RE: APPROVE GRANT PARTNERSHIP AGREEMENT FOR PATHWAYS TO SUCCESS
(ADULT EDUCATION AND LITERACY ACT GRANT) AND AUTHORIZE CHAIR TO
SIGN - AIMS COMMUNITY COLLEGE
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Grant Partnership Agreement for
Pathways to Success (Adult Education and Literacy Act Grant) between the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of
the Department of Human Services, Weld County Employment Services, and Aims Community
College, commencing July 1, 2017, and ending June 30, 2020, with further terms and conditions
being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Grant Partnership Agreement for Pathways to Success (Adult
Education and Literacy Act Grant) between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, Weld County Employment Services, and Aims Community College be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 8th day of January, A.D., 2018, nunc pro tunc July 1, 2017.

Request Board Approval to Sign the Aims Community College Adult Education and Literacy Grant
Pathways to Success Grant Partnership Agreement. Aims Community College in Greeley proposes to
implement Pathways to Success, a three-year program (July 2017 — June 2020), through the Adult Education
and Literacy Grant funded by the Colorado Department of Education. The overarching goal is to increase the
number of low-income, low -literate adults who increase basic educational skills necessary to compete in today's
workforce. Employment Services of Weld County (ESWC) has been a partner in this initiative since its
inception, in 2014. To support the grant initiative, ESWC will facilitate ten (10) employment related workshops
at a reimbursement rate of $100 per workshop.
I do not recommend a Work. Session. I recommend approval for Tami Grant, Director of Employment Services,
to electronically sign the Grant Partnership Agreement.

Grant Description/ Purpose/ Summary:
Aims Community College (Aims) in Greeley, Colorado, proposes to implement Pathways to Success —a three-year
program (July 2017 -June 2020) through the Adult Education and Literacy Grant with the overarching goal of increasing
the number of low-income, low -literate adults attaining their literacy and numeracy skills needed to join the workforce
and be successful in obtaining a self-sufficiency wage.
The program will identify and recruit eligible adults from the underserved Greeley area, assess their literacy and
numeracy skills, and create an individualized plan of action for each adult, with the long-term goal for each participant
to achieve self-sufficiency by being postsecondary or workforce ready. The services provided will include: (1) pre -
assessment activities for each student to gauge his or her preparedness for GED and the career pathways goals; (2)
instruction for the GED exam; (3) academic advising, mentoring and coaching by a case manager; (4) monitoring of
student progress through an individual action plan; (5) administering the GED exam; (6) providing career skills training
in collaboration with partners; (7) enrollment in the Workforce Innovation and Opportunity Act (WIOA) Adult,
Dislocated Worker or Youth program, facilitated by ESWC, for extra support (i.e., aid in finding jobs suited for
qualifications, internships, career workshops, job applications, etc.); (8) career -related skills workshops provided by
ESWC (i.e., resume and cover letter writing, interview skills, mock interviews, career counseling, etc.); (9) workshops
related to work ethics and teamwork through the Aims Center for Workforce and Innovation; (10) preparing students
for the Accuplacer tests, aid in college enrollment, and financial aid and remediation courses through Aims and the
Educational Opportunity Center; (11) workshops from Right to Read for the students and their families, including
family literacy, family financial literacy; and (12) transportation and child care assistance.

Terms and Conditions
This agreement is made and entered into by and between Employment Services of Weld County (ESWC), hereinafter
referred to as GRANT PARTNER and Aims Community College, hereinafter referred to as PRIME RECIPIENT.
Terms and conditions:
• ESWC, the Workforce Center, certified by the Colorado Department of Labor and Employment, will enable
the participants to enroll in their specialized programs: Workforce Innovation and Opportunity Act (WIOA),
Dislocated Worker or Youth. The participants will take advantage of multiple benefits, including workshops
on resume building seminars, interviewing and transferable skills workshops, and job applications building
courses. But most importantly, this partner will aid the participant in finding and receiving adequate
employment. All students will be able to enroll in the different programs offered by ESWC. Just by being
enrolled, participants will benefit from the resources, job fairs, partnerships, job and internship opportunities,
as well as the workshops provided on career readiness. ESWC also offers low-cost workforce -ready
certifications, career focused and information technology skills certifications that participants could take
advantage of.
• ESWC will provide 10 workshops per year, priced at $100 per workshop. ESWC will invoice Aims
after the workshops are performed. There will be no prepaid amount for the workshops.
• Pathways to Success staff will coordinate quarterly meetings with the partners to discuss the student
progress and how things can be improved. The case manager, in collaboration with the project
director, will provide each partner a monthly list of the current participants, to ensure transparency
and that all participants can be helped immediately. During the quarterly meetings, the partners will
provide Aims a summary report of the students they have served and what activities were performed.
These reports will be used to fill in the progress reports for each student and better assess their program
standing by the case manager.
• "Personally identifiable information (PII) means information that can be used to distinguish or trace
an individual's identity, either alone or when combined with other personal or identifying information
that is linked or linkable to a specific individual. Some information that is considered to be PII is
available in public sources such as telephone books, public Web sites, and university listings. This
type of information is considered to be Public PII and includes, for example, first and last name,
address, work telephone number, email address, home telephone number, and general educational
credentials. The definition of PII is not anchored to any single category of information or technology.
Rather, it requires a case -by -case assessment of the specific risk that an individual can be identified.
Non-PII can become PII whenever additional information is made publicly available, in any medium
and from any source, that, when combined with other available information, could be used to identify
an individual." (2 CFR 200.79) "Protected PII means an individual's first name or first initial and last
name in combination with any one or more of types of information, including, but not limited to,
social security number, passport number, credit card numbers, clearances, bank numbers, biometrics,
date and place of birth, mother's maiden name, criminal, medical and financial records, educational

transcripts." (2 CFR 200.82) The partner agrees to protect any PII collected for the purpose of this
program. The partner will not share any PII through unsecured methods (email, unencrypted flash
drives, etc.)
• The entire framework for the Pathways to Success program is based on the seven components for
career pathways, as explained in the table below.
