The parents or caregivers are then contacted at designated time points, probably six and twelve months, to complete a standardized evaluation interview by phone or using SurveyMonkey. All participants are provided with a $25 gift card for their participation. ' McMaster Family Assessment Device is a self -report measure of satisfaction with family functioning. It was developed by Epstein, Baldwin, & Bishop in 1983. At JusticWorks, we are using the subscales of Problem Solving, Communication, Roles, and Behavior Control. The scale is reported to have good reliability and validity and able to detect changes in functioning over time "The Columbia Impairment Scale is a 13 -item scale that can be completed by a parent/caregiver or youth to provide a global measure of impairment. The instrument was developed by Bird, Shaffer, Fisher, and Gould in 1993 and is widely used in the social service and behavioral health fields. The 13 items tap 4 major areas of functioning: interpersonal relations, broad psychopathological domains, functioning in job or schoolwork, and use of leisure time. The CIS score obtained through the parent's interview appears to provide a useful global measure of impairment. The instrument is reported to have good reliability and validity. "' The Restrictiveness of Living Environment Scale, is a measure to assess an environment's level of restrictiveness for children and youth. It was developed by Hawkins, Almeida, Fabry, and Reitz in 1992. The scale rates the restrictiveness of the setting on a 10 -point scale, using a rating method designed to yield an interval scale. It is often used as an outcome measure to determine effectiveness of care. It is used at JusticeWorks to determine if youth are moving from more restrictive setting to less restrictive settings at time of discharge. EXHIBIT C PROPOSAL TEMPLATE 1. Bidder's legal entity name: 2. Program name or service type being proposed: JusticeWorks YouthCare. Inc. Aftercare Services - JustCare® 3. Modalities, curriculum or tools that will be utilized in the delivery of the service. JusticeWorks developed the JustCare® program in 1999 with the goals of reducing the length of stay in residential placements for both delinquent and dependent children and youth, and to establish successful outcomes when children and youth return to the community. JustCare® ensures that gains made in placement endure at home. Unlike most 'aftercare' programs, planning for the transition home is done throughout the residential stay, and includes input from family, caregivers, and other stakeholders. During the youth's placement, we work with the placement agency to focus their services in accordance with the reality of home and community. We also work with the child/youth and family to ensure a smooth transition back home. We accomplish this by assisting families to understand issues from a child's perspective, demystify placement language and coordinate (supervise, if applicable) visits. We work to help families resolve problems in the home (i.e., substance abuse, employment and economic problems, mental health concerns, housing). We work with families to develop their strengths and to set appropriate boundaries and supports for their children. JusticeWorks professionals work non-traditional hours, with an emphasis on being in the home, school, and community to provide the child and family with guidance, coaching, and support for continued success. JustCare® is an effective aftercare service to reduce reoccurrences of out -of -home placement. The cost savings in reducing residential stays more than pay for the cost of the service. Assessment Tools: Standardized measurements used include the Family Assessment Device (refer to website www.nctsnet.orq ) and the Columbia Impairment Scale (refer to the following link http://www.dhs.state.il.us/page.aspx?item=64974). Operating Methodology: .On -call emergency response 24 hours a day, seven (7) days a week. •Maintaining a 24 -hour maximum response time after receipt of a referral. •Providing a family -based, child -focused service model. We will go to the families, and delivering services in the home and community. •Communicate to the family that our priority is to keep children safe. We assist families and caregivers in developing the skills and resources to avoid future crises. •We work creatively and flexibly to assemble the resources and devote the time needed for family success. •At the conclusion of JustCare® services, families have a productive working relationship with a variety of community and kinship supports to sustain positive functioning. We utilize a variety of methods including evidence -based, cognitive behavioral 1 EXHIBIT C PROPOSAL TEMPLATE interventions and motivational interviewing techniques. We assist families in accessing community supports and resources and provide case management. Not all services are explicitly `therapeutic' in the clinical sense. Helping to prepare a meal, maintaining a family budget, follow-up on necessary appointments, introducing a board game for everyone, and planning an outing or other means of positive family engagement can be very significant for under-resourced families. Once we receive a referral, we are in contact with the referral source to discuss the case. We contact the families within 24 hours to schedule an intake appointment. Services are provided in -home and in the community. Direct service provision is divided into four sub -domains: 1) Engagement, 2) Planning, 3) Implementation, and 4) Transition. Each of these sub -domains coincides with meeting performance objectives: