if within three days the subcontractor provides
information to establish that the subcontractor has not knowingly employed or contracted with an
illegal alien. Contractor shall comply with reasonable requests made in the course of an
investigation, undertaken pursuant to C.R.S. §8-17.5-102(5), by the Colorado Department of
Labor and Employment. If Contractor participates in the State of Colorado program, Contractor
shall, within twenty days after hiring an new employee to perform work under the contract,
affirm that Contractor has examined the legal work status of such employee, retained file copies
of the documents, and not altered or falsified the identification documents for such
employees. Contractor shall deliver to County, a written notarized affirmation that it has
examined the legal work status of such employee, and shall comply with all of the other
requirements of the State of Colorado program. If Contractor fails to comply with any
requirement of this provision or of C.R.S. §8-17.5-101 et seq., County, may terminate this
Agreement for breach, and if so terminated, Contractor shall be liable for actual and
consequential damages.
Except where exempted by federal law and except as provided in C.R.S. § 24-76.5-103(3), if
Contractor receives federal or state funds under the contract, Contractor must confirm that any
individual natural person eighteen (18) years of age or older is lawfully present in the United
States pursuant to C.R.S. § 24-76.5-103(4), if such individual applies for public benefits
provided under the contract. If Contractor operates as a sole proprietor, it hereby swears or
affirms under penalty of perjury that it; (a) is a citizen of the United States or is otherwise
lawfully present in the United States pursuant to federal law, (b) shall produce one of the forms
of identification required by C.R.S. § 24-76.5-101, et seq., and (c) shall produce one of the forms
of identification required by C.R.S. § 24-76.5-103 prior to the effective date of the contract.
29. Acknowledgment. County and Contractor acknowledge that each has read this Agreement,
understands it and agrees to be bound by its terms. Both parties further agree that this
Agreement, with the attached Exhibits A, B and C, is the complete and exclusive statement of
agreement between the parties and supersedes all proposals or prior agreements, oral or written,

and any other communications between the parties relating to the subject matter of this
Agreement.
IN WITNESS WHEREOF, the parties hereto have duly executed the Agreement as of
the day, month, and year first above written.

EXHIBIT A
SCOPE OF SERVICE
1. Contractor will provide behavioral health engagement, training, consultation, linkage,
and case management support for foster care families and children in Weld County, and
support and education for the County's Foster Care Coordinators (FCCs) and Kinship
Care Coordinators (KCCs).
2. Planning Period:
a. During the first 120 days of this Agreement, there will be a planning period for
the Contractor and the County to work together to identify specific deliverables,
quantitative and qualitative outcomes, and assessment tools used to measure the
success of this agreement.
b. The Contractor and the County staff will meet a minimum of every two weeks,
during the planning period and provide regular updates to the County Director of
Human Services and the Executive Director of North Range Behavioral Health on
progress, identified objectives and deliverables, and methods of measurement.
c. At the completion of the planning period, a report will be completed by the
Contractor and the County staff, detailing the specific deliverables, quantitative
and qualitative outcomes, and assessment tools to be used to measure the success
of the project. A report will be completed by August 1, 2018.
d. The mutually agreed upon results and deliverables of the planning period will be
incorporated in Exhibit A, Scope of services, through written concurrence of the
Director of the Weld County Department of Human Services and the Executive
Director of North Range Behavioral Health.
e. If mutually agreed upon results and deliverables are not able to be attained during
the planning period the parties will meet to determine a new timeline.
3. The following specific services will be available under this Agreement:
a. Consultation
b. Education
c. Skilled observation of families and children
d. Individualized strategies and early identification of behavioral health,
developmental and behavioral needs of children and youth
e. Support and education for Foster Parents and Foster Care Coordinators (FCCs) ad
Kinship Care Coordinators (KCCs).
4. Services will be provided at the Weld County Department of Human Services (WCDHS),
Division of Child Welfare offices, in a foster care family home, or at an alternate agreed
upon location, by a Foster and Kinship Care Engagement Specialist identified and
employed by the Contractor, herein referred to as the "employee". Refer to Exhibit C,
Job Description.

