Home My WebLink About20213580.tiffRESOLUTION RE: APPROVE CARE COORDINATION AND COMMUNICATION AGREEMENT AND AUTHORIZE CHAIR TO SIGN - NORTH RANGE BEHAVIORAL HEALTH WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Care Coordination and Communication Agreement between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and North Range Behavioral Health, commencing upon full execution of signatures, and ending December 28, 2026, with further terms and conditions being as stated in said agreement, and WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Care Coordination and Communication Agreement between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and North Range Behavioral Health, be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair, be, and hereby is, authorized to sign said agreement. Request Board Approval of the Department's Care Coordination and Communication Agreement with North Range Behavioral Health (NRBH). The purpose of this agreement is to ensure effective communication and coordination between agencies at the local level for the care of mutual clients and their families. This agreement is a stipulation of the contract between the Mental Health Centers and the Office of Behavioral Health. There will be no monetary exchange associated with this agreement which will commence upon the date of final signature and will be in effect no more than five years. The Department has no financial obligations under this Agreement. This five (5) year Agreement will expire in November of 2026. I do not recommend a Work Session.
