Water service is currently being served by Tap #748 to property located at Casagrande Estates and additional
water service can be made available to this property, provided all requirements of Central Weld County
Water District. Northern Colorado Water Conservancy District and the Bureau are satisfied. Please note that

no case will a customer be permitted to extend a service line from one parcel or property to another
Parcel to provide additional water service.
Currently the annual allocation for the 5/8" meter is being exceeded by almost twice the allowed amount,
The District recommends that the current meter be upsized to the next size \\ hicll is a 3/4" meter and would
increase your annual allocation or an additional Budget or 5/8 tap be purchased for the second house. A
study would need to be assessed to determine capacity on the existing line. We were not aware that two
houses were on the property. An additional dwelling unit charge of $3.5 0/month will have to be assessed to
the water service bill beginning November 1, 2017.
Central Weld County Water District requires that contracts be consummated within one (1) year from the
date of this letter or this letter shall become null and void unless extended in writing b) the District. The
District will not notify, by separate letter, any prospective landowners of cost increases. The District has the
right to review the annual allotment and compare it to actual usage to determine if additional raw water will
be purchased and transferred to the District by Tap Holder.
It is your responsibility to confirm with your local Fire Protection Authority if fire flow water capacity is
required for your project.

TERMS AND CONDITIONS
A permit fee, as set by separate ordinance of the Board of Weld County Commissioners, shall be required of applicants for onsite wastewater treatment systems,
payable at time of application_ Permit fees are non-refundable; permit applications are non -transferable. If both a building permit and an O''TS are issued for tt-
same property and construction is not commenced prior to the expiration date of the building permit, the OWr'S permit shall expire at the same time as the building
permit_ If an MIS permit is issued fora propedy that does not require a building permit, the OWT'S permit shall expire one year after its issuance if construction on
the septic system has not commenced. Any change in plans or specifications after the permit nas been issued invalidates the perrmt unless approval is secu red from
the Health Officer or his/her designated agent. Expired permits can be renewed by payment of the permit fee only if:
A. There has been no change in the plans and specifications of the proposed system as set out in the original application or such change is reviewed and
approved by a Division Representative.
F3. The surrounding land, its use or zoning has not changed so -as -to cause the original application not to be acceptable under these regulations..
NOTICE
The completeness of this application is conditional upon further mandatory and additional tests and reports as may be required by the Weld County Department of
Public Health and Environment (WCIDPHE) The issuance of the permit is subject to such terms and conditions as deemed necessary to insure compliance with rules
and regulations adopted under article 10 title 25, CRS as amended. The applicant certfies that the proposed system will not be located within 400 feet of a community
sewage system. The undersigned certifies that all statements made, information and reports submitted herewith are, or will be, represented to be true and correct to
the best of his/her knowledge and are designed to be relied on by the WCDPHE for evaluation for purposes of issuing the permit applied for herein. Applicant further
understands that falsification or misrepresentation may result in the denial of the application or revocation of any permit granted, and in legal action or perjury as.

CONDITIONS:
Description: New OWTS
Comment: Onsite wastewater treatment system must be 10 feet from property line. Install the soil
treatment area within 50 feet of the soils test location. Construct onsite wastewater treatment
system according to Engineer design. Septic tank must be at least 10 feet from any potable
water line, unless water line is encased. Soil treatment area must be 25 feet from any
potable water line, unless water line is encased. Sewer line to septic tank does must have at
least 1/8inch/ft. fall. Pump floats installed on discharge piping. Floats must be mounted on a
stem separate from the pump discharge piping to allow for removal, adjustment, and
replacement of the float without removing the pump. Need to install inspection ports at the
ends of each trench or bed. All Weld County regulations are applicable.

Public Health and Environment for reasons set forth in the Weld County Onsite Wastewater Treatment System Regulations including failure to meet
any term or condition imposed thereon during temporary or final approval. The issuance of this permit does not constitute assumption by the
department or its employees of liability for the failure or inadequacy of the sewage disposal system. This permit is non -transferable and
non-refundable. Before issuing final approval of this permit the Weld County Department of Public Health and Environment reserves the right to
impose additional terms and conditions required to meet our regulations on a continuing basis- Final permit approval is contingent upon the final
