RE: APPROVE 2018-2019 AREA AGENCY ON AGING OUTREACH SERVICES VENDORS
LIST AND RATES
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with 2018-2019 Area Agency on Aging
Outreach Services Vendors List and Rates between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, and various vendors, as listed on the attached list, and
WHEREAS, after review, the Board deems it advisable to approve the 2018-2019
Purchase of Service Agreement Amendment for Area Agency on Aging Outreach Services
Vendors List and Rates, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the 2018-2019 Area Agency on Aging Outreach Vendors List and Rates
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and various
vendors, be and hereby are, approved.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 24th day of September, A.D., 2018.

Please review and indicate if you would like a work session prior to placing this item on the
Board's agenda.
Request Board Approval of the Department's Fiscal Year 2018-19 Purchase of Service
Agreement Amendments for the Area Agency on Aging (AAA) Outreach Services. AAA
wishes to continue to purchase outreach services through Older Americans Act funding. The
vendors noted below were initially brought under contract in FY2015-16 and have been renewed
each year thereafter (FY2016-17 and FY2017-18). They are being renewed a third term for the
period of July 1, 2018, through June 30, 2019, for the services and amounts noted below.

I've been verifying some tracking on our end and have found two more PAs that do not appear to have been put on
agenda or adopted through formal resolution. Please see the attached threads where these items were forwarded to
the CTB, as well as the approved PAs in question. All of the associated contracts noted on these PAs have gone through
as Consent, similar to the Respite and Compass items recently remedied. I'm wondering if these two were also missed.
