HomeMy WebLinkAbout20200336.tiffWELD COUNTY DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT 1555 North 17th Avenue, Greeley, CO 80631 www.weldhealth.org LETTER OF INTENT between Weld County Department of Public Health and Environment and North Range Behavioral Health Regarding Colorado Office of Behavioral Health RFP 2020000117 — EBP Programs & Policies January 2, 2020 Weld County Department of Public Health and Environment (WCDPHE) works in partnership with the communities they serve to ensure all Weld County residents cart be healthy in mind, body, and spirit. WCDPHE's Communities That Care (CTC) grant supports an evidence -based approach for communities to address problematic youth behaviors such as substance use by forming coalitions and identifying research -based prevention strategies to address community selected priorities. This Letter of Intent is designed to reflect the understandings and partnership between WCDPHE and NRBH and will be in effect from July ls`, 2020 until June 30th, 2021, with opportunity for extension beyond this date. It shall remain in effect unless terminated by either party with thirty (30) days written notice. RESPONSIBILITIES OF WELD COUNTY DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT: • Support the outreach and expansion of North Range Behavioral Health's (NRBH) programs, Parents as Teachers and Strengthening Families 10-14, by: o Assisting in the promotion of Parents as Teachers and Strengthening Families 10-14 classes throughout Weld County. o Assist in the recruitment of Strengthening Families10-14 class participants in Milliken through the Communities that Care (CTC) Coalition. o List NRBH programs in the Milliken CTC Community Action Plan and other community reports. • Provide letter of support for activities of Parents as Teachers and Strengthening Families. • Train appropriate staff and partners on Parents as Teachers and Strengthening Families 10-14 • Provide Community Health Survey data to support NRBH program evaluation efforts. RESPONSIBILITIES OF NORTH RANGE BEHAVIORAL HEALTH: • Offer evidence -based programming in Weld County • Strengthening Families 10-14 coordinator participates in monthly Milliken Communities that Care (CTC) coalition meetings. Implement 8 Strengthening Families 10-14 courses and 1 Strengthening Families Booster course. o Approximately two (2) of these courses will take place in the Milliken CTC community, Iirktneyer, and of CountyCommissioners January 3, 2020 ot-A., 0l' 7I @ a 2020-0336 Hello
