HomeMy WebLinkAbout740353.tiff RESOLUTION RE: 1975 WELFARE VISION CARE SERVICES CONTRACT BETWEEN WELD COUNTY ON BEHALF OF THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES AND THE WELD COUNTY OPTOMETRISTS AND OPHTHALMOLOGISTS ASSOCIATION. WHEREAS, the Weld County Optometrists and Ophthalmologists Association has submitted a contract to the Board of County Commissioners, Weld County, Colorado, on behalf of the Weld County Department of Social Services for the year 1975, a copy of which is attached hereto and made a part hereof by reference, and WHEREAS, after careful review and consideration of the contract, the Board believes it to be in the best interest of the County to accept the same. NOW, THEREFORE, BE IT RESOLVED, by the Board of County Commissioners, Weld County, Colorado, that the 1975 Welfare Vision Care Services Contract as submitted by the Weld County Optometrists and Ophthalmologists Association be, and the same is hereby approved and accepted for signature by the Board. D AS TO:.:FORM: ((( 7 Aid"( � 17J�1 J' County Attorney 21C00% 740 353 h-1 _ 1 f WELD COUNTY DEPARTMENT OF SOCIAL SERVICES VISION CARE CONTRACT This contract executed at Greeley, Colorado this day of January, 1975 , by and between the Board of County Commissioners of Weld County, Colorado hereinafter called the County, and the Optometrists and Opthalmologists of Weld County, hereinafter called the Association. WITNESSETH: That in consideration of the mutual promises and obligations and other con- siderations herein expressed, it is mutually agreed until December 31, 1975 that: 1. All eyeglass services shall be provided by prior written authorization by the Weld County Department of Social Services. 2. Glasses are to be provided for children of recipients of Aid to Dependent Children assistance, children of General Assistance recipients, children in foster home care, and for eligible adults within funding limitations. 3. The fee for examination is established at $12.00; the County to pay $10.00; the patient to pay $2.00. If the examining doctor feels tonometry is ad- visable and performed, an additional $4.00 may be added for this service. 4. The cost of lenses is to be determined by a standard lens service fee of $5.00 per pair for single vision lenses; $7.00 per pair for bifocal lenses, and $9.00 per pair for trifocal lenses plus laboratory prescription prices for lenses from the current Bausch and Lomb price list. If Photo-Gray lenses are chosen, the patient is to pay the difference in cost between clear lenses and Photo-Gray lenses. 5. The cost of frames is determined by a standard frame service fee of $6.00 plus actual laboratory cost of the frame from the manufacturer's current price list. If a lower priced frame is used, the County will be billed the lower cost. If a higher priced frame is used, the patient will pay the ad- ditional laboratory price over $6.00. 6. Temples and minor frame repairs not exceeding $5.00 in cost are to be billed at customary charges. Lens and frame replacements are to be billed as stated in paragraphs four and five. 7. The patient is to pay 15 per cent, to the nearest dollar, of the lens and service fees and material costs. 8. The only exceptions to the above 15 per cent to be paid by the patient are welfare children in foster care homes and for repairs and lens and frame re- placements. 9. All patient fees must be paid by receipt of the lenses and frames. 10. When submitting vouchers for payment, authorization forms and itemized state- ments must accompany the vouchers. Along with the fees for services and laboratory costs, the statement must show the patient's name, prescription, type of lenses provided (single vision, bifocal, or trifocal) and amounts paid by the patient. Failure to comply with these provisions will result in disapproval by the adjudicating committee and non-payment of the statement. 11. A committee consisting of representatives of the Optometric and Opthal- mological professions will recommend policies and initiate procedures of the program and will review the billings and adjudicate any unusual fees. Billings will be adjudicated quarterly the first three quarters of the year and monthly the fourth quarter of the year. This committee will furnish a semi-annual report of expenditures of funds in detail to the County Department of Social Services and the County Commissioners. 12. Only members in good standing in their respective and accredited society and/or association will receive appointments as a participating member of the Social Services Vision Care program. 13. Total amount paid by the County shall not exceed $10,000.00 for the year 1975. When this budgeted amount is reached, no provision is made for further services. IN WITNESS WHEREOF, said parties have subscribed and executed these presents, the County in its proper name and by its proper authorized officers, attested by its Clerk and sealed, and said Association by its proper name and proper officers thereunto duly authorized by it, the date first above mentioned.
