Health Department for reasons oet forth in the Weld County individual sewage disposal system regulations including failure to meet
any term or condition imposed thereon during temporary or final approval. The issuance of this permit does not constitute
assumption by the department or its employees of liability for the failure or inadequacy of the sewage disposal system.

made and furnished by the applicant or by the Weld County Health Department for purposes of the evaluation[ of the application; and

Septic System Inspection, July 15, 2020
Building: Pole framed, metal sided shop building with monolithic concrete slab floor, includes
shop bays, office area, and bathroom with storage shelves and water heater. Building has
added on area 16 ft by 48 ft on south end of original building. Toilet and lavatory sink confirmed
flow to septic tank via camera inspection. Add on part confirmed by color on metal has
different shades.
Septic Tank: Located septic tank via Ridgid CS65X SeeSnake camera with 512hz transmitter and
receiver system. Septic tank is located south of the restroom toilet approx. 11 ft in the middle
of the added on area of the shop building. A 4 inch "cleanout" pipe was located in the concrete
floor of the building approx. 9.5 feet north of the restroom wall and 13.5 feet east of the
overhead door on the west wall. We were unable to determine what the tank is constructed of,
working capacity sizing, working level in tank or condition of the tank structure. Inlet Sanitary
tee was observed to be present in tank via camera inspection. Access lids on the septic tank
need to be excavated up, which would require cutting and removing the concrete slab floor.
We are unable to determine if the tank is in Acceptable Condition or not.
Soil Treatment Area: Was unable to determine exact location and size of the STA. We used a
method of dowsing to give us an indication of a possible location, can not confirm due to the
unscientific and unreliable science of dowsing. STA is believed to be located under the concrete
slab floor. Stress tested STA with approx. 150 gallons of water. Water level rose in septic tank
access pipe 1.5 inches during test but dropped back down to original level in approx. 30
minutes. STA accepts effluent at time of inspection.
Pipes: 4-inch PVC pipe from building restroom to septic tank. No bellies, cracks, or signs of
crushing were observed by camera inspection.
Conclusion: The Septic System is in Undeterminable Condition.

Close up of access pipe show that it is a steel/iron pipe. It will be suseptable to rusting/
corroding out and not maintain water tight conditions with the surrounding soils.
