RE: 2019-20 Child Protection Agreement for Services
Dear Ms. Wurth,
This letter serves as 30 -day written notice of termination, pursuant to Paragraph 19 of your current executed
original agreement with the Weld County Department of Human Services (WCDHS) for Home Studies and
Relinquishment Counseling. Your original agreement is identified by the Weld County Board of County
Commissioners as document number 2019-1898 and is enclosed. This termination is effective August 30, 2019.
The agreement is being terminated as WCDHS believes the quality of home studies you have submitted does
not meet the standards set forth by the State and required under your contract.
Any questions or concerns regarding this action may be directed to the Department's Contract Management
team at HS-ContractManagement a co.weld.co.us.
