HomeMy WebLinkAbout20181765.tiffRESOLUTION RE: APPROVE AMENDMENT #3 TO SUBCONTRACT AGREEMENT FOR ASSESSMENT PROCESS AND AUTHORIZE CHAIR TO SIGN - TRUVEN HEALTH ANALYTICS, LLC WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with an Amendment #3 to Subcontract Agreement for Assessment Process between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and Truven Health Analytics, Inc., commencing April 1, 2018, and ending September 17, 2018, with further terms and conditions being as stated in said amendment, and WHEREAS, after review, the Board deems it advisable to approve said amendment, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Amendment #3 to Subcontract Agreement for Assessment Process between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and Truven Health Analytics, Inc., be and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said amendment. This amendment to a subcontract entered into between Area Agency on Aging (AAA) and Truven Health Analytics Inc. (Testing Experience and Functional Tools (TEFT), Tyler #2016-3423) will allow AAA's Options for Long Term Care program to continue to participate in testing of a new assessment tool, the Functional Assessment Standardized Items (FASI), and to receive reimbursement for follow up work on previously completed assessments with Long Term Care clients. May 9. 2018 - CMS ID '809 Lelt5 Page I 2018-1765 H ROO8'f Amendment #3 CMS-TEFT TA -WELD by and between Truven Health Analytics LLC, an IBM Company and Weld County Area Agency on Aging This third amendment is made to the Subcontract by and between Truven Health Analytics LLC, an IBM Company ("Truven Health"), doing business at 100 Phoenix Drive, Ann Arbor, Michigan 48108 and The Weld County Board of County Commissioners on behalf of Weld County Area Agency on Aging ("Subcontractor") with offices at 315 North 11th Avenue, Building C, PO Box 1805, Greeley, Colorado 80631. Individually, Truven Health and Subcontractor may be referred to as "Party" or collectively as "Parties." WHEREAS, the Parties entered into a Subcontract on November 10, 2016, for services to be performed pursuant to Truven Health prime contract number HHSM-500-2010-000251, Medicaid and CHIP Policy Implementation and Evaluation (MACPIE) Task Order HHSM-500-T0006 with the Centers for Medicare and Medicaid Services ("CMS" or "Client") for the TEFT Technical Assistance ("Project"); and WHEREAS, the Parties wish to authorize Task 7 a standalone task part of the Project; NOW THEREFORE, the Subcontract is amended as follows: 1. Subcontractor's Services. Pursuant to the Article 1, Subcontractor's Services, Attachment 1-7, Statement of Work- Task 7, is attached and incorporated herein. 2. Manner of Payment. Pursuant to Article 1, Paragraph c. Manner of Payment, the clause is revised for Task 7 as follows: Subcontractor shall prepare and submit to Truven Health an invoice by the fifteenth day following each month for complete FASI assessments. An assessment is considered complete when the assessor obtains the participant's consent or assent to participate in the study, conducts the in -home FASI assessment and submits the FASI data collection tool in accordance with Attachment 1-7. Subcontractor shall reference the Truven Health 155136 for the assigned work for Task 7. The invoice shall be sent to IBM Watson Health, Truven Health 4th Floor, Payment and Value Neighborhood, 75 Binney Street, Cambridge, Massachusetts 02142, Attention: Pauline Goulart, or via email to pgoulart@us.ibm.com, with a copy to Lisa Gold, lgold@us.ibm.com. Truven Health shall pay Subcontractor within thirty (30) days after receipt and approval of the invoice. 3. Compensation. Pursuant to Article 1, Paragraph a. Compensation, the total not to exceed value for work under this Subcontract Amendment for Task 7 is $18,000.00. 4. Term. Weld County Area Agency on Aging ("Subcontractor") shall be responsible for work required to successfully complete the tasks listed below during Task 7 of the prime contract from April 1, 2018 through September 17, 2018. Introduction: The purpose of this effort is to test two Functional Assessment Standardized Item (FAST) based performance measures (PMs) in Community -Based Long -Term services and supports (CB-LTSS) settings. The FASI is a reliable and valid set of assessment items used for assessing the need for assistance with daily life activities among individuals receiving or applying for CB-LTSS. The FASI builds on national efforts to create standardized, validated, and exchangeable data for use across all care levels. The FASI also includes a process for assessors to discuss and document up to two personal priorities each for self -care, mobility, IADLs, and living arrangements that the individual within CB-LTSS