In an effort to advance the health of patients and transform the delivery of prehospital health
care, UCHealth EMS submits this letter of notification per 6 CCR 1011-3 — Standards for
Community Integrated Health Care Services Agencies, to operate a Mobile Integrated
Healthcare Agency - Community Paramedic Program in Weld County.
The activities under this program will be directed towards integrating the services of a
community -based team of qualified, out -of -hospital community paramedics, advanced practice
providers, care management teams, and technology, in collaboration with our fire/rescue
partners, law enforcement partners, and mental health partners, and other community
resources, to assess and treat patients outside of the hospital setting for the purpose of
improving access to care for our most vulnerable populations.
UCHealth EMS believes we will be able to accelerate our commitment to make out -of -hospital
care more patient centric; grow our capabilities to manage the health of populations in the
prehospital field; and be able to deliver the right care to the to the patient, with the right type of
provider and technology in the field.
This MIH Program starts the next chapter in the legacies of service and innovation for
UCHealth EMS in Northern Colorado. It will not only make a profound difference in the lives of
Weld County residents and visitors; it will also serve as a national model for EMS delivery in
the field.
