RE: APPROVE CONTRACT FOR YOUTH AT -RISK OF HOMELESSNESS SUMMATIVE
EVALUATION PHASE (YARH-3) PATHWAYS TO SUCCESS GRANT PROJECT AND
AUTHORIZE CHAIR TO SIGN - CENTER FOR POLICY RESEARCH
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Contract for Youth At -Risk of
Homelessness Summative Evaluation Phase (YARN -3) Pathways to Success Grant Project
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Division of
Employment Services of Weld County, and the Center for Policy Research, commencing March 1,
2021, and ending February 28, 2022, with further terms and conditions being as stated in said
contract, and
WHEREAS, after review, the Board deems it advisable to approve said contract, a copy
of which is attached hereto and incorporated herein by reference.

NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Contract for Youth At -Risk of Homelessness Summative
Evaluation Phase (YARN -3) Pathways to Success Grant Project between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf
of the Department of Human Services, Division of Employment Services of Weld County, and the
Center for Policy Research, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said contract.

Request Board Approval of the Department's Contract with the Center for Policy Research (CPR).
Employment Services of Weld County (ESWC) is seeking to enter into a contract with the Center for Policy
Research (CPR) to participate as a site for the Pathways to Success program. The Colorado Department of
Human Services (CDHS) received a Youth At Risk of Homelessness (YARN) planning grant (Phase I) in 2013.
They were one (1) of 18 grantees. Since then, CDHS was moved forward as one (1) of six (6) grantees and has
received additional funding for additional phases. CDHS' program under this grant is Pathways to Success. The
program's purpose is to prevent and address homelessness among Colorado's eligible youth, ages 14-23, who
are homeless or at -risk of becoming homeless by implementing, with fidelity, the Pathways Model Intervention
Engaging Youth in a Coach Like Way.
CPR will reimburse ESWC a maximum of $10,000 for participation as a Pathways to Success site (ESWC will
invoice CPR quarterly). Participating as a site will involve specific activities as set forth in the contract,
including enrolling eligible Chafee youth at risk of homelessness, entering data in the Trails system and the
Pathways Information Management System (PMTS), as well as participating in monthly evaluation calls and
other evaluation related activities.

I do not recommend a Work Session. I recommend approval of this Contract and authorize the Chair to sign.

CONTRACT BETWEEN
THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES,
EMPLOYMENT SERVICES OF WELD COUNTY AND
THE CENTER FOR POLICY RESEARCH
This contract between Center for Policy Research, a Colorado nonprofit corporation (CPR) and
Weld County Department of Human Services — Employment Services of Weld County (ESWC) is
effective as of March 1, 2021.
The parties agree as follows:
1. Scope of Work. ESWC shall perform the work described in Schedule 1, which is incorporated into
this contract by this reference. Any change in the scope, nature, timing, or budget will be effective
only if approved by both parties in writing. CPR will compensate ESWC asprovided in Schedule 1.
2. Work Performance. ESWC shall devote the time, attention, knowledge, skills, and efforts
reasonably required to successfully perform the work described in Schedule 1 in a professional and
timely manner. ESWC shall comply with all federal, state, and local laws and regulations that relate
to or affect in any manner the performance of the work described in Schedule 1, and shall
immediately inform CPR, in writing, of any notices of violations of such laws or regulations and
promptly correct any violations.
3. Reports. ESWC shall furnish such reports and information as CPR may reasonably request
from time to time.
4. Confidential Information.
a. Confidential Information is non-public information or data about an individual or CPR's
proprietary, non-public and confidential information. The information may or may not contain
personally identifying information (information by which an individual may be identified, e.g.,
name, Social Security Number, orother identifying number, address, finger or voice print,
passport number, or a photograph).
b. ESWC shall use any Confidential Information provided or developed under this contract
solely for the purpose for which the data or information were provided or developed.
ESWC shall adhere to all federal or state statutes, rules, and regulations, and shall
establish administrative, physical, and technical safeguards to prevent the unauthorized
disclosure and/or use of all Confidential Information provided or developed under this
contract. ESWC agrees not to disclose or release in any manner information that may
reveal the identity of an individual or organization, including information that can lead to
re -identification, except as may be required by law or as authorized inwriting by CPR.
c. The obligations of confidentiality specified in this Section 5 shall not apply to any information

