Over the years, we have built a solid reputation as an agency and we always do our best to maintain that reputation with integrity. We provide ethical and effective psychological services for individuals, couples, families and a variety of human service agencies in Weld County. We developed our Lifeskills and Mental Health programs to serve client populations in Weld County through the Department of Human Services. The Lifeskills program consists of therapeutic visitation and parent coaching for families that have children in foster care or families who are at risk of an out -of - home placement. In this program, we provide services at our offices, in the community and in -home, depending on the needs of the family and the circumstances of each case. We continually assess parent -child interactions, parent capabilities and life skills development with the goal of reunification and preservation of families. In the mental health program, we offer services geared towards helping clients overcome barriers to safe and healthy family interaction and parent -child relationships. Our mental health program is designed to use brief therapies that help clients make the necessary changes in order to reach treatment goals. The mental health program incorporates many modalities of treatment including solution - focused therapy, EMDR, couples therapy, and cognitive behavioral therapy. We provide services to children, adolescents, and adults. Transitions therapists work closely with caseworkers and other providers who are involved in each case in an effort to coordinate care. We use a tailored approach in working with all of our clients that emphasizes their personal goals and the treatment goals set by WCDHS. Mental Health Services: Family, Couples and Individual Counseling The Transitions Mental Health Services program offers family, couple and individual counseling services customized to meet the needs of the family as well as the needs of the Department of Human Services and the court. Transitions utilizes our signature team approach to link family, couple and individual services with existing programs such as Transitions Lifeskills and other community services in order to provide a continuum of care delivered at the opportune time and at the most appropriate level of service. Counseling services address family system dynamics and individual issues that impact the safety and well-being of children. Transitions therapists use brief therapy models that are solution oriented and geared towards completing services in a 3-6 month time frame. Services are proactive with the goal of preventing out -of -home placement, during placement to address safety, mental health and relationship issues or during the reunification process in order to promote family stability and permanency. Transitions Psychology Group, LLC FY2017-18 Mental Health Services 3 Transitions believes in using strength based approaches to family counseling designed to enhance the overall functioning of the family. Family characteristics such as communication, togetherness, flexibility, family structure, family lifecycle, multigenerational issues, interactions and resiliencies are assessed and treated. Couples issues and tension often spill over into parenting and can have an impact on behavior in children and overall functioning of the family. Therefore, it is often necessary to address couple issues in order to facilitate long-lasting, systemic change in families. Couples therapy builds on the strengths and resiliency of the couple in order to correct certain deficit areas that undermine the family foundation. Individual counseling services are provided to adults, adolescents and children to address a wide variety of issues that impact the individual and family. The aim of this service is to remove any barriers to safety and well-being in the home which may include but not limited to behavior problems, depression, anxiety, stress, trauma, grief and loss, health, self-image, character and treatment compliance. EMDR (Eye Movement Desensitization and Reprocessing) is a powerful process designed to resolve memories of a past difficult emotional experience affecting current functioning. EMDR is extensively researched and shown to be effective in the treatment of trauma. EMDR is used with children, adolescents and adults and may be used to work through stressors such as abuse, addiction, anxiety, life stress, chronic pain and disturbing memories. EMDR may be used in conjunction with, or independent of, other services. Therefore, the system, which can be a family, a group of individuals or an organization, holds the power for change and has the ability to find solutions to any problems that may arise by examining the relationships within that system. Using this framework, I work to maximize the system's power in its ability to change by looking at the unique web woven by these relationships. As each member's contribution to both the problem and the solution, as well as openness for change are assessed, the system can be brought together in such a way where everyone becomes an active member in searching for, and finding creative answers, for a more permanent change.
