RE: APPROVE AMENDMENT #2 TO SHORT-TERM LEASE AGREEMENT OF THIRTY (30)
APARTMENTS (HERITAGE 2 BUILDING, BONELL COMMUNITY, GREELEY,
COLORADO) FOR USE AS ALTERNATIVE CARE FACILITY AND AUTHORIZE CHAIR
TO SIGN - EVANGELICAL LUTHERAN GOOD SAMARITAN - BONELL COMMUNITY
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Amendment #2 to the Short -Term Lease
Agreement of Thirty (30) Apartments (Heritage 2 Building, Bonell Community, Greeley, Colorado)
for Use as an Alternative Care Facility between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, and Evangelical Lutheran Good
Samaritan — Bonell Community, commencing June 8, 2020, and ending October 1, 2020, with
further terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Amendment #2 to the Short -Term Lease Agreement of Thirty (30)
Apartments (Heritage 2 Building, Bonell Community, Greeley, Colorado) for Use as an Alternative
Care Facility between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, and Evangelical Lutheran Good Samaritan — Bonell Community,
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.

SECOND AMENDMENT TO SHORT-TERM LEASE AGREEMENT OF THIRTY (30)
APARTMENTS, HERITAGE 2 BUILDING, BONELL COMMUNITY, GREELEY,
COLORADO, FOR USE AS ALTERNATIVE CARE FACILITY
THIS AMENDMET TO SHORT-TERM LEASE AGREEMENT ("Lease") is made
and entered into thid ay o , 2020, by and between The Evangelical Lutheran Good
Samaritan Society d/b/a Good S itan Society — Bonell Community, whose address is P.O.
Box 1508, 708 22"d Street, Greeley, CO 80631("Lessor"), and the Board of County
Commissioners of Weld County, Colorado, whose address is P.O. Box 758, 1150 O Street,
Greeley, CO 80632 ("Lessee").
WHEREAS, on April 8, 2020, the parties hereto executed a Short -Term Lease Agreement of
Thirty (30) Apartments, Heritage 2 Building, Bonell Community, Greeley, Colorado, for Use as
Alternative Care Facility ("Original Lease"), with a termination date of June 7, 2020, and
WHEREAS, Lessee is making repairs to the Premises for it to be habitable with an expected
cost of $30,000 to $40,000, and
WHEREAS, Lessee will need further use of the Premises from June 8, 2020, to and until
October 1, 2020, and
WHEREAS, Lessor agrees to grant Lessee further use of the Premises and hereby extends
the Original Lease for no additional rental under the same terms and provisions set forth in the
Original Lease, except as amended herein.
WITNES SETE:
Paragraphs 2 and 3 of the Short -Term Lease Agreement of Thirty (30) Apartments, Heritage
2 Building, Bonell Community, Greeley, Colorado, for Use as Alternative Care Facility
("Original Lease"), executed by the parties hereto on April 8, 2020, are amended as follows:
3. Term. The term of this Lease shall be for a period of one -hundred seventy-six (177) days
from April 8, 2020, to and until October 1, 2020.
4. Rent. Lessee has paid the Lessor as rent for the use of the Premises the amount of
$18,000 for the use of the entire Premises for the entire term hereof. Rent shall include
all utilities, such as water, sewer and electricity. Trash removal shall be arranged and
paid for by Lessee.
All remaining paragraphs and exhibits to the Original Lease shall remain the same.

IN WITNESS WHEREOF, the parties hereto have executed or caused this Second
Amendment to Short -Term Lease Agreement of Thirty (30) Apartments, Heritage 2 Building,
Bonell Community, Greeley, Colorado, for Use as Alternative Care Facility to be executed as of
the day and year first -above written.