that: (a) isalready in the possession of ESWC without obligation of confidentiality; (b) is or
becomes publidy available without breach of this contract by ESWC (c) disclosure is required
pursuant to C.R.S. 24-72-202 et seq or other Colorado law or (d) is rightfully received, free of
restrictions, by ESWC from a third party.
d. This Section 5 s call survive termination or completion of this contract for a period of five
years. The obligations set forth in this Section 5 regarding personally identifying information
of an individual shallremain in perpetuity unless otherwise indicated by law.
e. In the event that ESWC becomes compelled by law to disclose any Confidential Information,
ESWC shall provide CPR with prompt written notice so that CPR may seek a protective order or
other appropriate remedy and/or waive compliance with the provisions of this contract. ESWC
shall cooperate with CPR to obtain a protective order or other appropriate remedy. In the event
that a protective order or other remedy is not obtained, or CPR waives compliance with the
provisions of this contract, ESWC shall: (i) disclose only the portion of Confidential Information
that is legally required to disclose; and (ii) exercise all reasonable efforts to obtain reliable
assurances that confidential treatment will be afforded to Confidential Information.
5. Term and Termination.
a. Term. This contract will have a term beginning on the effective date of this contract and ending
at midnight on February 28, 2022, unless terminated as provided below. This contract may be
renewed annually upon the agreement of both parties.
b. Termination for Cause. Either party may terminate this contract effective immediately for
cause. Causemeans: a) a breach of this contract by the other party, which the other party fails
to cure within 30 daysafter receipt of written notice stating the nature of the breach; or b) any
grossly negligent, fraudulent, criminal, malicious, or willful act or omission by a party affecting
or relating to its obligations under this contract.
c. Survival. Upon expiration or termination of this contract, any provisions of this contract which
by their nature are expected to survive the expiration or termination of this contract, will
survive in ful force andeffect.
6. Notice. All notices and communications required under this agreement must be in writing,
and will be considered given when delivered personally to the recipient, sent by e-mail to
the recipient, with verificationof delivery or receipt, sent to the recipient by reputable
overnight courier service, charges prepaid, with delivery confirmation, or sent by registered
or certified mail, charges prepaid, with return receipt requested, addressed to the recipient
at the following address, or such other address as the recipient may havefurnished to the
other party in writing:

7. Dispute Resolution. In connection with any dispute between the parties arising under or related in
any wayto this contract, the parties agree to first attempt to negotiate in good faith to resolve the
dispute. If the dispute is not resolved by negotiation within 14 days, then the dispute must be
submitted to mediation before a qualified mediator mutually acceptable to the parties, either in
person at a mutually acceptable location, or remotely through a mutually acceptable method. The
parties shall bear their own respective costs and attorney's fees in any mediation. If mediation is
not successful, the parties consent and submit tovenue and exclusive jurisdiction in the federal
and state courts located in Colorado.
8. Governmental Immunity: No term or condition of this agreement shall be construed or interpreted as a waiver,
express or implied, of any of the immunities, rights, benefits, protections or other provisions, of the Colorado
Governmental Immunity Act §§24-10-101 et seq., as applicable now or hereafter amended.
9. General Provisions.
a. Relationship of Parties. Nothing in this contract will be construed to imply a principal -agent
relationship between the parties, and no party will have the right, power, or authority to
obligate or bind the other party in any manner whatsoever, except as otherwise agreed to in
writing. Rather, the parties will be considered independent contractors as to each other. The
employees of one party will not be considered employees of the other party, and vice versa,
within the meaning of any applicable federal, state, or local laws or regulations.
a. Governing Law. This contract is to be governed in all respects by the laws of Colorado
without givingeffect to its conflicts of law principles.
b. Non -assignment. Neither party may assign any rights or obligations under this contract
without the other party's prior written approval. Subject to this limitation, this contract will
bind the parties and inure to the benefit of their respective successors, assigns, and legal
representatives. Any attempted assignment in violation of this paragraph will be void and of
no force or effect.
c. Legal Authority. Each person executing this contract on behalf of a party represents and
warrants on behalf of such party that: 1) such party has full power and authority to enter into
and perform this contract; 2) such party has taken all requisite actions to approve the

execution and performance of this contract; 3) such person has been duly authorized to
execute this contract in the name of and on behalfof such party; 4) the performance of this
contract is not in conflict with such party's governing documents (if applicable) or any contract
or other requirements by which such party is bound; and 5) this contract is binding upon such
party.
d. Entire Agreement. This contract, including any schedules or exhibits hereto, constitutes the
entire agreement between the parties relating to the subject matter hereof, and supersedes
any prior understanding between them. No representations, arrangements, understandings,
or agreements existexcept as expressed in this contract.
e. Amendments, Waivers. Except as otherwise provided in this contract, this contract may be
amended only by a written instrument signed by both parties. If for any reason one party does
not insist upon strict adherence to any provision of this contract or waives a breach of this
contract by the other party,that party will not be prevented from pursuing remedies or
insisting upon strict performance for a future breach of the same or another provision.
f. Severability. If a court having jurisdiction determines any provision of this contract to be
unenforceable,the remaining provisions will nevertheless remain enforceable to the fullest
extent allowed by law, and the court may replace the provision in question with a lawful
provision that most nearly embodies the original intention of the parties.
g. Counterparts. The parties may execute this agreement in counterparts, which taken
together will constitute one instrument. A signature may be delivered by fax or may be
scanned and e -mailed, andsuch fax or scanned signature will be accepted and effective as an
original signature.