Engagement: Families have a high level of decision -making power in all aspects of planning, delivery, and evaluation of services and supports. We assess and address immediate crisis/safety needs. We support and encourage family members to participate as partners in resolving issues that can lead to a re -occurrence of out -of -home placement. We encourage fathers (where applicable), often relegated to secondary or non-existent roles, to take an active, participatory role throughout services. Our services implement, promote, and support the use of a non -judgmental, non - blaming, family -centered approach in partnering with families. Each person is recognized as having unique individual, family, and community strengths. We incorporate these in goal planning and development of protective factors. Strengths, needs, and cultural identity are communicated in case supervision and team meetings. Planning: Goal plans are developed with input from the family and child (if age -appropriate), the referring source, and any other child -serving agency involved with the family. Goal plans are strength -based and family -focused. They incorporate the family's vision and hope for the future. All plans are fluid documents and change as family needs change. Goal plans are comprehensive and address the priority life domains of the child, youth, and family which include, but are not limited to: Safety, emotional wellbeing, family, housing, education, employment, cultural, spiritual, parenting skills, social/fun, legal, medical/health, finances, relationships, developmental needs or independent living skills, substance abuse, and behavioral issues. Implementation: Families have timely access to an array of services and supports that are well - coordinated, tailored to meet their individual needs, and delivered in the communities in which they live. Transition: Transition to autonomy occurs throughout services. Transition includes strategies to shift from stabilization and formal services to informal services and supports that can be sustainable after discharge. 2 EXHIBIT C PROPOSAL TEMPLATE 4. STOPPO: Crisis Intervention and Stabilization 3. Modalities, curriculum or tools that will be utilized in the delivery of the service. STOPPO is a high impact, short-term program designed to meet challenging situations in a child's home that put the child at risk for out -of -home placement. By using a proactive approach, STOPPO works with families to address the issues that need to be resolved to keep the child safe in the home setting and the family well -functioning. We work to maintain children in their homes by addressing the concerns which led to the child welfare referral. Now being utilized in dozens of locations, STOPPO has proven to be extremely effective. Outcome studies have demonstrated that STOPPO routinely prevents out -of - home placements in more than 90% of cases, despite the most challenging conditions. Assessment Tools: Standardized measurements used include the Family Assessment Device (refer to website www.nctsnet.orq ) and the Columbia Impairment Scale (refer to the following link http://www.dhs.state.il.us/page.aspx?item=64974). Meeting the Need: Once a referral is made to the STOPPO program, staff are in the home within three (3) hours. They maintain a high level of response to any of the family's emergencies throughout services, which are usually 60 days in duration. Within 72 hours after the initial contact, staff conduct a thorough family assessment and establish a goal plan that guides the services. The goal plan outlines target areas and expectations for all family members and is monitored throughout care. Staff typically have daily contact with families and spend six (6) or more hours a week in direct contact with them to build needed skills and help them to identify and access community supports. Once the family is stable, and children are safe, service hours can be reduced as they progress through the program. In all circumstances, STOPPO addresses life domain issues by using a broad array of professional and other community resources including extended family members, churches and friends to form a network of powerful supports. Direct service provision is divided into four sub -domains: 1) Engagement, 2) Planning, 3) Implementation, and 4) Transition. JustCare® Foster Support: Foster Parent Consultation 3. Modalities, curriculum or tools that will be utilized in the delivery of the service. At JusticeWorks YouthCare, we respect the role of foster parents as dedicated individuals focused on the task of being a resource family for the child or children being fostered until a permanency plan is established. Most foster children eventually return to their biological parents. Some are adopted by foster parents or kinship adoptions. Many foster parents will find themselves in unfamiliar territory. They may already be parents or have had parenting experience, but only for parenting non -traumatized children. They have not cared for children who have extreme impulse control issues and who have problems with boundaries, oppositional -defiant behavior, and expressing emotions appropriately. They find themselves in situations they have never been in before. Foster parenting has stopped feeling good, and they do not like the people they are becoming. The job of being a foster parent will never be an easy one. It can be extremely stressful, even for those who have done it for years with expert coping skills and a strong support system. Deciding to foster a vulnerable child may also affect other children in the household, marital