RESOLUTION RE: APPROVE 2021-2023 COMMUNITY SERVICES BLOCK GRANT (CSBG) PROGRAM APPLICATION AND PLAN AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with the 2021-2023 Community Services Block Grant (CSBG) Program Application and Plan from the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, to the Colorado Department of Local Affairs, Division of Local Government, commencing upon full execution of signatures, with further terms and conditions being as stated in said application and plan, and WHEREAS, after review, the Board deems it advisable to approve said application and plan, a copy of which is attached hereto and incorporated herein by reference. Request Board Approval of the Departments' Community Services Block Grant (CSBG) Application Packet, The 2021-2023 Community Services Block Grant (CSBG) Application and Community Action Plan is the three-year application and plan submitted, as required, to the State Department of Local Affairs for the use of U.S. Department of Health and Human Services, Community Services Block Grant. l'he three-year plan for the grant will begin on January 1, 2021 and continue through December 31, 2023. The Application and Community Action Plan identify the strategic use of CSBG funds in assisting low-income residents of Weld County. Attached you will find the three-year application, plan and 2021 budget. The attached budget is for the projected allocation of $265,175.00 for 2021. The three-year plan includes; emergency assistance, which includes funding to the shelters in Weld County, case management and support toward employment and training, including living stipends, case management support to senior citizens and disabled populations, Additionally, it includes training and travel costs to employees and board members as well as food and beverages for board meetings. The Application is due by October 1, 2020, submitted through the DOLA Grant Portal. I do not recommend a Work Session. Applicant must be able to demonstrate that customers of CSBG programs or services will be low-income individuals and/or families living at 125% Federal Poverty Level or below. Describe how customer eligibility based on Federal Poverty Level is determined, evaluated and tracked for the purpose of CSBG program activities. The county will verify income through all resources available like CBMS and Department of Labor. If not available, the county will make a collateral contact to verify over the phone or require the client to provide verification. 2. If the proposed project requires customers to complete an application or there is a selection process (e.g., emergency assistance, human services program, etc.), describe what procedures will be used to ensure that customers receiving assistance will be selected through an open and equitable process and that greatest community needs are addressed. The selection process for our CSBG paid case managers is determined by the percentage of the caseload that is at or below 125% of poverty. The application for emergency assistance is determined eligible by income at or less than 125% of poverty and those that meet the emergency criteria. All clients are served that have cooperated with program requirements. 3. Please describe the notification process and grievance procedures for customers who are declined assistance. The customer is mailed a letter notifying them their application has been denied and explains their appeal rights. 4. If sub -recipients determine customer eligibility, please describe monitoring procedures the eligible entity uses to ensure the federal poverty level income requirement, selection process and notification/grievance procedures as listed in Question 1-3 are met. If no sub -recipients are used in this program, please indicate "not applicable." N/A 5. Weld County Human Services gathered data from our local Health Department Needs Assessment, Community Commons.org and United Way Trend Reports of unmet needs. Describe how the federal domains your agency will be working in, identified in Section E, will address the needs identified in the community needs assessment and what are the general services and strategies that will be used? Workforce Center programs that serve the unemployed populations who meet 125% of poverty income. Case management services and worksite development that staff would be able to provide to designated target groups and caseloads. Individuals who encounter numerous employment barriers such as poor work histories, lack of work experience, limited education and offender status in need of more structured job seeking assistance to compete within the labor market. Weld County will be providing emergency (i.e. rent, utilities, clothing etc.) and health (i.e. vision, dental, etc.) services to individuals and families to stabilize the household, maintain, or improve the self-sufficiency of the household. Assess the need for ongoing case management and care planning. The need to make referrals to other medical resources and to provide ongoing case management for youth aging out of Foster Care, seniors, disabled individuals and families needing extra support to ensure quality living in the least restrictive setting. 2. Describe the expected outcomes for the customers or community that will be achieved. Indicate whether each outcome is a family, agency or community level outcome. Projected number of clients to receive case management services per year: 300. 20% of clients served in case management will obtain employment at exit. Provide emergency shelter and case management to over 110 homeless individuals. Provide emergency assistance to approximately 150 individuals/families per year, 85% will stabilize or improve self- sufficiency. Provide case management to 150 seniors or disabled individuals per year, they will retain their least restrictive living setting. 