From the application information supplied and the on -site soil percolation data the following minimum installation
specifications are required according to the Engineer's evaluation:

CONDITIONS:
Description: Permit Conditions
Comment: Approved based on Engineer evaluation. Construct onsite wastewater treatment system
according to Engineer design. Need an engineer approval letter approving system as
installed per engineer design. Engineer designed OWES must be inspected by the design
Engineer prior to backfilling. Onsite wastewater treatment system must be 10 feet from
property line. Install the soil treatment area within 50 feet of the soils test location. Keep
vehicle traffic off of onsite wastewater treatment system. All easements shall be verified prior
to installation of septic system. All Weld County regulations are applicable.
Need to submit EPA Class V Injection Well questionnaire to EPA for Onsite Wastewater
Treatment Systems that will service more than 20 persons per day.

This permit Is granted temporarily to slow constructlQn to commence. This permit may be revoked or suspended by the Weld County Department of
Public Health and Environment for reason$ set forth in the Weld Dourly Onsite Wastewater Trealmeni System Regulations including failure to meet
any term or condition imposed thereon during temporary or final approval. The issuance of this peemitdoes not constitute assumption by the
department or its employees of liability Tar the faiure or inadequacy of the sewage disposal system. This hermit is nowtransferabla and
non-refundable. Before issuing final approval of this permit the word County Department of Public Health and environment reserves the right 10
impose additional terms and cortdiltons required to meet our regulalions on a continuing basis. Final permit approval is contingent upon the final
inspection of the completed system by the Weld Counly Department of Public Health and Environment.
