RE: APPROVE 2018-2019 CHILD WELFARE RESPITE CARE CONTRACT PROVIDER
LIST AND RATE
WHEREAS, the Board of County Commissioners of Weld County. Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado. and
WHEREAS, the Board has been presented with 2018-2019 Child Welfare Respite Care
Contract Provider List and Rate between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Human
Services. and various providers. as listed on the attached provider list. and
WHEREAS, after review, the Board deems it advisable to approve the 2018-2019 Child
Welfare Respite Care Contract Provider List and Rate. a copy of which is attached hereto and
incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the 2018-2019 Child Welfare Respite Care Contract Provider List and Rate
between the County of Weld. State of Colorado, by and through the Board of County
Commissioners of Weld County. on behalf of the Department of Human Services, and various
providers, be and hereby are. approved.
The above and foregoing Resolution was, on motion duly made and seconded. adopted
by the following vote on the 5th day of September. A.D., 2018.

RE: Child Welfare 2018-19 Respite Care Providers
Please review and indicate if you would like a work session prior to placing this item on the
Board's agenda.
Request Board Approval of the Department's Child Welfare 2018-19 Respite Care
Provider Agreements. The list below reflects the providers that have elected to continue to
provide respite care services for the term of July 1, 2018, through June 30, 2019. These
individuals and couples, most of whom are Weld County Certified foster parents, have met
certification requirements. Respite care is limited to four (4) hours per week per child. The hours
of care may be provided in any combination throughout a month, but may not exceed 16 hours
per month. Payment is $16.75 per each four (4) hour period, but may not exceed $67.00 per
month for 16 hours. Upon Board approval of this pass -around, individual signed agreement
amendments (renewals), will be obtained from providers and submitted through the Contract
Management System (CMS) for final Board approval.