3. How will success be measured? Include how data will be collected and the quantitative and qualitative evaluation techniques that will be used. Success will be measured by the outcomes identified in the Community Action Plan. These outcome indicators include: -Achieve and maintain capacity to meet basic needs for 90 days - households experiencing homelessness receive safe temporary shelter or a hotel voucher -low-income households obtain self-sufficiency or stability -Obtained and/or maintained safe and affordable housing project to serve 25 families and 80% will maintain for 90 days. - Households avoiding eviction, project to serve 25 families of those 80% of families served will avoid evictions for a minimum of 90 days. Page 6 of 11, Revised 2020 -Improved health and safety due to improvements in their home -Obtained access to reliable transportation, project to serve 10 families. -Unemployed and obtained a job -Employed and maintained a job for at least 90 days -Employed and obtained an increase in employment income and/or benefits -Obtained skills/competencies required for employment - Completed a high school diploma and/or obtained an equivalency certificate or diploma. - Seniors (65+) who maintained an independent living situation -Individuals with disabilities who maintained an independent living situation. 4. What other community entities, organizations, or stakeholders are contributing to this project and how services will be coordinated? Describe how duplication of services will be avoided. Our team works with our community partners to leverage funds to ensure there is no duplication of services and we are working together to support families and individuals. We attend regular community meetings to make sure we know what services are available and how to access them. 5. If you will still be spending down your CARES Act allocation in 2021 describe how will they complement your 2021-2023 services and strategies? We will be using our Cares Act Funding to support those that are unemployed due to Covid19. They will be supported though employment -based programs, support in acquiring public assistance and emergency assistance in the community. 6. Describe how CSBG funds are leveraged with other cash and in -kind resources in the community. In what ways does CSBG fill gaps in services or address unmet needs in the community? Our Outreach Team works closely with our community partners and all Human Services divisions. They work to ensure clients have access to all services in the community and work to leverage funds. 7. How is Results Oriented Management and Accountability (ROMA) — the 5 -step process of assessment, planning, implementation, achievement of results and evaluation — used in your agency and program? Describe how you achieve each of the five steps in the process. The principles of ROMA are embedded in Weld counties everyday process. Assessment -We worked with the agency doing a needs assessment to have additional questions added to determine community need. Reviewed additional community data from partnering agencies and internally to determine needs. Planning -Reviewed assessed data to determine the need. Implementation -Combined funding sources to leverage existing program. Achievement of results and evaluation -Providing services to families or individuals that may not have received the services. Executive Summary Based off reports from United Way, the Health Department, and Human Services, our community found an increased need for rental assistance, food assistance, housing assistance, utility assistance, and shelter assistance In addition, added supports were offered for those seeking employment and seniors who face poverty With 10 5% of those in poverty through weld county, services are in place to help eliminate barriers and address their needs 2. Evaluation of Past Performance and Environmental Scan - 211 has been a platform that the community can use to obtain resources needed - From March to July, United Way 211 had an increase of calls from people looking for resources and assistance for rent, food, utility, and health care 3. Geographic Area Covered by the Assessment - The geographic area served is the urban and rural populations throughout Weld County - Weld County has 10 2% of households living in poverty (Community Action Partnership Report) - The south west side of the county has the highest population - Hispanic and Latino populations make up almost 16% of those experiencing poverty in Weld County - Weld county is comprised of both urban and rural communities, but the majority of the population is located in the urban areas - Geographical factors that affect poverty are a lack of resources available in the rural parts of the county For example, rural populations lack transportation to access resources available - In addition, females have a higher representation in poverty (12 17%) than males (9 08) in the county, and higher rates than Colorado overall (yet less than the U S rate) Weld County has almost 13% of females experiencing poverty, and 45% of households experiencing poverty in Weld County are Female Head of Households (47 7% Married Couples and 7 3% Male Head of Household) 4. Community Resources - Agencies in Weld County aided with payments for rent, financial assistance, food assistance, health care, housing assistance, shelter/transitional housing, and information services that are offered throughout the county - Employment First assisted individuals with applications for Unemployment benefits - Area Agency on Aging assisted the senior population by providing resources and food baskets 5. Data Analysis - Standard 3 2 - As part of the community assessment, the department collects and includes current data specific to poverty and its prevalence related to gender, age, and rac%thnicrty for their service area(s). The data analysis section should include - Qualitative Data — (Org Standard 1 2, Standard 3 2, Standard 3 3) o Based off the Weld county CAP National Quantitative