relationships, friendships, and communities. It is normal for any parent, including foster parents, to feel, at times, stressed and frustrated with their child's behavior. JusticeWorks YouthCare can help. We provide support and behavior management tools to prevent foster parents from reaching a breaking point. Our JustCare® Foster Support program is highly responsive to the needs of foster parents and foster children while creating collaborative relationships between foster parents and the child welfare agency. Also, staff are cognizant of the trauma a child may have been exposed to and support the foster family with resources and guidance. JustCare® will focus on improving relational and behavioral problems in the foster family system. JusticeWorks staff will assist foster families with the educational, social, and emotional needs in the fostering of traumatized children. The JustCare® program (2 to 4 hours of direct family contact per week) is utilized as a supportive program to increase the ability of foster parents to be successful in fostering children and in supporting the reunification process. Hours can be reduced over time as progress is made. JusticeWorks professionals are experts in family engagement. We incorporate the following essential elements (but not limited to) to achieve successful outcomes for all participants. •Listening to each family member. •Demonstrating respect and empathy for both foster family members, and biological parents as 1 EXHIBIT C PROPOSAL TEMPLATE applicable. • Developing an understanding of a family's past experiences and current situation. • Being aware of family and child trauma and its implications for parenting. •Having open communication regarding concerns, incorporating strengths. • Immediately responding to concrete needs. • Establishing the purpose of involvement with the foster family. • Being aware of biases and prejudices. • Advocate for foster families as needed in the home and community. We work to identify and assist foster families in accessing community resources and supports that address the myriad of concerns that are often present in fostering children. By the conclusion of JustCare® services, foster families and the child or youth have an unshakable bond in which a stable, structured environment exists, with the provision of the child's essential needs, including a nurturing and safe household. If appropriate, we also identify and formalize extended kinship and neighbors who can play a supportive role in keeping families safe and intact. All of these resources are documented in a final discharge summary at the conclusion of services. JustCare® has the fundamental goal of keeping children safe within a nurturing environment where they can thrive. At JusticeWorks, we hope that a child's first placement is their last placement! 4. We assist families and caregivers in developing the skills and resources to avoid future out -of -home placement. 1 EXHIBIT C PROPOSAL TEMPLATE •We work creatively and flexibly to assemble the resources and devote the time needed for family success. •At the conclusion of JustCare® services, families have a productive relationship with a variety of community and kinship supports to sustain positive functioning. Once we receive a referral, we are in contact with the referral source to discuss the case. We contact the families within 24 hours to schedule an intake appointment. Direct service provision is divided into four sub -domains: 1) Engagement, 2) Planning, 3) Implementation, and 4) Transition. Each of these sub -domains coincides with meeting performance objectives: Engagement Families have a high level of decision -making power in all aspects of planning, delivery, and evaluation of services and supports. We assess and address immediate crisis/safety needs. We support and encourage family members to participate as partners in resolving issues that can lead to an out -of -home placement. We encourage fathers, often relegated to secondary or non-existent roles, to take an active, participatory role throughout services. Our services implement, promote, and support the use of a non -judgmental, non - blaming, family -centered approach in partnering with families. Each person is recognized as having unique individual, family, and community strengths. We incorporate these in the goal plan for the development of protective factors. Strengths, needs, and cultural issues are communicated in supervision and team meetings with the county, and are ongoing. Planning The written goal plan is based on the strengths, needs, and culture of the youth and family, and focused on the vision and hope for the future. Safety assessments are ongoing. All plans are fluid documents and change as family needs change. Family plans are comprehensive and address the priority life domains of the child, youth, and family which include, but are not limited to: Safety, emotional wellbeing, family, housing, education, employment, cultural, spiritual, parenting skills, social/fun, legal, medical/health, finances, relationships, developmental needs or independent living skills, substance abuse, behavioral. Implementation Families have timely access to an array of services and supports that are well - coordinated, tailored to meet their individual needs, and delivered in the communities in which they live (i.e., mental health, substance abuse). We access and maximize the use of sustainable community resources to meet family and child needs in the communities in which they live. We communicate with the county and other child -serving agencies involved with the case to ensure continuity of care. Transition Transition planning occurs throughout services. Transition includes strategies to shift from formal to informal services and supports that can be sustainable after discharge. 