5. Employee, will:
a. Serve as Liaison between the North Range/DHS representatives and foster
families who experience difficulty with the foster care experience or are at risk for
disruption in foster care placement;
b. Work with DHS to identify those youth/families who require outreach;
c. Devise and implement strategic and individualized outreach plans;
d. Engage foster care families in a comprehensive plan to prepare, stabilize,
maintain, or strengthen the foster care placement;
e. Facilitate communication between families, services, and community partners;
f. Reduce external barriers by providing systems' navigation supports;
g. Coordinate referrals and support engagement to increase treatment adherence;
h. Provide parenting psychoeducation and support groups;
i. Behavioral health consultation and education for parents, foster parents and DHS
staff;
j. Assist DHS representatives to identify options for providing parent support;
k. Provide consultation and feedback to DHS team regarding both general health
issues and challenges specific to identified youth;
1. Partner on community presentations or projects to expand foster care program;
m. Refer and coordinate with other clinical staff regrading ancillary services:
i. Direct care, including outpatient and intensive assessment and
psychotherapy;
ii. Substance use disorder treatment;
iii. Medication management;
iv. Crisis stabilization and hospital diversion/step-down services.
6. Contractor will collaborate with the WCDHS to target both current and potential foster
care families, and County staff to strengthen behavioral health workforce development.
Contractor will partner to expand the scope of support within in the community, helping
to develop a larger system of highly engaged, highly trained, and highly retained Weld
County foster care families.
7. All services under this Agreement will be overseen/supervised by a licensed professional.
8. Duration of Service: Services shall remain in place so long as the family is an identified
foster care provider or Foster or Kinship Care Coordinator for the County.
9. Goals of service:
a. Partner with the County to engage, empower, train and retain high quality foster
and kinship care families;
b. Equip foster and kinship care families with the skills and resources needed to
independently address the difficulties they are having managing children in their
care, and the overall foster and kinship care experience; and
c. Provider behavioral health training and support for County staff.

10. Outcomes of service:
a. Increase in foster and kinship parent comfort and skill in managing behavioral
health situations; and
b. Decrease in foster and kinship care placement disruption; and
c. Facilitate 1-2 psychoeducation or support groups per week; and
d. Facilitate a minimum of quarterly parent and County staff training on agreed upon
behavioral health topics.
11. Reporting:
a. Contractor shall produce and submit monthly a report to the Department, that at a
minimum, provides the following information:
i. Names of families served;
ii. Number of contacts and number of hours provided per family and/or child;
iii. Written recommendations and/or plans specific to a child;
iv. Topics, hours, and number of individuals involved in training and/or
consultation activities with Department staff; and
v. Any other mutually agreed upon data. Such additional data will be
determined by no later than the third monthly meeting following the
execution of the Agreement.
12. Responsibilities of the County:
a. The County, as the Lessor, will provide "Office Space", located in the Weld
County Department of Human Services Building at 822 7th Street, Greeley,
Colorado, 80631, hereinafter referred to as "the Premises," until such time that the
County no longer offices at 822 7th Street, Greeley, Colorado, 80631, as follows:
i. County, as the Lessor, will provide one work space to the Contractor, as
Lessee, for the employee, which will be located at 822 7th Street.
ii. In consideration of leasing of the aforesaid, the parties covenant and agree
as follows:
1. The County will charge no rent to Contractor for use of the Office
Space;
2. Contractor agrees to use ordinary care when using the Office
Space;
3. Contractor may not sublet any part of the Office Space;
4. Contractor may not use the Office Space for any purpose other
than conducting the business and duties under this Agreement;
5. Contractor shall keep the Office Space clean and in sanitary
condition;
6. Contractor shall allow County any reasonable hour of the day to
enter into or upon the Office Space.
7. Contractor shall be responsible for loss or damage of equipment.
Any time equipment is assigned to a Contractor, who is acting in
his or her capacity under this Agreement, is lost or damaged, the
situation will be assessed on a case -by -case basis with regard to
whether the employee will be issued replacement equipment and/or

whether Contractor will be required to reimburse the County for
the lost or damaged equipment.
8. The County shall pay all utilities and trash removal expenses.
iii. Furthermore, the parties hereto agree as follows:
1. That no assent, express or implied, to any breach of any one or
more of the covenants or agreements hereof shall be deemed or
taken to be a waiver or any succeeding or other breach;
2. That, to the extent permitted by law, Contractor agrees to
indemnify, save, and hold harmless The County from any and all
loss, injury, or damage whatsoever, to all office equipment,
furniture, equipment, office supplies and other personal property
owned by Contractor or its employees as the result of fire, flood,
inclement weather, or other casualty, other than by negligence of
The County;
3. That the County shall own and maintain the filing cabinets and
computer systems including computers, modems, printers, and
associated computer equipment, used in the Office Space, unless
provided by Contractor;
4. That Contractor's employee shall escort all clients to and from the
Office Space and reception area;
5. The County shall pay for the use of the telephones and all
miscellaneous office supplies used by Contractor, including
photocopying for purposes of functions under this Agreement only,
while the Contractor is on the County premises.
b. The County shall work with the employee in foster/kinship care team and help the
employee develop a clear understanding of expectations and duties.
c. The County shall appoint a WCDHS supervisor to work with the employee and
Contract Coordinator.
d. The County shall evaluate and assess the performance and interactions of the
employee through feedback from staff and foster/kin providers.
e, The County shall provide co -supervision of the employee and the direct
supervisor monthly, to occur at NRBH or other agreed upon location.

