SUBJECT: SAM Gov Registration Processing
In order for the county to submit some of our grants we have to sign into a portal called SAM. I
have been the authorized agent for this process since I became the controller. They are now
asking that we sign the attached letter and send back to them to continue with the access to the
site.

Purpose of Letter
The purpose of this letter is to formally appoint an Entity Administrator for the named Entity and to
attest to the accuracy of the information contained in the entity registration
