Below is an email notification received from Child Welfare staff communicating closure of a Weld County Foster Care
home — Callaway Home Provider # 1655848. The closure was officially on 11/19/2018. This home is tied to agreement
2019-0798, which was approved on Consent Agenda for February 20, 2019.
Please advise if there is anything further needed.

They had sent an email as we were trying to get them a contract and they advised that they were not going to renew
their contract as they had lost several family members and they did not feel that could give any kids the attention they
deserved. The email was on 11-19-2018. I closed them out of Trails on 11-19-2018.

I previous discussion via email it has been communicated that the Callaway home is closing. Upon closure, BOCC will
need a formal communication stating such, as this will officially be the means of terminating the agreement we finally
received their signature on. Please send to HSCM upon receiving notice of closure.
