HomeMy WebLinkAbout20190488.tiffRESOLUTION RE: APPROVE REVISIONS TO SECTION 2.000 CHILD WELFARE OF THE DEPARTMENT OF HUMAN SERVICES POLICIES AND GUIDELINES MANUAL WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with the Revisions to Section 2.000 Child Welfare of the Department of Human Services Policies and Guidelines Manual, and WHEREAS, after review, the Board deems it advisable to approve said additions, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Revisions to Section 2.000 Child Welfare of the Department of Human Services Policies and Guidelines Manual be, and hereby is, approved. Griego, Director, Human Services RE: Department of Human Services' Policies and Guidelines Manual Revisions Please review and indicate if you would like a work session prior to placing this item on the Board's agenda. Request Board Approval of the Department's Policies and Guidelines Manual Revisions. These policies were revised to reflect current processes and/or further define the Department's activities and responsibilities. The policies were developed by involved Department staff and were provided to Legal for review and comments. • Teleworking Program Guideline (Revised) This guideline was revised to outline the specifics surrounding the Teleworking Program, primarily for caseworkers. The guideline provides eligibility criteria and defines the conditions of such arrangement. This guideline includes Exhibit A, Teleworking Program Acknowledgment, and Exhibit B, Telework Program Request. 2.2.10. Transitioning Youth with Intellectual and Developmental Disabilities (Revised) This revised policy communicates the purpose for the transition of youth with Intellectual and Developmental Disabilities (IDD) and the transition requirements in place for staff to follow. The key points of the revision state youth who meet criteria and are pending SSI adult benefits in order to officially transition on to the adult Home and Community Based Services with IDD (HCBS-IDD) waiver can remain in the Department's custody and be placed on an Independent Living Stipend in order to move into their chosen adult host home. The Department may contract with the Program Approved Service Agency (PASA) overseeing the home to provide residential and supportive case management services, as needed, to maintain the youth's placement pending formal transition onto the waiver. Purpose Youth eighteen (18) years of age or older, being served through Child Welfar determined by the Community Centered Board (CCB) to meet intellectual an °F i ntal disability (IDD) criteria, must transition to Home and Community Based ;l D (HCBS-IDD). In extenuating circumstances, the Court or interdisciplinar determine that it is not in the best interest of the youth to transition. B. Transition Requirements 1.
