Dear Director Ulrich,

WELD COUNTY
COMMISSIONERS
The Colorado Department of Human Services, Division of Child Welfare is pleased to inform you that Weld County
OHS is being awarded Prevention and Intervention funding, as applied for via the application distributed with IM-
CW-2019-0061 (CORRECTED).
As described in the memo, there are three funding streams that may pay for intervention or prevention
services in State Fiscal Year 2020 (SFY 20) that do not require a county match: IV -E Waiver Cash Fund
(Waiver), Prevention and Intervention Cash Fund- AU County Account (SB 19.258), and Prevention and
Intervention Cash Fund- Small and Medium-sized County Account (SB 18-254). Awards have been
determined by an allocation methodology developed by a collaborative workgroup of State and county
stakeholders. Based on the allocation methodology, it was determined your county's portion of the
available allocation is as follows:

Each funding stream has, or will have, distinct County Financial Management System (CFM5) codes associated with
it. The CFMS codes associated with the IV -E Waiver interventions are included with this award letter. CFMS codes
associated with prevention services will be provided upon availability. Intervention or prevention costs accrued at
any point within SFY 20 may be applied to each funding stream, but are not to exceed the "total" associated with
any of the boxes within the table above. Any expenditure that exceeds the total award amounts may be applied
against the county block during year end closeout.
Please direct any questions you have about your Prevention and Intervention award to Tyler Allen, Title IV -E
Waiver Administrator, at tyler.alten®state.co.us or at (303)866-2154.
