I am pleased to inform you that Weld County has been awarded $4,000. These funds should be utilized to further your investment in the activities outlined in the application. I would like to remind you of the following deadlines associated with this funding: • Completion of effort/activity: April 30, 2021 • Expenditures entered into CFMS (counties only): May 7, 2021 • Submit invoice for expenditures to CDHS (CPAs only): May 7, 2021 • NOTE: CDHS can only accept invoices (not receipts). This sample invoice offers important guidance for invoices and can be used as a template. Multiple invoices can be submitted throughout the duration of the funding period. Please allow 30 days to receive payment. • Submit an article and complete survey: May 14, 2021. If you have an issue meeting a deadline, please email prior to the deadline to discuss alternative arrangements. I would also like to remind you of the following guidelines for the Innovation Fund: • Informational and marketing materials printed through the Local Innovation Fund must include the Colorado Department of Human Services logo. The appearance of the CDHS logo must be approved before items are printed. Email Adrienne Cooper for approval of logo use and questions about branding. • Funds may not be used to fund May Foster Care events or November National Adoption Month events or activities. • Funding may not be used for participation in community events or hosting recruitment events. • Funding may not be used for staff time. • All adjustments must be requested and approved by CDHS in writing. • Counties and CPAs may include several different activities and efforts in their Local Innovation Fund application. However, retention activities must make up more than half of all funding requests. • Failure to submit an article, invoices and/or enter expenditures into CFMS by the associated deadlines may impact future funding. • Photographs of the funded event or activity are not required but highly encouraged.
