RESOLUTION RE: APPROVE 2018-2019 AMENDMENTS FOR AREA AGENCY ON AGING IN -HOME VOUCHER PROGRAM PARTNER LIST AND RATES WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with 2018-2019 Amendments for Area Agency on Aging In -Home Voucher Program Partner List and Rates between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and various partners, as listed on the attached list, and WHEREAS, after review, the Board deems it advisable to approve the 2018-2019 Amendment for Area Agency on Aging In -Home Voucher Program Partner List and Rates, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the 2018-2019 Amendment for Area Agency on Aging In -Home Voucher Program Partner List and Rates between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and various partners, be and hereby are, approved. Request Board Approval of the Department's Fiscal Year 2018-19 Purchase of Service Agreement Amendments for the Area Agency on Aging In -Home Voucher Program Partners. The Weld County Area Agency on Aging In -Home Services Voucher Program provides assistance to eligible Weld County residents 60 years of age and older who require assistance with personal care and homemaking services. Personal care services include, but are not limited to, bathing, dressing, mobility, and hygiene assistance. Homemaking services include, but are not limited to, laundry assistance, dusting, vacuuming, and bathroom cleaning. Program participants can choose a provider of their choice from the list of contracted agencies at a rate of $25 per hour for up to three (3) hours per week per participant. On a case -by -case basis, some participants may receive additional hours, if funds arc available, to ensure their safety. The term for all agreements is July 1, 2018, through June 30, 2019. Please see the attached threads where these items were forwarded to the CTB, as well as the approved PAs in question. All of the associated contracts noted on these PAs have gone through as Consent, similar to the Respite and Compass items recently remedied. I'm wondering if these two were also missed. Thank you.
