RE: APPROVE LOCAL PUBLIC HEALTH AGENCY APPLICATION AND ACCEPTANCE
OF GRANT AWARD FOR COVID-19 VACCINATION SUPPORT FUND, AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Local Public Health Agency Application
and a Request for Acceptance of the Grant Award for the COVID-19 Vaccination Support Fund
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Public Health and Environment,
and Northeast Health Partners, LLC, with further terms and conditions being as stated in said
application and acceptance letter, and
WHEREAS, after review, the Board deems it advisable to approve said application and
acceptance letter, copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Local Public Health Agency Application and the Request for
Acceptance of the Grant Award for the COVID-19 Vaccination Support Fund between the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Department of Public Health and Environment, and Northeast Health Partners,
LLC, be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said acceptance letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 23rd day of June, A.D., 2021.

Health Agency COVID-19 Vaccination
Support Award
For the Board's approval is a request to accept a one-time grant from Northeast Health Partners
LLC (NHP) for the use and benefit of the Weld County Department of Public Health and
Environment (WCDPHE).
The WCDPHE is committed to mitigating the disparity experienced by racial and ethnic minority
communities by taking additional measures to outreach and assist Medicaid members of color
and Medicaid members who may be homebound and unable to travel outside of the home to
received their COVID vaccine. As a member of Northeast Health Partners, our Regional
Accountable Entity (RAE), Region 2, WCDPHE is being offered a one-time grant of $5,000 to
support these activities. This $5,000 grant award will assist the WCDPHE and Weld County to
provide COVID vaccine education and address vaccine hesitancy that benefit our Medicaid
members and our community.
Activities will be conducted by current staff; no additional FTE is being requested.
Assistant Weld County Attorney, Karin McDougal, has reviewed this grant award and had no
concerns.
The Board approved placement of this grant award on the Board's agenda via pass -around dated
June 11, 2021.
I recommend acceptance of this Vaccination Support Award from Northeast Health Partners
LLC.

Brief description of the problem/issue:

Approving this grant will provide WCDPHE with additional funding to enhance our outreach and educational
COVID vaccination efforts to our Medicaid members of color and those that are home -bound, as well as other
similar populations within Weld County.
Declining this grant will results in the loss of this additional funding opportunity that will assist the WCDPHE
with COVID vaccination education and outreach efforts.
Recommendation: I recommend acceptance of this Grant Award from Northeast Health Partners LLC.

Who is Northeast Health Partners (or NHP)?
In Colorado, Medicaid is also called Health First Colorado. Every Health First
Colorado member belongs to a regional organization that manages their
physical and behavioral health care. Northeast Health Partners is the
Regional Accountable Entity (or RAE) for Region 2 which includes Medicaid
members attributed to primary care providers in Cheyenne, Kit Carson,
Lincoln, Logan, Morgan, Phillips, Sedgwick, Washington, Weld, and Yuma
counties.

Provide a brief description of the work you are doing related to COVID-19
vaccinations within your community.

The Weld County Department of Public Health and Environment is an enrolled COVID-19 Vaccine
Provider, and as such we have been vaccinating the public in accordance with CDPHE requirements
and the State's vaccine distribution plan. We started with internal vaccination clinics within our
building with a throughput of approximately eighty first dose vaccinations a day, Monday through
Friday, back in December of 2020. We also recruited and were successful in getting forty other
COVID-19 vaccine providers enrolled with CDPHE. We meet weekly with our providers to facilitate a
local support structure for them and provide updates from CDPHE and other technical assistance as
needed. Starting at the end of March, as vaccine supplies started increasing significantly, we
transitioned to Community Vaccination Sites (CVS) four days per week with a throughput of 3,300
vaccinations per week. Our CVS locations are in Evans (three days per week) and in Firestone on
Wednesdays.

the distribution of the COVID-19 vaccination.

We have faced many challenges during this unprecedented pandemic. LPHAs in Colorado did not
have the opportunity to review or provide input into the Colorado Vaccine Distribution plan before it
was rolled -out to the public. Many times, we learned about updates and/or changes to the
distribution plan during the Governor's press conferences, along with the general public. Additionally,
Weld County received the second lowest allocation of vaccine in Colorado when adjusted for our
population as a percentage of Colorado's population. We had to bring this to the State's attention
and have only recently started receiving increased allocations of vaccine from CDPHE. Many of our
providers are also not receiving enough vaccine to meet their vaccination capacities. As many of the
providers we recruited were selected due to their location (near or within poverty and/or minority
census tracts), the inadequate allocation of vaccine has been and continues to be an equity
distribution challenge.
Provide a description of how Northeast Health Partners' financial support
will assist your organization to increase vaccine accessibility and ensure
equitable vaccine distribution, especially for homebound members and
members of color.

Currently, the percentage of our Hispanic population that has received at least one dose is 20.9%, The
Hispanic population in Weld County is estimated to be around 100,000. This presents an opportunity
for improvement for us as a County. At our current rate of vaccination for this specific population, we
will not reach our goal for 60% of this population to be vaccinated until early December 2021 per our
modeling. This is unacceptable. These funds, should they be awarded to the Weld County
Department of Public Health and Environment, will be part of the resource allocation that we are
making to conduct vaccine outreach to more people in our poverty and minority census tracts, and
also reach -out to our homebound populations. We are currently working with community champions
to make inroads with our vaccination efforts, and we are working collaboratively with the Area
Agency on Aging and the County's Human Services Department to find vaccine solutions for our
homebound populations.
