INSTRUCTIONS FOR COMPLETING ANNUAL
INSPECTION FORM FOR SOLID WASTE DISPOSAL
SITES
1. COMPLIANCE
Mark "X" in the / / under "Yes" if all items a-h are marked yes. Mark
"No" if any item a-h is marked no.
a. Check under "Yes" if the site is designated by the County Commissioners
or if it is inside the corporate limits of the city or town. Otherwise
check under "No".
b. Check under "Yes" if the site is free of obnoxious odors and if rats
or an excess of flies are not seen at the time of inspection. Check
under "No" if obnoxious odors are present or if rats are seen or if
flies are a problem.
c. Check under "Yes" if the site is covered regularly. Check under "No"
otherwise. (Daily S. Landfill, Weekly SPD).
d. Check under "Yes" if there is satisfactory access for machinery. Other-
wise check under "No". (Note: If the covering is satisfactory, that
would be prima facie evidence of adequate area to accommodate machinery.)
e. Check under "Yes" if the site is fenced and there is no evidence of
scattering of debris beyond the site or along fence lines. Otherwise
check under "No".
f. Check under "Yes" if no flame or smoke is seen at time of inspection.
Otherwise check under "No".
g. Check under "Yes" unless toxic materials are seen at the site and
identified. (This is interpreted to mean quantities of toxic materials
and does not refer to casual discard of containers.) Otherwise check
under "No".
h. Mark the opposite way from item 2.
2. WATER POLLUTION EVIDENCE
Mark "Yes" if evidence of water pollution has been established. Otherwise
Mark "No".
3. LOCATION
Mark "M" if the site is inside the corporate limits of a city or town;
otherwise mark "C".
4. OPEN BURNING
Mark "Yes" if smoke or flame is observed. Otherwise mark "No".