SCHEDULE 1
SCOPE OF WORK — YEAR 1
This Scope of Work describes the work to be conducted by Weld County Department of Human Services
(ESWC) in partnership with the Center for Policy Research (CPR) and its subcontractors, and the Colorado
Department of Human Services, Division of Child Welfare, for the YARH-3 project led by Mathematica Policy
Research, Inc. (Mathematica) for the U.S. Department of Health & Human Services, Administration for
Children & Families, Office of Planning, Research, & Evaluation (HHS, ACF, OPRE), Contract Number
HHSP2332015000351/ 75P00119F37041).
Background information
Starting in 2013, the Administration for Children, Youth and Families/Children's Bureau funded a
series of grants called Youth At -Risk of Homelessness (YARH): Planning Phase (YARH-1), Formative
Evaluation Phase (YARH-2), and the Summative Evaluation Phase (YARH-3). Colorado's YARH grant
is known as 'Pathways to Success'. Its purpose is to prevent and address homelessness among
Colorado's eligible youth (ages 14-23) who are homeless or at -risk of becoming homeless by
implementing with fidelity the Pathways Model Intervention Engaging Youth in a Coach Like Way.
In July 2019, ACF launched YARH-3, which will continue to provide important information to the field by
supporting YARH Sites' evidence -building activities. YARH Sites may engage in a range of evidence -
building activities, with related deliverables, requirements, or milestones. YARH-3 will incorporate
evaluation TA; assessments of YARH Sites' readiness for summative evaluation; federally led evaluation
of at least one comprehensive service model, including an implementation study and an impact study;
and ongoing dissemination of knowledge gained through YARH-3 Site activities.
Mathematica is the prime YARH-3 contractor to OPRE, Building Capacity to Evaluate Interventions for
Youth/Young Adults with Child Welfare Involvement at Risk of Homelessness described in Contract
Number HHSP2332015000351/ 75P00119F37041, including any modifications or extensions thereof
(YARH-3). CPR will serve as a YARH Site for Colorado under the Building Capacity to Evaluate
Interventions for Youth/Young Adults at Risk of Homelessness.
Overview of the Project
The objective of the YARH-3 project is to assist OPRE in building evidence for the comprehensive service
models developed and initially implemented in YARH-2 by supporting YARH Sites and conducting a
federally led summative evaluation.
The YARH Site will complete work related to building evidence for the comprehensive service model they
developed and initially implemented in YARH-2, with modifications as suggested by YARH-2 work, and/or

documentation of readiness for a summative evaluation. CPR expects ESWC to work with CPR liaisons
throughout the course of the term of the Agreement. ESWC will begin their YARH-3 workin March 2021.
CPR expects activities to end no later than February 28, 2022 pursuant to this agreementand annual
renewal provisions. ESWC will work with CPR to complete the activities outlined in the Scope of Work
below in a timely manner where the parties agree to the following.
As a comparison site for the YARH-3 summative evaluation, ESWC commits that staff assigned to YARH-3 will perform
the following activities:
Training and Technical Assistance (TA) Activities
• Attend YARH 3 kick-off event.
• Attend evaluation trainings for comparison sites.
Implementation Activities
• Operate ESWC Chafee program as it is currently designed and implemented (i.e. 'business as
usual') ESWC agrees that it will not substantially change, alter or adjust its' Chafee program during
the course of ESWC participation in the YARH-3 summative evaluation. Notify and discuss with CPR
any contemplated changes and refrain from any changes that would disrupt the YARN -3
summative evaluation per CPR's recommendations.
Evaluation
• Participate on monthly CPR YARH-3 evaluation calls.
• Gain informed consent/assent for each youth to be enrolled in YARH-3.
• Enter quality data on a timely basis in Trails, and the Pathways Management Information
System (PMTS) to collect information not available through Trails and administrative
sources.
• Participate in other YARH-3 evaluation activities as requested.
Project Management
• Attend YARH-3 meetings and events as needed. All attempts will bemade to streamline
Pathways' meetings and the number of them.
Fees, Reimbursement and Invoicing
CPR will compensate ESWC in an amount that will not exceed a $10,000 site payment. ESWC will
invoice CPR on a quarterly basis in accordance with Attachment A.
