SUBJECT: MAJOR ACTIVITY NOTICE SUBMISSIONS
IMPORTANT! !
Every municipality in the State of Colorado is statutorily required
to submit to the Colorado Land Use Commission MAJOR ACTIVITY NOTICES
pursuant to C.R.S. 139-59-25(1)(1972 Session Laws). This provision
reads as follows:
139-59-25. Major activity notice. (1) Whenever
a subdivision or commercial or industrial activity is pro-
posed which will cover five or more acres of land, the gov-
erning body of the municipality in which the activity is

posal prior to approval of any zoning change, subdivision,
or building permit application associated with such a pro-
posed activity. Such notice shall be in a standard form,
promulgated as a rule and regulation prescribed by the land
use commission and shall contain such information as the
land use commission shall prescribe.
A copy of the form needed for compliance with this statutory require-
ment as approved by the Colorado Land Use Commission in the manner
proscribed by the Colorado Administrative Procedure Act is attached.
This is the only form that should be used in reporting the maior
activity notice to the State!
The Land Use Commission is providing the enclosed copies of this form
for your immediate use and to serve as a model for duplication pur-
poses. Each municipality is expected to provide its own copies for
future use!
Periodically, the municipality or its designated agent should trans-
mit all such completed forms to the office of the Land Use Commission.
The L.U.C. Staff is always prepared to assist you in any way.

SUBJECT: SUBDIVISION MONTHLY SUMMARY REPORT
IMPORTANT! !
The enclosed form has been devised by the Land Use Commission for your
use in notifying the Commission of subdivision applications within
your county pursuant to C.R.S . 106-2-37(4)( 1972 Session Laws) . Besides
forwarding them each month to the L.U.C. , copies of this form may also
be kept in your files for your own growth monitoring purposes.
This Summary Report form should be completed by the subdivider and
should accompany any preliminary plan or final plat submission to the
Board of County Commissioners.
If this Summary Report form is not submitted by the subdivider, the
Board should require its submittal or should complete the form from
other materials submitted. The Summary Report form should also be used
to document any request for exemption from the Subdivision Regulations
and subsequent action taken by the Board regarding that request.
This form was approved by the Colorado Land Use Commission in compliance
with and in the manner prescribed by the Colorado Administrative Pro-
cedure Act. THIS IS THE ONLY FORM THAT SHOULD BE USED IN REPORTING THE
MONTHLY SUBDIVISION SUMMARY TO THE STATE! Please DO NOT submit any
other document including plats, plans, etc.
The Land Use Commission is providing the enclosed copies of this form
for your immediate use and to serve as a model for duplication purposes .
EACH COUNTY IS EXPECTED TO PROVIDE ITS OWN COPIES FOR FUTURE USE.
Each month, the County Commissioners or their designated agent must
transmit all such forms to the Land Use Commission' s office. The L.U.C.
staff is always prepared to assist you in any way. --

SU BJECT: MONTHLY SUBDIVISION SUMMARY FORM SUBMISSIONS
The Land Use Commission requests that all counties submit a monthly
Subdivision Summary Form regardless of the amount of subdivision activ-
ity or a lack thereof. If no such activity has occurred, please write
NO SUBDIVISION ACTIVITY THIS MONTH across the form.
Since almost all Colorado counties now have county subdivision
regulations that comply with the provisions of C.R.S. Chapter 106,
Article 2, 1963 (as amended) , the Land Use Commission expects to re-
ceive a submission from every county, every month from now on pursuant
to the requirements set forth in C.R.S. 139-59-25(1972 Suppe ) .

The Commission Staff calls to your attention two frequent misunder-
standings concerning the Subdivision Summary Forms
*(1) The column entitled "Number of Dwelling Units" is in-
tended for recording the number of existing or proposed dwellings
located or to be located on the subdivided land. If you wish to
record the number of divisions of land in this column instead of
dwelling units, please indicate this difference by inserting the
word "lots" after the appropriate numeral.
+( 2) The column entitled "Area(Acres)" is intended for re-
cording the total area in acres of the entire property including
both the subdivided lots intended for transfer or sale and any re-
maining acreage.
-EXAMPLE-
The following is the way a county would record this information
for an exemption request where the subdivider owns 58 acres of land and
intends to seperate 3 lots from the original amount (for instance, lots
of 1.5, 2 and 5 acres) :
