On June 20, 2019, the Board approved the templates and provider lists for the 2019-20 out -of -home (OOH) placement agreements, identified as Tyler ID 2019-2252. I recently conducted an internal audit of our OOH agreements and determined that we did not issue agreements to 13 Child Placement Agencies (CPAs) and 12 Residential Child Care Facilities (RCCFs) that were among those approved by the BOCC. Agreements were not issued because the entity or level of care either closed, was not utilized by Weld, or was not a placement option for Weld. Those entities and the associated level of care are noted in the table below. Additionally I have highlighted them in the June 2019 PA documentation for reference. Please refer to pages 13 (CPAs) and 26-27 (RCCFs) of the attached PDF. Please let me know if you have additional questions regarding this item. Thank you.
