HomeMy WebLinkAbout20213150.tiffRESOLUTION RE: APPROVE APPLICATION AND MEMORANDUM FOR PERMANENCY INCENTIVE FUND WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with an Application and Memorandum for the Permanency Incentive Fund between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and the Colorado Department of Human Services, Office of Children, Youth, and Families, commencing October 26, 2021, and ending June 30, 2022, with further terms and conditions being as stated in said application and memorandum, and WHEREAS, after review, the Board deems it advisable to approve said application and memorandum, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Application and Memorandum for the Permanency Incentive Fund between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and the Colorado Department of Human Services, Office of Children, Youth, and Families, be, and hereby are, approved. Request Board Approval of the Department's Colorado Department of Human Services Permanency Incentive Fund Application SFY 2022. The Department of Human Services' Division of Child Welfare (CW) requests to apply for the Colorado Department of Human Services' (CDHS) Permanency Incentive Fund. CW has identified children and youth with families involved with child welfare Dependency and Neglect cases who have been placed with kinship and foster care caregivers where permanency is an option, however, permanency has not yet occurred. Permanency Assessments, conducted by providers with motivational interviewing and coaching skill sets, will be conducted with these children and families with the goal of determining the barriers to permanency and identifying avenues to address and mitigate these barriers so that permanency can be achieved. Additionally, in - home services may be provided for up to an additional four (4) hours, if the assessment determines that service is beneficial for the child and caregiver to support permanency. CW will request $7,500.00 for Permanency Assessments and $2,400.00 for in -home services, for a total budget of $9,900.00. If awarded, CW will have until June 30, 2022 to spend all funds. Permanency incentive services and support could include respite, transportation support for services, a coaching model that is developed to support families, 24/7 crisis and/or therapeutic support (including in -home), etc. These are interventions that may be considered using a regional or county department's specific approach. However, permanency has not occurred despite the Dependency and Neglect case moving forward to the point where permanency is possible for these children and youth. To respond to this, providers with motivational interviewing and coaching skillsets will be paired with children and youth placed with kinship care providers and foster care providers who have identified themselves as permanency options but have not moved forward with the steps necessary for permanency to occur. The provider will meet with the children, youth, and caregivers to collect information and thoroughly assess the family's functioning and dynamics, the child's/youth's behavioral, emotional, developmental, and interpersonal needs and strengths, natural supports, and the caregivers needs and resources. This assessment will occur with the goal of determining the barriers to permanency, and avenues for addressing and mitigating these barriers so that permanency can be achieved. The assessing provider will generate a report and make recommendations based on the assessment and this will be given to the child welfare caseworker and other stakeholders to the case. The child welfare caseworker will utilize the assessment report to implement recommendations and make necessary referrals to resolve barriers to permanency. The provider may offer some short-term, in home interventions, if the assessment determines that service is beneficial for the child and caregiver to support permanency. This service will be a "Permanency Assessment". Service(s) Timeline (ending no later than June 30, 2022) We currently contract with providers able to provide this type of service. We know of youth and families who would benefit from this service. If funding is received, referrals for this service could happen immediately with the expectation that services begin in the home within 48 hours to one week from the time of referral. Collecting information from the children, youth, and providers and assessing barriers and subsequent recommendations should require no more than 3 hours, depending on the size of the family and the number of children in the home with pending permanency. Writing the report and generating the recommendations should require no more than 2 hours. The referral for the assessment service will be authorized for up to 5 hours, with an end date that is 30 days from the first day of the referral. In -home services may be provided as indicated by the assessment for up to an additional 4 hours within 30 days to support permanency. Measurable Objectives and Data Collection (Qualitative and/or Quantitative) * Measurable Objectives: • The Permanency Assessment will provide new information and insight regarding the barriers to permanency for children and youth placed with kinship care or foster care providers, where permanency has been anticipated but not achieved. • The Permanency Assessment will provide recommendations that Weld County Department of Human Services and other stakeholders to the case can act on and/or implement, to mitigate barriers to permanency for these children and youth, including, but not limited to the provider attending Trust -Based Relational Intervention (TBRI) and other trauma informed training. • Permanency will be achieved for children and youth within 3 to 6 months of receiving a Permanency Assessment. • If permanency has not been achieved despite the implementation of recommendations per the Permanency Assessment, stakeholders to the case will have the data needed to support a change in placement and/or permanency plan. • Data collected from the Permanency Assessment project will be used to inform case management practices and permanency planning for subsequent cases. Data Collection: • Barriers to permanency due to the child's or youth's needs, including ongoing access to necessary services and supports. • Barriers to permanency due to dynamics within the family. • Barriers to permanency due to the needs of the caregiver(s). • Barriers to permanency due to the needs of other children or youth within the family's home environment. • Barriers to permanency due to financial concerns. • Barriers to permanency due to concerns brought forth by stakeholders to the case, such as the caseworker or the Guardian Ad Litem. • Barriers to permanency due to the judicial system. • Recommendations that require additional service referrals coordinated and funded by the Weld County Department of Human Services. • Recommendations that require additional service referrals accessed through systems of care and funding streams other than the Weld County Department of Human Services (such as Medicaid, the CCB, etc.). Recommendations that require referrals to community resources and supports. • Recommendations that require caseworker and/or Department advocacy for permanency. ® Length of time from receipt of the Permanency Assessment and correlating recommendations, to achievement of permanency for the child or youth. ® Number of children and youth with pending permanency on September 1, 2021. The providers will be selected based on their use of motivational interviewing, coaching, familiarity with community systems or care and resources, and familiarity with the child welfare system. Conversations have occurred with some of these providers and at least two have committed to serving in the needed role as assessor for the Permanency Assessment. Services Sustainability (if services are successful, how will the county sustain the services to pr mote permanency) If the service is successful, the Weld County Department of Human Services will contract with Core service providers for Permanency Assessments on an ongoing basis, as a County Designed Core Program. Permanency Sustainability (if services are successful, how will the county sustain the services to r.fmote permanency) The Permanency Assessment is a short-term, time -limited service and if successful, will provide insight, guidance, and recommendations that will be acted on and implemented by child welfare and the family, to achieve permanency. Sustainable solutions to address and mitigate barriers to permanency will be located within the family's community, natural supports, and social network. Prior to case closure, the caseworker will work with the family to identify and establish relationships with ongoing services and supports needed to sustain permanency, beyond specific interventions and supports needed to achieve permanency. These services and supports will be located and funded within systems of care and funding streams that the family can access when their case with Weld County Department of Human Services' Child Welfare Division is closed. The Joint Budget Committee authorized the Division of Child Welfare to provide counties with funding to use toward additional resources for supportive services to families moving toward permanency; building partnerships with nonprofits to provide intensive adoptive family recruitment for children/youth served in the child welfare system; and supporting families and children/youth through training, mental health services, and/or other relationship interventions. The Division of Child Welfare was given spending authority for up to $500,000 in federal Adoption Savings funds awarded to Colorado as a result of the permanency efforts of county departments. These dollars are to be accessed via the County fiscal Management System (CFMS), using the following accounting code: W515.15150 Your county's award is $9,900.