2. Court Testimony:
a. The County agrees to reimburse the Contractor for Court Testimony so long as
the Contractor is subpoenaed by the County. Reimbursement will occur at a rate
of $30.00/15 minutes, and this will shall apply to Expert Witness testimony,
preparation time and travel time.
b. Contractor will, on behalf of its employees and/or officers, accept any subpoena
for testimony from Weld County by e-mail and will return a waiver of services
within 72 business hours. For this purpose, Contractor will designate an e-mail
address prior to the start of this Agreement. If the Contractor receives a subpoena
via e-mail address but will only accept personal service, the Contractor will
contact the Weld County Attorney's Office immediately at (970) 352-1551,
x6503, and advise that the subpoena must be personally served.

3. Funding and Method of Payment:
a. The County agrees to reimburse the Contractor in consideration of the work and
services performed under this Agreement at the rates specified above in Paragraph
1, Budget, and Paragraph 2, Court Testimony. The total amount to be paid to the
Contractor during the term of this Agreement shall be reported by the County
after December 31, 2018.
b. Expenses incurred by the Contractor prior to the term of this agreement are not
eligible expenditures and shall not be reimbursed by the County.
c. Payment pursuant to this Agreement, whether in whole or in part, is subject to and
contingent upon the continuing availability of said funds for the purposes hereof.
In the event that said funds, or any part thereof, become unavailable as
determined by the County, County has the right to terminate this Agreement, with
or without cause on thirty (30) days written notice, or amend accordingly.
4. Fees for Services:
a. Services rendered will be reimbursed at rates specified above in Paragraph 1,
Budget, for the term of this Agreement. Rates will be prorated for any term less
than 12 months.
b. Maximum reimbursement under this agreement shall not exceed $87,464.00 for
the term of this Agreement.
5. Submittal of Vouchers:
a. Contractor shall prepare and submit an invoice by the 15" of each month to the
County to certify that the services authorized were provided on the date(s)
indicated and the charges made were pursuant to the terms and conditions of
Exhibit A.

POSITION SUMMARY
Provides on -going assessment, crisis intervention, individual, family, and group therapy and case
management services. Develops strength -based service plans, monitors treatment progress, documents
interactions and maintains electronic clinical records and other reporting data for individuals with mild
to severe mental illness and/or substance use disorders. Coordinates with internal as well as external
supportive services to deliver needed services to clients. Collaborates with clinical supervisor, other
team members and psychiatrist regarding treatment.
DUTIES PERFORMED
1. Adhere to utilization, quality improvement and corporate compliance policy and standards.
2. Provide services to clients as assigned by administrative/clinical supervisor. May work with specific
age group and/or level of care based on program assignment.
3. Provide person -centered assessment, goal planning, and treatment for mental health and/or
substance use issues. Provide screening, intakes, and assessment of clients utilizing required
instruments and formats within appropriate time lines.
4. Develop strength -based written treatment plans/updates and continuing care plans and make
appropriate referrals. Complete written discharge summaries and correspondence within
established time lines.
5. Observe clients in a variety of situations. Assess diagnosis and mental status of clients, evaluate
current level of functioning, and develop and implement treatment/service plan to address clients'
needs.
6. Intervene and de-escalate crisis situations with clients. Provide crisis intervention via telephone or
face-to-face in a variety of settings that may include crisis walk in services, mobile on -site services,
residential settings, etc. Assess risk of harm to self or others and arrange needed resources.
7. Collaborate with other clinicians, law enforcement, external providers, and psychiatrists to provide
services to clients. Maintain close and cooperative relationships with community agencies and other
public agencies in carrying out treatment/service plans.
8. Collaborate continuously with assigned clients in order to devise safety planning, provide symptom
management, identify support systems, reduce and mitigate risk factors. May deliver services to
clients in their homes or other settings in the community.
9. Assist individuals in improving their well-being and quality of life by training in behavioral skills;
educate clients to make informed choices; develop clients' coping skills; help clients to problem
solve and negotiate tasks of daily living.
10. Learn, utilize, and support fidelity of any evidence -based practices within assigned program to
ensure effective interventions with clients.
11. Perform case management services as needed to assist clients to meet their goals as stated in
his/her service and/or treatment plans. Refer clients to internal and external supportive services
