1) This well shall be used in such a way as to cause no material injury to existing water rights. The issuance cif this p-ermit does not
ensure that no injury will occur to another vested water right or preclude another owner of a vested water right from seeking
relief in a civil court action_
2) The construction of this well shall be in compliance with the Water Well C.onstruction Rules 2 CC.R 402-2, unless approval of a
variance has been granted by the State Board of Examiners of Water well Construction and Pump Installation Contractors in
accordance with Rule 18.
3) Approved pursuant to C RS 37-9 2-4502 (3)(b)(I 1)0) as the only well on a residential site of 1 acre(s) described as lot 3, bloc k
filing 3, Aristocrat Ranchettes Subdivision, Weld County.
4) The use of ground water from this well is limited to ordinary household purposes inside one single family dwelling. The ground
water shall not be used for irrigation or other purposes.
5) The total depth of the well shall not exceed 2B0 feet, which corresponds to the base of the Lower Arapahoe aquifer. ,eta
minimum, plain easing shall be installed and grouted through all unconsolidated materials and shall extend a minimum of ten
feet into the bedrock formation to prevent production from other zones..
The pumping rate of this well shall not exceed 15 GPM.
7) The return flow from the use of this well must be through an individual waste water disposal system of the non -evaporative type
where the water is returned to the same stream system in wh k h the well is located.
13) This well shall be constructed not more than 200 feet from the location specified on this permit,
MOTE: To ensure a maximum productive life of this well, perforated casing should be set through the entire producing interval
of the approved zone or aquifer indicated above.
MOTE: This permit will expire on the expiration date unless the well is constructed by that date. A Well Construction and Yield
Estimate Report (G -WS -31) must be submitted to the Division of Water Resources to verify the well has been constructed,.An

TERMS AND CONDITIONS
A permit fee. as set by separate ordinance of the Board of Weld County Commissioners, shall be required of applicants for onslte wastewater treatment systems,
payabie at time of application. Permit fees are non-refundable; permit applications are non -transferable If both a building permit and an OVVTS are issued for tt
same property and construction is not commenced prior to the expiration date of the building permit, the OVVTS permit shall expire at the same time as the building
permit If an OVVTS permit is issued for a property that does not require a building permit, the OVVTS permit shall expire one year after its issuance if construction on
the septic system has not commenced. Any change in plans or specifications after the permit has been issued invalidates the permit unless approval is secured from
the Health Officer or hisMer designated agent. Expired permits can be renewed by payment of the permit fee only if:
A. There has been no change in the plans and specifications of the proposed system as set out in the original application or such change is reviewed and
approved by a Division Representative.
B. The surrounding land its use or zoning has not changed so -as -to cause the original application not to be acceptable under these regulations.
NOTICE
The completeness of this application is conditional upon further mandatory and additional tests and reports as may be required by the Weld County Department of
Public Health and Environment (WCDPHE). The Issuance of the permit is subject to such terms and conditions as deemed necessary to insure compliance with rules
and regulations adopted under article 10 title 25. CRS as amended. The applicant certfies that the proposed system will not be located within 400 feet of a communiti
sewage system. The undersigned certifies that all statements made, information and reports submitted herewith are. or will be, represented to be true and correct to
the best of hislher knowledge and are designed to be relied on by the WCDPHE for evaluation for purposes of issuing the permit applied for herein. Applicant further

Attn: Katie Sall, REHS, Environmental Health Specialist, Weld County Health
Attached is a signed & wet sealed soils percolation reports for the subject site. The
percolation report includes three profile test holes and percolation results. Doug Leafgren,
of Northern Colorado Geotech, prepared the report on January 23rd, 2017. Three test holes
were drilled on October 10th, 2016 for the percolation study of the leach field.
A plot plan is enclosed, showing the set backs for the proposed leach field (also shown on
page 2 of the Percolation Test report) are as follows:
• 85' from the north property line
• 53' from the south property line
• 43' from the north line of the 10' utility easement along the south property lot line
• 65' from the west property line
• 140' from the east property line
The setback for the leach field from the proposed water well location is going to be
approximately 106', which exceeds the State requirements of 100'. The septic tank will be
placed 60' from the water well, exceeding the County regulations by 10'. The leach field is
approximately 3 feet lower than the elevation of the home, providing positive sewer flow.
There are no other existing wells on the site, or natural water features such as ponds, retention
or detention ponds, lakes, streams, or any other surface waters or ditches that would require a
setback requirement.
The site is undeveloped and has never had any previous OWTS construction on the site. The
site is native and undeveloped. Atmos will serve the site with natural gas, serviced from the
west end of the front of the lot. The electric will be serviced from the southwest corner of the
site(directly behind the leach field), so it will take some additional wire to avoid the leach field
location. The electrical transformer is in a 10' utility easement along the south portion of the
lot and it will need to be placed around the leach field. The wire can be placed in the easement
get to the home.
The public right of way for this site is Casler Avenue. Casler Avenue is a paved public road,
maintained by Weld County. This public ROW will not affect the placement of the proposed
OWTS in any way. Casler is a minor arterial and the proposed OWTS far exceeds the minimum
building setbacks from a public ROW.
The site is marked with an address sign and the test holes have been indicated with a stake &.
ribbon.

