Extraction plans to assigned and convey a Wellbore Only interest in the Lease to SRR Rockies, LLC ("Slur). Under the terms of the Lease, your consent is required to assign and convey any interests in and to the Lease. Extraction hereby respectfully requests your consent to assign and convey any interests in the Lease from Extraction to SRR by executing this Consent Notice in the space provided below.
