RE: APPROVE 2018-2019 CHILD WELFARE COMPASS PROGRAM VENDORS LIST AND
RATES
WHEREAS, the Board of County Commissioners of Weld County. Colorado. pursuant to
Colorado statute and the Weld County Home Rule Charter. is vested with the authority of
administering the affairs of Weld County. Colorado. and
WHEREAS, the Board has been presented with the 2018-2019 Child Welfare Compass
Program Vendors List and Rates between the Weld County Department of Human Services. and
various providers. as listed on the attached provider list. and
WHEREAS, after review. the Board deems it advisable to approve the 2018-2019 Child
Welfare Compass Program Vendors List and Rates. a copy of which is attached hereto and
incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado. that the 2018-2019 Child Welfare Compass Program Vendors List and Rates
between the Weld County Department of Human Services, and various providers, be and hereby
are. approved.
The above and foregoing Resolution was. on motion duly made and seconded. adopted
by the following vote on the 5th day of September. A.D.. 2018, nunc pro tunc July 1. 2018.

RE: Compass: Weld County Family Resource Team Partners and
Rates for the 2018-19 Program Year
Please review and indicate if you would like a work session prior to placing this item on the Board's
agenda.
Request Board Approval of the Department's Compass: Weld County Family Resource Team
Partners and Rates for the 2018-19 Program Year. The Compass: Weld County Family Resource
Team is a program within the Department of Human Services Family Resource Division that partners
with local community agencies, including school districts, community mental health, public health, and
faith -based entities. The program focuses on keeping children at home in a safe, stable and nurturing
family environment; improving parenting capacity and family functioning; improving children's well-
being; and preventing families from entering, or re-entering, the child protection or juvenile justice
system, thus preventing unnecessary out -of -home placement.
The attached document reflects the partners who have agreed to continue to provide services for the
period of June 1, 2018, through May 31, 2019, and their associated rates. Upon approval of the Board,
agreement amendments will be entered into extending the existing agreements for the 2018-19 program
year.
I do not recommend a Work Session. I recommend approval of these providers and rates, and approve the
Department to move forward with agreement amendments.