Data Report there are around 18,919 people living in poverty (census goy) o Agencies such as United Way, the health department, and the department of human services help identify these individuals o The department of human services can survey the amounts of families and individuals in poverty by determining eligibility for public assistance This shows the amount of person(s) in poverty based off their FPL In addition, United Way does their Point in Time Count, which gathers the numbers of those who are homeless in Weld County The Health Department also provided a community wide survey that gathered data about a household's health o Agencies like United Way focus on homelessness and Lutheran Family Services focuses on the refugee population. Quantitative Data — (Standard 3.2, Standard 3.3) o American Community Survey data o Census data o Colorado State Demography Data Resource o Community Action Partnership Data Hub o Since March, over 25,000 unemployment claims have been completed through employment services. o United Way 211 was able to gather data during and post COVID. Their data shows that during the first month of quarantine (March) 211 experienced an influx of calls requesting resources for rental assistance and food assistance. Towards the end of the quarantine in July, 211 calls decreased significantly by the thousands. For instance, March had a total of 19,682 calls versus in June, phone calls decreased to 9,684 for the month. (COVID-19 Cumulative Summary) Information gathered from key sectors of the community — (Org Standard 2.2) o United Way of Weld County, the Workforce Center o The Health Department, Human Services 6. Key findings on the Causes and Conditions of Poverty (Standard 3.4) - Cause of poverty: Factors that create or foster barriers can be lack of childcare, lack of transportation, lack of accessible resources, and lack of education. Condition of poverty: Evictions are conditions of poverty. - Needs should be identified as family, agency or community needs o Families who experienced layoffs are needing to regain stability. o Agencies were able to support each other in what the other lacked in for resources. o The Community experiences an increase for the need of resources and financial assistance to help support those in poverty. Org Standard requirements - Standard 1.2 - The department analyzes information collected directly from low-income individuals as part of the community assessment. - Standard 3.2 - As part of the community assessment, the department collects and includes current data specific to poverty and its prevalence related to gender, age, and race/ethnicity for their service area(s). Standard 3.3 - The department collects and analyzes both qualitative and quantitative data on its geographic service area(s) in the community assessment. - Standard 3.4 - The community assessment includes key findings on the causes and conditions of poverty and the needs of the communities assessed. Standard 3.5 - The tripartite board/advisory body formally accepts the completed community assessment. Standard 4.2 - Community Action plan is outcome -based, anti -poverty focused, and ties directly to the community assessment. Meeting called to order 8:21 a.m. Community Needs Assessment: Cindy Kronauge, Senior Health Data Specialist, Weld County Public Health and Environment. Cindy discussed the health monitoring survey conducted in the fall of 2019 to Weld County residents and the supplemental survey that will be sent again in July 2020. The survey process, goals and topics were shared. The percentage of returned surveys were broken down by age, income and ethnicity. Key findings of low-income residents were shared including income, disabilities and education. Some of the findings include: • 66% of the returned surveys were from low income woman, earning less than $25,000 per year • 3 out of 10 do not have a high school diploma • Only 4% of low-income responders do not have health insurance • 9 out of 10 have prescription drug coverage c 2 out of 3 have mental health services • 30 % delayed or went without healthcare in the past 12 months ® Health profiles were compared for the low-income vs the not low-income group O 4 out of 10 indicate they are usually or always worried about having enough money for food ® Housing, financial and food were the highest percentage used or needed and not used The supplemental survey will be sent at the end of July 2020 to the individuals who responded to the 2019 survey to gain additional information regarding COVID-19 and whether they have needed and used some services this year Childcare availability/difficulties is one question that has been added, due to COVID-19 Introductions — Adam Turk Each member shared their name and current position Review of Minutes from January 17, 2020 — Adam Turk Motion — Stanley 2nd - Enita Kearns-Hout Roll was called and all were in favor COVID-19 Update — Jamie Ulrich Jamie provided information regarding program changes to DHS processes due to new mandates The DHS staff stepped up and did a tremendous job with all the 2020 changes Assistance Payments (AP) has had a 53% increase in new applications since January 2020 There have been waivers for items that could be delayed or extended, which will end between July and October 2020 Interviews weren't required for applications and redeterminations were extended, which saved a great amount of time AP is gearing up to work applications without the waivers The backlog has greatly improved, due to the waivers, so AP is in a good position Area Agency on Aging ombudsman have been affected by COVID-19 changes and they have seen an increase in deaths, which has been difficult on staff During COVID-19 the meal sites have gone up from 1,200 to 1,600 meals per week, At the same time the volunteers have decreased, and staff have handed out meals at various meal sites The Child Support division has seen some