programs wishes to accomplish or improve over the next 6 -months. Reviewing and recording priorities is important for developing a person -centered service plan(PCSP) and can be used to help update and improve the PCSP. This process provides an opportunity for the individual to voice an opinion about what will help improve his or her quality of life and to identify assistance and/or services needed to align with his or her goals, priorities, and needs. Truven Health and subcontractor George Washington University (hereinafter referred to as the FASI team) under contract with CMS, are working on the next phase of FAST to test FASI-based performance measures for CB-LTSS. The goal of this effort is to submit two tested PMs to the National Quality Forum for endorsement. Two FASI-based PMs, drafted as process measures, will be tested in this next phase of FAST. 1. PM # 1 - Percentage of individual 18 years or older who received community -based long-term services and supports (CB-LTSS) with documented needs determined by a Functional Assessment Standardized Items (FASI) assessment AND who have identified at least 3 personal priorities related to self -care, mobility or IADL functional needs within the reporting period. 2. PM # 2 - Percentage of individuals 18 years or older who received Community -Based Long -Term Services and Supports (CB-LTSS) with documented functional needs as determined by the Functional Assessment Standardized Items (FASI) assessment AND documentation of a person - centered service plan (PCSP) that addressed identified functional needs within the reporting period. Tasks: Appropriate staff experienced with the PCSP process (case managers, supervisors, administrators, etc.) will conduct case record reviews in May and June, 2018 to gather data related to the 2 PMs. Staff participating in this effort (hereinafter referred to as 'reviewers') will also submit feedback on these 2 PMs via anonymous surveys. Entities are encouraged to select as many reviewers as possible to participate in this effort. Feedback from individuals with experience in CB- LTSS programs is a critical component of this effort. I. Review Process Subcontractor shall: Fruven Health Analy tics LI C. an IBM Company CONFIDENTIAL Page 3 of4 1. Select reviewers who are experienced with the PCSP process; 2. Identify one lead/manager to meet with the FASI team as needed but at least monthly throughout the contact period; 3. Ensure that each reviewer and lead/manager participating in the FASI Performance Measure (FASI PM) testing attends a 2 -hour training provided by the FASI team via webinar prior to conducting record reviews to test these 2 PMs; 4. Assign 2 reviewers (set of reviewers may vary) to review completed FASI assessment forms from the FASI field test and the associated PCSP at the time of the FASI assessment (hereinafter refer to as the 'record'); a. Two reviewers shall review each record, and each reviewer will submit a completed Data Abstraction Form provided by the FASI team for each record reviewed. i. FASI PM 1: Reviewers will record the number of self -care, mobility, and/or IADL personal priorities documented on the FASI for each record reviewed. ii. FASI PM 2: Reviewers will record self -care, mobility and IADL needs documented on the FASI and determine (according to the rules provided in the training) if the PCSP addresses the needs identified in the FASI assessment. iii. The entity shall submit 120 Data Abstraction Forms (2 for each 60 records reviewed). b. Each reviewer will complete and submit an anonymous feedback form (provided by the FASI team) to provide feedback on each of the 2 FASI-based PMs and to provide some details regarding the reviewer's professional experience. II. Participant Information Subcontractor shall: 1. Maintain data in a secure setting in accordance with the signed BAA (See Attachment 2); 2. Ensure that participant information (name, address, telephone number) is not included on any Data Extraction Forms or feedback forms; 3. Report suspected instances of Abuse, Neglect and Exploitation to appropriate state entity (See Exhibit B for State Policies and Procedures); 4. Inform the participant's case manager if a need identified in the FASI assessment was not addressed by the PCSP. Payment Terms: This is a firm fixed price type contract, with a total not -to -exceed value of $18,000. One complete review is $150. Truven Health is in no way liable to make payments to Subcontractor in excess of the funded value of this Subcontract. Costs incurred by Subcontractor in excess of the funded value are at Subcontractor's sole risk and expense. Should travel be approved, travel expenses must be within the Federal Travel Regulations limits and within the total not -to -exceed value.
