Dear Structure Owner/Representative:
Burnco Colorado, LLC plans to amend the City of Fort Lupton's Waste Water Treatment Plant Site into the existing
Inouye Gravel Mine(M2018-038-AM01),shown on the attached Figures starting in summer of 2020. Per the Colorado
State Division of Reclamation, Mining and Safety(DRMS) Bumco Colorado, LLC is required to issue structure
agreements to all structure owners within two-hundred feet of the mine boundary. Please read the information attached
and sign and return the agreement using the self-addressed stamped envelope. If you would like a full copy of the slope
stability analysis report and figures or require more information please contact Andy Rodriguez at 303 833 1416x202.

authority under the Colorado Land Reclamation Act for the Extraction of Construction Materials and
the Colorado Mined Land Reclamation Act for Hard Rock,Metal, and Designated Mining Operations.
Any alteration or modification to this form shall result in voiding this form.

The Clerk to the Board's Office received your structure agreement for Colorado Division of Reclamation,
Mining, and Safety (DRMS) Reclamation Permit M2018-038-AM01. Weld County will not be executing this
agreement. Upon request, Weld County can provide our template structure agreement for execution, which
requires a slope study.