SITE CONDITIONS
It is our understanding that the site will be used to construct a single-family residence that will have 4
bedrooms. The site is vegetated with native grasses and weeds. This site is bordered to the north by
Gasler Avenue. The site is bordered to the south, east and west by vacant lots. We are unaware of
an existing septic permit for the site or the presence of natural or cultural features at the site.
A total of three percolation test borings and one soil profile boring were drilled on October 10, 2016.
The percolation borings were drilled at depths ranging from of 3 to 5 feet and a soil profile test pit was
excavated to a depth of 8 feet. The borings were drilled at the locations show on the Site Plan, Figure
1. Soils in the area of the proposed absorption bed consisted of silty sand to a depth of 8 feet.
Summary boring logs are attached with this report.
Groundwater conditions were evaluated in each boring at the time of subsurface explorations, and
one day after the drilling. Groundwater, or the physical features present from past high groundwater
were not observed to a depth of 8 feet.
The borings were located in the field by pacing from property lines and/or existing site features.
Relative surface elevations at the boring locations were obtained by measurements with an engineer's
level from a temporary bench spark (TBM) shown on the Site Plan. The accuracy of the boring
locations and elevations should only be assumed to the level implied by the methods used to
determine each.

SEPTIC SYSTEM DESIGN SPECIFICATIONS
1 Septic Tank — Minimum = 4 Bedroom residence = 1,250 gallon tank. The tank will need to
meet the specifications of the Weld County Regulations 30-9-20. The lid of the tank must be
sufficient to withstand the load created by the burial depth of soil. We are not aware of water
soluble sulfate concentrations that would significantly affect a concrete septic tank. Risers must
be used to provide access to the tank.
2. Infiltrators — Minimum of 46 Standard Quick 4 infiltrators in an absorption bed (4 wide by 11 and
12 long = 46 Quick 4's for an absorption bed)
3. Distribution Box - Installed level to provide equal distribution of effluent. Riser must be used to
provide access to D -Box.
4. Backfill - 12 inches (minimum) to 36 inches (maximum) of on -site soil. The top 4 inches shall be
organic topsoil.
5. The system should maintain a minimum 1/4 -inch per foot slope from the house to the septic
tank. The slope of the effluent line from the tank to the absorption bed should be a minimum of
1/8 -inch per foot.
6. Each run of infiltrators must have an inspection port at the end of the run.
GENERAL NOTES
1. All construction shall be in compliance with Weld County Health Dept and State regulations,
2. Any deviation from the plan must be approved by WCHD and/or Northern Colorado Geotech
prior to construction.
3. No groundwater or bedrock was encountered in the soil profile boring located at the proposed
soil absorption bed. We are unaware of any limiting layers within S feet of the existing site
grade. Accordingly, the leach field may be constructed to a depth 4 feet below finished grade.
4. All parts of the field shall be located the minimum distances from land features including 100'
from any well, 50' from a pond or ditch, 20' from all dwellings and 10' from all property lines.
5. The system shall be inspected by WCHD and/or Northern Colorado Geotech during critical
phases of construction.
F. Positive drainage shall be provided to minimize surface runoff from entering the system.
7. The top of the field shall be seeded or sodded with low-water demand vegetation to control
erosion and aid evapo-transpiration. Large trees or shrubs should not be planted within 6 feet of
the field.
8. The completed leach field should not be paved or driven over at any time.
9. A licensed septic system installer should be chosen by the client. A list of licensed installers is
available from WCHD. The installer should use best construction practices during all phases of
the septic system construction.
10. Proper maintenance of the septic system will help lengthen effective life of the system.

INSTALLATION INSTRUCTIONS
1. Excavation should be performed in such a way as to limit/eliminate traffic over the area of the
absorption bed.
2. All wastewater lines shall be constructed of compatible pipe, primer, bonding agent and fittings.
3. Use Schedule 35 pipe except where piping is placed under driveways or roadways — in these
locations use Schedule 40 pipe.
4. All system piping shall be bedded with select, granular material. Material may be on -site or
imported material.
5. Compact soils around tank and distribution box to ensure piping will remain level and settlement
will not create bellies within the piping.
