Grizzly Petroleum Company, LLC (Grizzly), a subsidiary of Great Western Operating Company,
executed a Wellbore Assignment of Oil and Gas Leases with Extraction Oil & Gas, Inc. (Extraction). This
Wellbore Assignment is inclusive of Extraction's RBF 1-11 wells located in Sections 21 and 22: N/2,
Township 6 North, Range 67 West, 6th P.M. Within this Wellbore Assignment, Grizzly assigned the
subject Weld County, Colorado Lease to Extraction, limited to the RBF wellbores.

Grizzly hereby requests the consent of the Board of County Commissioners of Weld County, Colorado to
the Wellbore Assignment of the subject Oil and Gas Lease, described herein on Exhibit "A", from Grizzly
to Extraction. Additionally, please find attached a check in the amount of One Hundred dollars ($100.00)
as required for the consent.
Please so indicate your consent of this Wellbore Assignment by signing in the space provided below and
return an executed copy to Grizzly as soon as possible. Thank you in advance for your cooperation in this