2 EXHIBIT C PROPOSAL TEMPLATE Goal plans set benchmarks for transitioning to less restrictive, less intrusive, and less formal services, taking into consideration the ability of families to move through the process. 4. Capacity to Provide Services (ex. 4 hours/week). Services are delivered in the home and community two (2) to four (4) hours per week depending upon need. 5. Goals of the service. To prevent out -of -home placement for children at risk. To decrease risk factors and increase protective factors. 6. Outcomes of service. All of JusticeWorks' programs collect outcome information using a combination of standardized and non -standardized measurements. Across all programs the following data is collected: 1) if applicable, placement experience, 2) service goal achievement, and 3) key events a person experienced during services such as drug screen results, education status, employment, hospitalizations, independent living issues, legal system involvement, and life events. To efficiently monitor program effectiveness, the data collected includes measures of both outcomes and the processes used to reach them. Refer to the attached Outcome Summary Report and sample Success Story. 7. Target population for service. Families who have children from infant to age 17 in need of home -based services to avoid out -of -home placement. 8. Service access. Services are delivered in the home and Community for Weld County. 9. Languages service is available in. Cultural humility is an acknowledgment of one's barriers to true intercultural understanding. It is the difference between intellectually knowing another culture and being able to truly relate to it. Cultural humility is an alternative approach to that of cultural competency. It requires humility as individuals continually engage in self -reflection and self -critique as lifelong learners and reflective practitioners. Through cultural humility, individuals participate in the process of mutual understanding and awareness of self in relationship to others. JusticeWorks is committed to individual and organizational cultural sensitivity. Our goal is to have our staff composition reflect such diversity, including speaking the languages dictated by our target population. JusticeWorks maintains a staff complement that is diverse and reflects the families with whom the organization works. 10. Rates of service. ERICKA BRUNNER, ADAMS COUNTY FRS, RECEIVES WELL -DESERVED PRAISE FROM A VERY BRAVE AND DETERMINED MOTHER Good Morning, I'm sure I am somewhere in your files, but my name is Christine. I have two daughters, one age 12, and the other, age 9. In February 2017, my husband of 10 years poured gasoline on me and tried to light me on fire in front of my children. He was arrested and went to jail accordingly. Unfortunately, even though we were married, I lost everything, having no access to bank accounts, vehicles, or even my own driver's license. Of course, he was allowed no contact with the girls and me, but his parents tried to sue me for full custody of the girls, citing the trauma I had gone through, as well as the fact that I was homeless, jobless, etc. They even went so far as to take my vehicle, because it was in my husband's name, knowing a divorce settlement would take years due to his incarceration and unwillingness to sign. Inside the vehicle was the entire year of my children's homeschool work, which they admitted to destroying. We, in simple terms, we're screwed. Then came Justice Works, represented by Ericka Brunner. She began by simply giving me a ride to the DMV to get a new copy of my license. We then moved on to health insurance, food assistance, and educational materials to allow the girls to bypass their homeschool review and test out for the year. By the end of her time with my family, I managed to retain full custody, (they were forced to drop the suit entirely) find therapy services for my children, who now have PTSD, and encourage me to shoot for the stars and apply for my dream job. I now work from home, full time, as a legal marketing strategist. The girls can remain homeschooled, and I make enough to support us. This year I was even able to swing a lovely Christmas. The girls and I are eternally grateful for the help we received. Being an adult, I realize how close I came to losing my precious children, through no fault of my own. I wanted to take the opportunity to let you know Justice Works... WORKS. A year later I am able to support my family with no form of public assistance, I have a great job, and I learned it is ok to do things MY way and be successful. Without Ericka to encourage me, I would have given up. I was at the end of my patience, and frankly, my will to try. Please keep doing what you do. You truly are SAVING LIVES and families. I wanted to go beyond the paperwork and give you my personal thanks and support you as much as you supported us. If you EVER need any form of testimony, please contact me. I will never forget you, or Ericka, and my girls and I are living proof that you guys are truly needed and truly successful. Christine, you are not only in our files but also in our hearts. Thank you, for providing wonderful feedback on FRS, Ericka Brunner. Thank you, Ericka, for supporting and partnering with Christine in the spirit of the JusticeWorks Mission. It is through the power of positive relationships that true healing begins!