parents struggling to pay child support and others not receiving child support The stimulus checks were intercepted and from the child support payors and given to the payees, which created a large number of complaint calls There was a record high of $2 4 mil collected in March, which is expected to be higher because of the stimulus money received in the 2nd quarter Child Welfare referrals went down because schools were closed, and mandatory reporters were not seeing children There is currently an uptick in referrals, which will likely continue as children return to school Parent visitations were done remotely and are now being allowed minimally Employment Services has had a huge increase due to unemployment and individuals are required to register with the Workforce Center Staff has helped with registering up to 488 individuals per week and staff have been pulled from other areas to help with the increase Child Care has lost a few providers and they are reviewing the CARES Act funds to determine how to help with childcare needs in the community Family Resource Division assist the high -risk families who do not have a child welfare case Referrals are down 30% and they are expecting uptick soon, when children return to school Emergency funds have not been requested as much during COVID-19, but FRD is expecting to have an increase once the unemployment checks end Community Services Block Grant (CSBG) — Karin Crandall, Community Outreach Manager DHS is planning for the 3 -year CSBG contract and application process The Needs Assessment was delayed, due to COVID-19 The budget is due in October and it will be emailed for a vote to HSAC The CARES Act provided additional CSBG funding of $395,000 for COVID -19 related expenses The contract was changed by the State for all counties to allow flexibility in the decision -making process and approval of funding requests The Needs Assessment information is being reviewed with community data for food, homeless shelters, unemployment and other needs The CSBG budget was discussed in detail There will likely be a need for additional funding for utilities and rent/mortgage payments One new position will be used out of the CARES Act funding, from a vacant AP position, and $130,000 was allocated for emergency assistance The Outreach team does work with clients to negotiate with landlords to try and pay the balance over months, rather than one lump sum LEAP was extended beyond March 31 Community Services Block Grant (CSBG) Financial Report — Lennie Bottorff, DHS Senior Fiscal Advisor 17 bids and 15 contracts issued o Area Agency on Aging — 10 bids and 9 contracts issued o No Open Bids o Family Preservation Commission Report Overview — The Annual Core Services Report was developed through Colorado State University to help evaluate DHS services o CORE Services plan — Contract Management received the annual memo this week and detailed information will be discussed with HSAC in October Weld Works for Youth Update — Karma Amaya, Youth & Adult Employment Training Manager Karma presented the Weld Works for Youth program, which began in 2007 It has grown and the 2020 goal is to serve 80 individuals The Young Adult Job Fair in March is the beginning of the program each year There are 4 case managers who recruit youth and they reached out to schools and other organizations to recruit youth Employment Services received more than 137 applications and only 5 did not qualify for the program The youth earn minimum wage and work 20 hours per week with employers close to home, so they don't need transportation They can attend life -skills workshops and they do get paid to attend the workshops We had 82 individuals registered to participate this year Junior Core is one of the worksites, with outdoor community service work, which has decreased in number The workshops have been revised to virtual this year Skype was scheduled but the participation has not been ideal, as they are not able to see the presentation It has been challenging to have engagement Finding work sites was challenging and communities assisted to help find work sites for the youth Two organizations have asked how they can participate in the future WCDHS Division Updates ® Management Evaluation — Andie Garnand, Assistance Payments Division Head- Andie reviewed the SNAP audit, conducted every 3 years, and the 1 -page audit findings o Older Americans Act — Holly Darby, AAA Community Outreach Manager -Holly reviewed the audit, as compared to the 2019 audit, which focused on the voucher programs and moving to a new building in August ® Melanie Woolman — United Way, 211, update -identified by the State of Colorado for COVID-19 response referrals There was a huge increase in call volume from 400 to 2,187 calls per day 211 hired an additional 20 people to handle the increase in volume There is an increase in funds for rental assistance, including undocumented residents The United Way team is doing more direct services than they normally do, with the pandemic The physical office will be closed until August and they are collecting masks and distributing to the community © Steph Gausch — Food Bank has had a 56% increase overall Mobile Food Pantries have increased 150% from people accessing the pantries and the backpack program was extended through summer There has been a 300% increase with children's meals and the Food Bank has tripled the food budget for 2021 Additional staff have been hired and additional trucks have been purchased to deliver food to the communities ® Meredith Munoz— North Colorado Health Alliance has had an increase in calls, and many are mental health calls • Enita — Catholic Charities has been fortunate for rental assistance Utilities assistance received $200,000 and can help residents with stability $185,000 CDBG block grant for rental assistance was awarded for COVID related assistance and ESFP grants of $32,500 were awarded Emergency Assistance
