RE: Amendment to Approved List of Out -of -Home
Providers
On September 10, 2018 the Board of County Commissioners approved by resolution, Tyler
ID 2018-2873, a standard form of agreement for Individual Provider Contracts for Purchase of
Foster Care Services in a Foster Care Home and approved contracting with providers identified
in the attached list to the contract form. Some of the providers approved for contracts have
opted -out of certification for the fiscal year 2018-2019 and we will not be contracting with them.
The providers to be removed from the approved list are:
1. Salazar, Clara and Michael
2. Schweitzer, Shyanna and Nevin
3 Lechman, Pamela and Jay
4. Long, Kiley and Cole, Andrew
5. Westra, Michael and Geneva
Please update the approved list of Out -of -Home providers to reflect the above information.

Good Morning Esther,
Tyler id 2018-2873, includes Schweitzer, Nevin and Shyanna, on the list of providers. It has been
noted this provider closed July of 2017, and should not have been included for the 2018-2019 list of
providers. What, if any, action needs to be taken for update with the Board? Please advise.
