RE: APPROVE AMENDMENT #1 TO INDIVIDUAL PROVIDER CONTRACT FOR
PURCHASE OF FOSTER CARE SERVICES IN A FOSTER CARE HOME AND
AUTHORIZE CHAIR TO SIGN - ROGER AND ALLISON SEDLACEK
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Amendment #1 to the Individual Provider
Contract for Purchase of Foster Care Services in a Foster Care Home between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Department of Human Services, and Roger and Allison Sedlacek, commencing
July 1, 2022, and ending November 18, 2022, with further terms and conditions being as stated
in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Amendment #1 to the Individual Provider Contract for Purchase of
Foster Care Services in a Foster Care Home between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, and Roger and Allison Sedlacek, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 31st day of August, A.D., 2022, nunc pro tunc July 1, 2022.

AGREEMENT AMENDMENT BETWEEN
THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES
AND
ROGER SEDLACEK AND ALLISON SEDLACEK

This Agreement Amendment, made and entered into 3-I day of F u S 2022 by and
between the Board of Weld County Commissioners, on behalf of the Weld County Dep ent of Human
Services, hereinafter referred to as the "Department", and Roger Sedlacek and Allison Sedlacek, hereinafter referred
to as the "Contractor".
WHEREAS the parties entered into an Agreement for Individual Provider Contract for Purchase of
Foster Care Services in a Foster Care Home, (the "Original Agreement") identified by the Weld County Clerk to
the Board of County Commissioners as document No.2021-3588, approved on December 29, 2021.
WHEREAS the parties hereby agree to amend the term of the Original Agreement in accordance with
the terms of the Original Agreement and any previously adopted amendment, which is incorporated by reference
herein, as well as the terms provided herein.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as follows:
• The Original Agreement will end on November 18, 2022.
• These Amendments, together with the Original Agreement, constitutes the entire understanding
between the parties. The following additional changes are hereby made to the current Agreement
effective July 1, 2022:
1. Exhibit A, Paragraph 19, is hereby amended as follows:
County agrees to purchase, and Provider agrees to provide the care and services, which are listed in
this Agreement. The State approved rate for Child Maintenance/Respite will be paid for any child
placed with the Provider unless the Medically Fragile Tool has been completed due to intensive
medical/physical needs, or limitations that interfere with daily life functioning. The Medically Fragile
Tool will be completed every six months as long as the child remains in care with the Provider. The
rate will be adjusted if the Medically Fragile Tool indicates a change in the level of care. The
Medically Fragile Tool will determine the reimbursement rate as specified in Addendum 1.
2. Addendum 1, Medically Fragile Tool Rates, is hereby amended as attached.
All other terms and conditions of the Original Agreement remain unchanged.
IN WITNESS WHEREOF, the parties hereto have duly executed the Agreement as of the day, month, and
year first above written.
