I am pleased to inform you that the Weld County Department of Human Services has been awarded $5,000. Due to additional funds available to support local recruitment and retention efforts, Weld County Department of Human Services has been awarded $1,000 above the $4,000 initially requested. These funds should be utilized to further your investment in the activities outlined in the application. You are not required to utilize this additional funding. Please respond to me via email (mary.gerlach@state.co.us) to notify me if Weld County Department of Human Services intends to use the additional funds. I would like to remind you of the following deadlines associated with this funding: • SFY19 Local Innovation Fund recipients that did not complete all of the paperwork requirements for the SFY18 Recruitment and Retention Grant are required to submit a progress update via email by February 1, 2019. This requirement does not apply to the Weld County Department of Human Services. • Funded effort/activity must be completed by April 30, 2019. • The deadline to enter expenditures into CFMS (counties only) is May 6. 2019. • Counties: The CFMS code for your grant is E468.9277 • The deadline to submit invoices (CPAs only) is May 6, 2019. ■ CPAs: Please email your invoices to mary.gerlach@state.co.us • You may submit for reimbursement after you have incurred an expense, even if the grant project is not yet complete. • Innovation Fund recipients must a summary article summarizing their recruitment effort on or before May 15, 2019. • Innovation Fund recipients must complete a brief online survey on or before May 15, 2019. If you have an issue meeting a deadline, please email prior to the deadline to discuss alternative arrangements. I would also like to remind you of the following guidelines for the Innovation Fund: • Informational materials and giveaways printed through the Local Innovation Fund must include the Colorado Department of Human Services logo. The appearance of the CDHS logo must be approved before items are printed.
